Countertransference in End-of-Life Music Therapy

Abstract Countertransference is a complex and important issue for end-of-life music therapists. However, limited literature is available on the subject of end-of-life music therapy and countertransference to guide professionals as we encounter the effects of countertransference inside and outside sessions. There are circumstances associated with end-of-life music therapy that require a closer look, that demand recognition, and that may result in better music experiences for our clients. The purpose of this article is to explore the experiences and influences of countertransference in the unique situations presented by end-of-life music therapy, thus increasing readers’ awareness of the potential impact of countertransference upon their clinical practices.

Abstract Ethical thinking in music therapy is a vital aspect of the profession. However, little is published on the subject of ethics in the music therapy academic setting. Therefore, this article explores the complex issues of ethics and ethical thinking in music therapy education from the perspective of two music therapists who have held various positions within academics in music therapy. The purpose of this article is to encourage integrity within music therapy education and to draw attention to the complex ethical issues within music therapy academics. Four points are described for music therapy educators: (1) Assess positions of power, (2) Explore your adaptability, (3) Assess possible infringement of rights, and (4) Consider cultural bias in your responses. These points are offered as recommendations inserted into the Dileo Model of Ethical Thinking for Music Therapists for people in academia when faced with ethical dilemmas.

Music therapy aligns to the holistic approach to palliative and end-of-life care (PEOLC), with an emergent evidence base reporting positive effect on a range of health-related outcomes for both patient and family carer alongside high client demand. However, the current service provision and the role of music therapists in supporting individuals receiving PEOLC in the UK is currently unknown.ObjectivesThis service evaluation aims to identify the provision, role and perceived impact of UK music therapists in supporting patients receiving PEOLC, their families and health and social care professionals.MethodsA survey was distributed to the British Association for Music Therapy (BAMT) member mailing list in July 2017. BAMT is the professional body for Health and Care Professions Council registered music therapists in the UK.ResultsFifty respondents identified themselves as music therapists currently working with clients receiving PEOLC. The respondents largely reported (84.7%) less than 10 years of experience working in PEOLC settings, with only a minority receiving statutory funding for their role. Music therapists most commonly reported supporting adults with neurological conditions, cancers and dementia.ConclusionsAlthough promising that evidence suggests provision of music therapy in UK PEOLC settings in the past 10 years to have increased, lack of sustainable funding suggests the role to not be consistently accessible in PEOLC.

Abstract Physician-assisted suicide and voluntary euthanasia are two procedures of Medical Assistance in Dying (MAiD) that are readily requested by patients internationally. In April 2016, the legalization of these procedures occurred in Canada after the pursuit of many jurisdictional cases. Known as Bill C-14, this legislation aims to balance patient autonomy at end-of-life with revised standardizations for medical and healthcare professionals. Music therapists may be included in the medical and healthcare team that can be recommended by patients considering MAiD. At present, there are no written guidelines that empirically outline the role of music therapists in this area. This paper explores the global history of MAiD as well as MAiD in the Canadian healthcare system and its implications. Information on the legislation Bill C-14 will be explained, along with a discussion on the current practices of music therapy at end-of-life and the potential scope of a music therapist during the MAiD procedure. Ethical issues and future research are also presented as recommendations to healthcare and music therapy professionals.

Abstract Music therapists in end-of-life care settings are commonly requested by patients to self-disclose their spiritual beliefs and to participate in explicit spiritual practices or rituals. Four potential courses of action available to music therapists will be framed using an integrative ethical decision-making model in context of the American Music Therapy Association Code of Ethics. Stakeholdership, moderating obligations, and ethical values are concepts integral to this examination, providing music therapists dynamic constructs to continuously utilize when determining effective, culturally responsive ethical decisions. Specific ethical principles and codes are provided for additional points of reference, and recommendations are provided to guide future explorations into the ethical dimensions of music therapy and spirituality.

Abstract As a music therapy private practice is both a business and a healthcare service, it should adhere to ethical standards from both disciplines. However, this topic has rarely been examined in the music therapy literature. The purpose of this phenomenological study was to explore ethical dilemmas experienced by music therapy business owners (MTBOs) in their private practice and how MTBOs avoid or address ethical dilemmas. Utilizing convenience and snowball sampling techniques, 21 MTBOs in the United States were interviewed using semi-structured interviews. To answer the two areas of inquiry, we identified three themes and 12 subthemes: (1) Ethical issues related to client welfare, (2) Ethical issues related to business relationships and operation, and (3) Strategies to address or avoid ethical dilemmas. MTBOs also shared how they ensure ethical behavior in themselves, with their employees or independent contractors, and when interacting with professionals outside the private practice. These findings provide a better understanding of MTBOs’ lived experiences of ethics in their private practice and may benefit other music therapists who are in private practice or are wanting to go into private practice. Limitations and recommendations for further research are provided.

Abstract The purpose of this study was to descriptively analyze music therapy employment data from 2013 to 2019, including years in the field, gender, age, ethnicity, hours worked, jobs created, number of new board-certified music therapists (MT-BCs), funding sources, and wages. A database was created to analyze descriptive data from the 2013–2019 American Music Therapy Association Workforce Analysis Surveys as well as data from the Certification Board for Music Therapists. Results indicate a large portion of music therapists (MTs) have been in the field for five years or less. Though the majority of MTs work full time, there is a high rate of part-time employment. An estimate of the total number of new full-time jobs represented a ratio of 57% of new MT-BCs during the period. Private pay was the most reported funding source for music therapy services. Music therapy wages tended to be higher for those with higher levels of education. Limitations of the study and suggestions for future research are provided.
