Substance misuse/dual diagnosis

Dual diagnosis is used to refer to people with a psychotic illness plus harmful or hazardous substance misuse, which includes alcohol and any legal or illegal drugs. These coexisting morbidities present challenges for the delivery of services which have traditionally specialized in one or the other in the form of different approaches in addictions and community outreach services. The chapter provides data on the high incidence, poorer outcomes, associated risks, and high service use of these dual diagnosis patients. Best practice in service responses and clinical assessment and interventions are presented alongside a case study and care planning documentation.

Abstract On October 20, 1937 the Tank Barge ARGO sank during a fall storm on western Lake Erie. The barge was reported to be carrying 100,000 gallons of crude oil and 100,000 gallons of benzol and was thought to have sunk in Canadian waters. The ARGO was identified as a potential polluting wreck in NOAA’s Remediation of Underwater Legacy Environmental Threat (RULET) project and ranked as the greatest legacy underwater environmental threat on the Great Lakes. However, the exact location, condition, and disposition of the sunken barge and its cargo were a mystery for over 78 years. On August 28, 2015 the Tank Barge ARGO was discovered by the Cleveland Underwater Explorers in U.S. waters, beginning a three month response by the U.S. Coast Guard Federal On-Scene Coordinator to mitigate the substantial threat to the environment and public health posed by the ARGO’s cargo. Rife with challenges, the response included complex dive operations, hot tapping, chemical lightering and storage, environmental protection and monitoring and severe logistical constraints, all of which required an extensive incident management organization and utilized almost every “special team” under the National Contingency Plan. This case study summarizes the response to the Tank Barge ARGO and details how “special teams” were utilized by the Federal On-Scene Coordinator to safely and effectively respond to the environmental threat. Specifically, the capabilities of the National Strike Force, District Response Group and District Response Advisory Team, and Scientific Support Coordinator during this response are highlighted and offered as a best practice for other oil and hazardous substance responses.

BackgroundThe aim of this study was to investigate whether ‘dual diagnosis' (substance misuse and severe mental illness) is associated with aggression and offending.MethodTwenty-seven people meeting the criteria for both psychotic illness and a substance use disorder and 65 people with psychosis only were interviewed. Case notes were also examined and key workers asked to rate substance misuse and aggression.ResultsThe severity of aggression and offending among this community treatment sample was low. Individuals with a dual diagnosis were significantly more likely than those with psychosis only to report any history of committing an offence (P=0.001), or recent hostile behaviour (P=0.001). Keyworkers were more likely to report recent aggression among the dually diagnosed (P=0.01). Significant differences persisted when we used logistic regression to control for potentially confounding demographic and clinical variables.ConclusionsDual diagnosis may be an important factor in aggression and offending among severely mentally ill individuals in inner-city areas. Accurate risk assessment requires examination of substance use.

The chapter describes ways of understanding, predicting, and managing suicidality and self-harm in people with severe and enduring mental illness in contact with community outreach teams. Data on incidence are presented with evidence on associations with factors such as demographics, diagnosis, co-morbidity, and life events as a way of identifying risk factors. Validated tools and frameworks for assessment are presented and critiqued with research evidence for the effectiveness of interventions. The chapter concludes with a case study, care plan, and risk management plan of a man whose life has been devastated by psychotic illness.

This chapter discusses the politics, incidence, assessment, and management of challenging, hostile, and violent behaviour from the perspective of practising community outreach with people with more severe and enduring mental health problems. As a way of establishing risk factors, data are presented on incidence of hostility in both community and inpatient settings along with the research evidence on associations between severe mental illnesses, such as psychosis, substance misuse, and violence. Methods and frameworks for risk assessment are presented and critiqued. Risk management techniques with individuals are demonstrated and illustrated through case studies. Community team training, policies, procedures, support, and supervision are presented as examples of best practice.

This paper introduces the new concept of “eco-fusion” through an exploratory case study project. It suggests the importance of multi-scalar practice in the broader field of eco-urbanism. This study introduces eco-fusion as a multiplexed paradigm, which is then discussed in two different development models. This paper first highlights the position of “eco” in urbanism by providing a brief account of key terms and how they relate to one another. It then points out the associations between eco-fusion and sustainable urban development. Through an exploratory case study example in China, the practical factors of eco-development are assessed. The study aims to provide a set of intermediate development stages while maintaining each spatial level’s interface in their own defined and distinguished contexts. The key objective is to consider integrating the natural and built environments, which is considered the best practice of eco-development in urbanism. This study’s findings highlight integrated methods in eco-urbanism and suggest new directions for eco-planning/eco-design strategies.

The number of family caregivers to individuals with dementia is increasing. Family physicians are often the first point of access to the health care system for individuals with dementia and their caregivers. Caregivers are at an increased risk of developing negative physical, cognitive and affective health problems themselves. Caregivers also describe having unmet needs to help them sustain care in the community. Family physicians are in a unique position to help support caregivers and individuals with dementia, but often struggle with keeping up with best practice dementia service knowledge. The Dementia Wellness Questionnaire was designed to serve as a starting point for discussions between caregivers and family physicians by empowering caregivers to communicate their needs and concerns and to enhance family physicians’ access to specific dementia support information. The DWQ aims to alert physicians of caregiver and patient needs. This pilot study aimed to explore the experiences of physicians and caregivers of people using the Questionnaire in two family medicine clinics in Ontario, Canada. Interviews with physicians and caregivers collected data on their experiences using the DWQ following a 10-month data gathering period. Data was analyzed using content analysis. Results indicated that family physicians may have an improved efficacy in managing dementia by having dementia care case specific guidelines integrated within electronic medical records. By having time-efficient access to tailored supports, family physicians can better address the needs of the caregiver–patient dyad and help support family caregivers in their caregiving role. Caregivers expressed that the Questionnaire helped them remember concerns to bring up with physicians, in order to receive help in a more efficient manner.

The use of contemporaneous evaluation in personal injury insurance enables schemes to maintain and enhance their viability through access to quality information on cost, liabilities and outcomes. Best practice in research programs in the sector requires data on client outcomes and financial performance to be collected. This article presents a case study of the research and evaluation program for the National Serious Injury Service of New Zealand's Accident Compensation Corporation.
