Abstract A total of 656 pigs (623 live-born and 33 stillborn) from 43 sows were used to evaluate hematological criteria at birth and their association with piglet survival. At birth of each piglet, birth time and order within the litter, weight, umbilical cord status (intact or ruptured) and whether the pig was live-born or stillborn were recorded. A 200µL sample of blood from the umbilical cord was collected and immediately analyzed for concentrations of glucose, oxygen partial pressure (pO2), carbon dioxide partial pressure (pCO2), pH, base excess (BE), bicarbonate (HCO3), saturated oxygen (sO2), total carbon dioxide (TCO2), sodium, potassium, ionized calcium (iCa), hematocrit (Hct), and hemoglobin (Hb) on a hand held iStat portable clinical analyzer (iStat Alinity, Abbott Point of Care Inc., Princeton, NJ). Piglets were categorized into quartiles based on birth order and cumulative birth interval (CumBI). Live-born pigs had higher (P < 0.01) umbilical cord blood pH, HCO3, BE, sO2, TCO2, and birth weight compared with stillborn pigs, but lower (P < 0.01) pCO2, K, iCa, and glucose compared with stillborn pigs. Pigs with intact umbilical cords at birth were associated with higher (P < 0.01) blood pH, HCO3, BE, and TCO2 compared with piglets born with a ruptured umbilical cord. Pigs with intact umbilical cords were associated with lower (P < 0.01) Hct and Hb concentrations and born earlier (P < 0.01) in the birth order compared with pigs born with a ruptured umbilical cord. Pigs that did not survive to weaning had lower (P < 0.01) umbilical cord blood pH, HCO3, BE, sO2, TCO2, Na, glucose, and birth weight, and 24 hr weight compared with pigs alive at weaning. Pigs born in the first quartile for CumBI had higher (P <0.05) pH compared with pigs in the other three quartiles. Umbilical cord blood HCO3, BE, and TCO2 decreased (P <0.05) with each change in CumBI quartile from first to last. Blood glucose was lowest (P <0.05) in pigs born before 44 min and highest in pigs born after 164 min. Umbilical cord blood pH, HCO3, BE, TCO2, Na, glucose, Hct, and Hb were positively associated (P <0.001) with colostrum intake, indicating increased blood values resulted in higher colostrum intake. Although a pig may be live-born, their survival to 24 hr and to weaning is reduced when blood pH, HCO3, BE, and sO2 are lower reiterating the importance of management practices that can reduce the birth interval between pigs and the number of pigs experiencing moderate to severe hypoxia.

Abstract Iron stores at birth are essential to meet iron needs during the first 4–6 months of life. The present study aimed to investigate iron stores in normal birth weight, healthy, term neonates. Umbilical cord blood samples were collected from apparently normal singleton vaginal deliveries (n=854). Subjects were screened and excluded if C-reactive protein (CRP) > 5 mg/l or α1-acid glycoprotein (AGP) > 1 g/l, preterm (<37 complete weeks), term < 2500g or term > 4000g. In total, 762 samples were included in the study. Serum ferritin, soluble transferrin receptor (sTfR), hepcidin, and erythropoietin (EPO) were measured in umbilical cord blood samples; total body iron (TBI) (mg/kg) was calculated using sTfR and ferritin concentrations. A total of 19.8% newborns were iron deficient (ferritin 35 μg/l) and an additional 46.6% had insufficient iron stores (ferritin < 76 μg/l). There was a positive association between serum ferritin and sTfR, hepcidin, and EPO. Gestational age was positively associated with ferritin, sTfR, EPO, and hepcidin. In conclusion, we demonstrate a high prevalence of insufficient iron stores in a Chinese birth cohort. The value of cord sTfR and TBI in the assessment of iron status in the newborn is questionable, and reference ranges need to be established.

Background: Congenital cytomegalovirus infection is currently the leading cause of congenital infection in 0.2-2.2% of live births worldwide leading to variable serious sequalae. The aim of the study was to determine if low birth weight is an indicator of CMV congenital infection evidenced by detecting CMV-DNA in umbilical cord blood at the time of delivery. Methodology: CMV-IgG and IgM antibodies and CMV-DNAemia were assessed in umbilical cord blood of two hundreds newborns, one hundred of whom had birth weight ≤ 2700 gram and/or head circumference ≤ 32 cm. Results: CMV-IgM was not detected, while CMV-IgG was positive in 80-90% of the two hundreds tested newborns. CMV-DNA was detected in four out of the 200 newborns. One of them was over the adopted weight limit (> 2700 gram). Conclusions: CMV-IgM and IgG antibodies assessment was not a potential discriminative test to identify congenitally infected newborns. In addition, low birth weight and small head circumference at birth failed to predict congenital CMV infection. CMV-DNA detection in umbilical cord blood at the time of delivery using real-time PCR of all newborns is recommended as decisive, rapid and non-invasive test.

Worldwide neonatal sepsis is among the most frequent causes of neonatal death. Various studies have tried to establish the relationship between prevalence of neonatal septicemia risk factors and bacteriological profiling, low birth weight, prematurity, etc. Current study was aimed to compare early onset of neonatal sepsis (EONS) among primigravida and multigravida mothers using umbilical cord blood (UCB) and peripheral venous blood (PVB) samples. It was also aimed to establish the utilization of umbilical cord blood culture (UCBC) in comparison to peripheral venous blood culture (PVBC) in identifying EONS. In present study the blood samples were collected from high risk neonates for the clinical blood culture and screening. Among the 75 neonates in the study, 24 (32.0%) were observed to have sepsis screen positive. Study of high risk neonates umbilical cord blood culture (UCBC) positivity was 17.3% while Peripheral Venous blood culture positivity was 5.3%. Moreover,  in this study all risk factors like Prematurity, Low birth weights, Premature rupture of membrane, and birth asphyxia were significantly (p<0.05) associated with UCBC growth/positivity. Low birth weight (86%) was mostly reported in the high risk neonates with other associated sepsis factors. Similarly maternal fever and prolonged rupture of membrane was highly significantly (p<0.01) associated with UCBC positivity. Gram negative bacterias  were  more  commonly found,  such as Pseudomonas (5.3%), followed by E. coli (4%), and Klebsiella (2.7%) and gram positive Streptococcus sp. (2.7%), etc. From our analysis it can be said that the UCBC has strong diagnostic outcomes as compared to the PVBC for etiological evaluation of bacterial sepsis in neonates at high risk.
