Transdermal extraction of clinically relevant analytes offers a potentially noninvasive method of diagnostics. However, development of such a method is limited by the low permeability of skin. In this paper, we present a potential method for noninvasive diagnostics based on ultrasonic skin permeabilization and subsequent extraction of interstitial fluid (ISF) across the skin using vacuum. ISF extracted by this method was collected and analyzed for glucose and other analytes. Glucose concentration in the extracted fluid correlates well with blood glucose concentration over a range of 50–250 mg/dl. A mathematical model describing vacuum-induced transport of ISF through ultrasonically permeabilized skin is presented as well. The model accounts for convective, as well as diffusive, transport processes across blood capillaries, epidermis, and the stratum corneum. The overall predictions of the model compare favorably with the experimental observations.

Measurements of the glucose concentration in venous blood and parotid saliva taken from 31 diabetics attending a diabetic clinic showed values ranging respectively from 3.9 to 19.1 mmol/l and 0.06 to 0.83 mmol/l (means 9.6 mmol/l and 0.32 mmol/l respectively). Linear regression of salivary glucose on blood glucose gave a simple correlation coefficient of 0.18 (NS). Since salivary glucose levels did not reflect blood glucose levels, the possibility of diabetics regulating their metabolic control by the noninvasive technique of monitoring salivary glucose concentrations is not possible.

Objective: Today, there are various non-invasive techniques available for the determination of blood glucose levels. In this study, the level of blood glucose was determined by developing a new device using near-infrared (NIR) wavelength, glass optical waveguide, and the phenomenon of evanescent waves. Materials and Methods: The body's interstitial fluid has made possible the development of new technology to measure the blood glucose. As a result of contacting the fingertip with the body of the borehole rod, where electromagnetic waves are reflected inside, evanescent waves penetrate from the borehole into the skin and are absorbed by the interstitial fluid. The electromagnetic wave rate absorption at the end of the borehole rod is investigated using a detection photodetector, and its relationship to the people's actual blood glucose level. Following precise optimization and design of the glucose monitoring device, a statistical population of 100 participants with a maximum blood glucose concentration of 200 mg/dL was chosen. Before measurements, participants put their index finger for 30 seconds on the device. Results: According to this experimental study, the values measured by the innovative device with Clark grid analysis were clinically acceptable in scales A and B. The Adjusted Coefficient of Determination of the data was estimated to be 0.9064. Conclusion: For future investigations, researchers are recommended to work with a larger statistical population and use error reduction trends to improve the accuracy and expand the range of measurements.

Glycaemic control, in particular at postprandial period, has a key role in prevention of different diseases, including diabetes and cardiovascular events. Previous studies suggest that postprandial high blood glucose levels (BGL) can lead to an oxidative stress status, which is associated with metabolic alterations. Cinnamon powder has demonstrated a beneficial effect on postprandial glucose homeostasis in animals and human models. The purpose of this study is to investigate the effect of cinnamon tea (C. burmannii) on postprandial capillary blood glucose level on nondiabetic adults. Participants were given oral glucose tolerance test either with or without cinnamon tea in a randomized clinical trial. The data revealed that cinnamon tea administration slightly decreased postprandial BGL. Cinnamon tea ingestion also results in a significantly lower postprandial maximum glucose concentration and variation of maximum glucose concentration (p< 0.05). Chemical analysis showed that cinnamon tea has a high antioxidant capacity, which may be due to its polyphenol content. The present study provides evidence that cinnamon tea, obtained fromC. burmannii, could be beneficial for controlling glucose metabolism in nondiabetic adults during postprandial period.

Diabetes Mellitus (DM) is a systemic disorder characterized by hyperglycemia either due to insulin resistance or insulin deficiency. This can lead to many serious life-threatening complications if not managed properly by regular monitoring of glycemic status. Prevalence of fear of needles in the society make people non-compliant to regular monitoring. Thus, there is a need for a non-invasive method for determining the glycemic status of the individual. Salivary Glucose has the potential to be one such tool. This study aimed to find whether a correlation between fasting blood glucose levels and fasting salivary glucose levels could be established in diabetic and non-diabetic individuals. 50 patients with DM and 50 patients without DM were studied. 5 ml of venous blood and 5 ml of unstimulated saliva after overnight fasting were collected from each participant and processed using standardized enzymatic methods. The data was analyzed using SPSS software. There was a strong and very significant positive correlation (r=0.800, p=0.001) between fasting salivary glucose levels and fasting blood glucose levels in patients with DM whereas the correlation was weak and insignificant in patients without DM (r=0.111, p=0.441). The cut off value for diagnosing DM was found to be ˃ 2.2mg/dl with 100% specificity and 100% sensitivity.

Background: Hypoglycemia is the most common event of failure of metabolic adjustments in the newborn. Changes in maternal and fetal monitoring techniques, administration of glucose-containing solutions during labor, delivery and early feeding in neonates significantly alter blood glucose concentrations during the first week of postnatal life. Subjects and Methods: A total of 90 healthy (60 born by FTND, 30 born by LSCS) term, AGA infants were longitudinally evaluated at birth, at one hour after feeds (post feed), and after 6 hours of life. Plasma glucose was estimated from Heel Prick capillary samples by glucometer method. The influence of mode of delivery, the interval between feeds, sex, birth weight, on blood glucose was analyzed. Results: The way of delivery did not affect the plasma glucose concentration in neonates. There was a substantial increase in blood glucose concentration after the first feed irrespective of their birth weight. It was found that female babies had a higher blood glucose concentration than male babies during our study period. All babies maintained normal blood glucose with the continuation of breastfeeding. Conclusion: Plasma glucose levels are satisfactorily maintained in healthy term infants without resort to pre-lacteal feeds and mode of delivery did not influence plasma glucose. There is no need to check blood glucose levels routinely in an asymptomatic, healthy, term, breastfed infants.

AbstractBackgroundRapid blood glucose estimation is required to prescribe treatments and to make dose adjustments in diabetic patients. However, measuring plasma glucose levels is time consuming. Therefore, the use of glucometers has greatly increased.ObjectivesTo measure the correlation between capillary and venous blood glucose levels.MethodsSeventy patients with type 2 diabetes mellitus (T2DM) were enrolled in the present study and informed written consent was obtained from all participants. Demographic characteristics and clinical information was noted. Capillary and venous glucose levels were determined. Statistical Package for Social Sciences version 21 was used for data analysis.ResultsMean age of patients was 52 ± 12 years. It included 29% men; 71% women; 9% smokers; and 90% poor. Mean venous glucose was 11.73 ± 4.64 mmol/L and mean capillary glucose 12.57 ± 5.21 mmol/L. These findings demonstrated a significant intermethod mean difference of 0.84 mmol/L (P < 0.001). Inter-glucose difference was not significant at glucose levels near normal. However, it increased gradually with rise in glucose measurements and was significant at elevated glucose levels. Both concordance correlation coefficient and intraclass correlation coefficient demonstrated positive correlation and more consistency between glucose estimations. A Bland and Altman plot presented excellent agreement between glucometrically and photometrically determined glucose levels.ConclusionA positive correlation coefficient showed strong association between capillary and venous glucose measurements.
