Why Families Matter

Families are valuable for many reasons, but here the focus is on two: their function of creating and maintaining the identities of those within them, and their function of caring. These give rise to an ethos that is common in structures of intimacy but not shared by healthcare providers. The tension between the two systems of ethics creates problems unless healthcare providers understand the difference.

Abstrak. Pada penelitian ini, telah dilakukan analisa karakterisasi pada teknologi Direct Sequence Spread Spectrum dan Frequency Hopping Spread Spectrum, sebagai salah satu teknik multiple-access pada sistem komunikasi. Karakterisasi dilakukan untuk mencari bagaimana cara meningkatkan keoptimalan kedua sistem tersebut, dalam mengatasi masalah interferensi dengan sistem dan channel yang sama. Dan juga untuk menentukan veriabel apa yang mempengaruhi keoptimalan kedua sistem tersebut. Karakterisasi dilakukan dengan menentukan variabel-variabel yang mempengaruhi keoptimalan keduanya. Hasil dari karakterisasi, diketahui variabel-variabel yang mempengaruhi kemampuan sistem DSSS yaitu nilai frekuensi spreading (). Sedangkan untuk sistem FHSS yaitu nilai frekuensi spreading ( dan ) dan selisih antara frekuensi hopping data dengan frekuensi hopping interferensi . Kata Kunci: BER, DSSS, FHSS, Interference, Spread spectrum. Abstract. In this study, characterization of Direct Sequence Spread Spectrum and Frequency Hopping Spread Spectrum technologies have been done, as one of the multiple-access techniques in communication systems. Characterization is done to find out how to improve the ability of the two systems, in solving interference problems with the same system and channel. And also to determine what veriabel affects the ability of the two systems. Characterization is done by determining the variables that affect the ability of both. The results of the characterization, known variables that affect the ability of the DSSS system are the spreading frequency value (). As for the FHSS system, the spreading frequency value ( and ) and the difference between frequency hopping data with frequency hopping interference .

An active in vitro protein-synthesizing system has been developed from Rhodospirillum rubrum grown under either photosynthetic or heterotrophic conditions. A protease activity has been found in both of these systems, and this activity can be readily inactivated by treating the cells with KCl and phenylmethyl sulfonylfluoride. The difference in protein-synthesizing activity between the photosynthetic and the heterotrophic systems has been tested in regard to the requirement of various chemicals and the response to protein synthesis inhibitors or various chemical compounds. It has been concluded that only minor differences in protein-synthesizing activity exist between these two systems.

Background Parents of children born preterm with a disability often experience profound psychological distress with transition from neonatal to rehabilitation services. Four interventions were found effective to support parents throughout this critical transition period whereby parental stress can threaten the child’s development. Purpose To examine parents’ perceptions of the acceptability of four evidence-based interventions to support their transition. Methods A quantitative design using survey methods was employed with 24 parents with experience in transition to rehabilitation services. Each participant rated the acceptability of the interventions using the Treatment Perception and Preference scale. Descriptive statistics and repeated measures analysis of variance were used for data analysis. Results Mean overall acceptability scores differed across the four interventions ( p = .042); the difference was of moderate size (η2 = .11). Parents perceived psychoeducation and narrative therapy as most acceptable, followed by website consultation with healthcare providers and parent support program, then parent self-help program. Conclusions Psychoeducation and narrative therapy should be accessible to parents experiencing transition from neonatal to rehabilitation services.

A system of registration of title to land was first introduced int England by the Land Transfer Act 1862. Many commentators have since charted its development and have described the struggle between those espousing title registration and those advocating a reformed system of unregistered conveyancing. There has, however, been little discussion of an alternative which was strongly advocated in the nineteenth century; a general register of deeds, rather than title. The article explains the difference between the two systems and describes the various proposals for the establishment of a deeds register. It then analyses the relative strengths and weaknesses of the two systems of registration and suggests that the eventual ascendancy of title registration was due less to any inherent superiority than to external factors.

The reliability importance of one or more components when another component is assumed to be working/non-working is measured by Conditional Marginal Reliability Importance (CMRI) and Conditional Joint Reliability Importance (CJRI) respectively. We consider two systems viz the series-in-parallel and series-parallel. The expressions for CMRI and CJRI are derived for both the systems when the components are independent but not identically distributed. It is shown that the sign of the joint importance of three components and Conditional Joint Importance (CJI) can be determined using Schur-convexity (concavity) of the reliability function. The difference in the reliability functions of two coherent systems with n ≥ 3 statistically independent and with dependent components is derived. It is shown to be measured by their covariance, the JRI and the CJRIs. CMRIs and CJRIs of a phased type electronic system and a bridge structure are worked out.

Abstract. The integration of Geographic Information System (GIS) and the Building Information Modeling (BIM) referred to as the merging of the two systems for the purpose of data interoperability. The need to share information between the two systems is what motivated the integration process purposely for geospatial analysis. This can be achieved through their data exchange formats such as; City Markup Language (CityGML) and Industry Foundation Classes (IFC). The formats are the two most prominent key schemas of GIS and BIM systems respectively. The integration is a step towards information exchange or sharing (data interoperability) between the two systems. The selection of the two most prominent data exchange formats is as a result of their widespread applications in the GIS and BIM domains. However, the differences in geometric and the semantics information hinders data interoperability (information sharing) between GIS and BIM. Also, coupled with the difference in schema structure and the level of information richness between IFC and CityGML. This paper, propose a geometry transformation process that can be used to extract and transform IFC building objects to that of CityGML building objects to enable 3D model design and constructed using BIM tool to be easily reused in 3D GIS applications which will be able to support the CityGML model format. Where the geometric information will be extracted using the IFC tree-structure (hierarchy) and transformed to destination CityGML.

Aims: To evaluate the Russian Federation healthcare providers’ websites compliance to legal requirements on availability of patient-oriented medicines supply information and compare the evaluation results between public and private healthcare facilities. Study Design: Cross-sectional study. Place and Duration of Study: The evaluation of compliance to legal requirements to medicines supply information on public and private healthcare providers’ websites available on the Internet was conducted in September 2021. Methodology: The study included a simple random sample of 66 websites of Russian healthcare providers containing two groups:  public (n=33) and private (n=33) healthcare facilities’ websites. The compliance evaluation was performed by checking the availability of 4 medicines lists on the websites: (1) essential medicines list; (2) list of medicines for the most expensive chronic diseases to treat; (3) list of medicines that are prescribed only by shared decision of healthcare facility medical commission; (4) list of medicines that are dispensed for certain social groups with no charge or with 50% discount in outpatient care settings. Results: The difference of availability of the first list was 90.9% (95% CI 77.7%–97.4%) vs. 33.3% (95% CI 19.2%–50.3%) on public and private healthcare facilities’ website, respectively, P<.001. The difference of that in the second list was 42.4% (95% CI 26.8%–59.3%) vs. 15.2% (95% CI 6.0%–30.1%), P<.028. For the third no statistical significance was revealed (P>.05). For the fourth list the difference was 66.7% (95% CI 49.7%–80.8%) vs. 21.2% (95% CI 10.0%–37.2%), P<.001. Conclusion: It is required to improve the supervision approaches for both public and private healthcare facilities for better patient-oriented medicines supply information provision. The problem may be solved by the implementation of a centralized government policy repository with regularly updated lists, requirements, and best practices.

Abstract Objective The objective of this study was to review the utilization of a standardized tracheostomy handoff sheet to reduce tracheostomy-related complications. Methods A retrospective chart review was carried out of patients who underwent tracheostomy and had a dedicated tracheostomy handoff sheet utilized in the postoperative period. The handoff sheet contains trach size, backup trach size, airway findings, suction depth, location of maturation suture, location of stay sutures, and photograph of the wound bed on postoperative day 0. An anonymous survey of healthcare providers participating in the patient's care was undertaken regarding the utility of the dedicated trach handoff sheet. Results There were a total of 18 completed questionnaires. The majority of respondents felt the trach handoff sheet was helpful and provided vital information (4.61, scale 1–5). Hundred percent of respondents referenced the handoff sheet at some point during the hospital stay; 83% of respondents (15/18) felt the handoff sheet was more productive than a verbal handoff and 67% of respondents (12/18) preferred the sheet be available bedside rather than a digital format for reference in the electronic medical record. 89% of respondents (16/18) utilized the handoff sheet for wound care. Conclusion There are numerous aspects to proper tracheostomy care. From the immediate postoperative time period to several years out, proper care can make the difference for quality of life, respiratory standpoint, and airway safety. With a daily nursing handoff that at times can become redundant, utilization of a standardized tracheostomy handoff can lead to improve outcomes. These handoff sheets provide information deemed essential to optimal patient care and are always available bedside for review. Standardization and utilization of these tracheostomy sheets have led to decreased airway complications in emergency situations, reduced wound breakdown, and increased safety and efficiency in handoffs.

As in Australia school education is the responsibility of State Governments, this article will consider two computer systems in the Australian State of Victoria. The article takes a socio-technical stance to examine two computer systems currently in use in schools in Victoria: CASES21 and the Ultranet. After describing these systems, the article makes use of actor-network theory to explore the actors involved in their creation, development, implementation and use (or in one case non-use), and the networks they established in doing so. It looks at the associations involving both the human and non-human actors and how these contributed to successful adoption and use of these systems. A comparison of two systems within the same organisational environment allows a unique perspective on the formation of networks. The ANT approach permits an understanding of the difference in adoption where very few factors differ between the cases.

We assess the association between survey layout and response rates (RRs) in the 2017 Medicare Advantage Consumer Assessment of Healthcare Providers and Systems mail survey. Among 438 Medicare Advantage plans surveyed by six vendors, there was latitude in survey layout, and plans could add up to 12 supplemental items. Regression models predicted survey response from survey characteristics (page count, number of supplemental items, and survey attractiveness), and beneficiary sociodemographics. Beneficiary-age-by-survey-characteristic interactions assessed whether survey characteristics were more strongly related to RRs among older beneficiaries. We found that surveys with more supplemental items and less attractive layouts had lower adjusted odds of response. RRs were more sensitive to format among older beneficiaries. The difference in adjusted RRs for the most favorable versus the least favorable survey design was 14.5%. For a 65-year-old, this difference was 13.6%; for an 80-year-old, it was 21.0%. These findings suggest that even within a relatively standardized survey, formatting can substantially influence RRs.
