Abstract Background Aging of the skin consists of a combination of genetically predisposed factors (intrinsic aging) and environmental factors (photo aging). The resulting clinical manifestations include mottled pigmentation, wrinkles, and change of the superficial texture leading to coarseness of the skin Dermal components such as collagen and elastic fibers gradually decrease, skin loses its elasticity and shows wrinkles. Loss of skin moisture and subcutaneous fat accelerates the process. Objective To evaluate the efficacy, possible side effects, durability and study of platelet rich plasma injection versus Plasma gel injection in periorbital rejuvenation. Methods This comparative split face study included 40 females with periorbital wrinkles and/or dark circles recruited from the Dermatology outpatient clinic of Ain-Shams University Hospitals.Right periorbital wrinkles was injected by platelet rich plasma while left periorbital side was injected by plasma gel. Results Treatment with plasma gel resulted in significant improvement of periorbital wrinkles with minimal complications compared to platelet rich plasma. While, dark haloes and skin texture were minimally affected whether with plasma gel or PRP injection. Conclusion Plasma gel as well as platelet rich plasma injections may play role in treatment of periorbital wrinkles but mostly for short durations. Plasma gel injection is more effective in periorbital rejuvenation than PRP injection although it has relatively higher complications rate. Both plasma gel and PRP injections has only minimal role in treatment of periorbital dark hallows.

Abstract Background shoulder pain is the third most common musculoskeletal complaint. And rotator cuff muscles tendinopathy is one of the most common causes for shoulder pain. Aim of the Work to evaluate the effectiveness of the percutaneous ultrasound guided PRP injection in improving or treating chronic Rotator Cuff muscles tendinopathy. Patients and Methods this study was conducted on 33 patients presented to Police Hospital Nasr City and diagnosed as cases of rotator cuff tendinopathy for more than 6 months and not responding to NSAIDs. Results there is high significant improvement of the ultrasound findings and clinical manifestations of patients as regard pain, range of motion and power comparing to the base line start. According to this study, PRP was found to be more effective on the long term then short term “comparing results after one and five month”. Conclusion ultrasound guided PRP injection is a safe and an effective treatment modality for rotator cuff tendinopathy as evidenced by decreased pain and improved range of motion across the shoulder joint after the application and follow up of the PRP.

Abstract Elastin is the main component of elastic fibers, which provide stretch, recoil, and elasticity to the skin. Normal levels of elastic fiber production, organization, and integration with other cutaneous extracellular matrix proteins, proteoglycans, and glycosaminoglycans are integral to maintaining healthy skin structure, function, and youthful appearance. Although elastin has very low turnover, its production decreases after individuals reach maturity and it is susceptible to damage from many factors. With advancing age and exposure to environmental insults, elastic fibers degrade. This degradation contributes to the loss of the skin’s structural integrity; combined with subcutaneous fat loss, this results in looser, sagging skin, causing undesirable changes in appearance. The most dramatic changes occur in chronically sun-exposed skin, which displays sharply altered amounts and arrangements of cutaneous elastic fibers, decreased fine elastic fibers in the superficial dermis connecting to the epidermis, and replacement of the normal collagen-rich superficial dermis with abnormal clumps of solar elastosis material. Disruption of elastic fiber networks also leads to undesirable characteristics in wound healing, and the worsening structure and appearance of scars and stretch marks. Identifying ways to replenish elastin and elastic fibers should improve the skin’s appearance, texture, resiliency, and wound-healing capabilities. However, few therapies are capable of repairing elastic fibers or substantially reorganizing the elastin/microfibril network. This review describes the clinical relevance of elastin in the context of the structure and function of healthy and aging skin, wound healing, and scars and introduces new approaches being developed to target elastin production and elastic fiber formation.

Lateral epicondylitis (tennis elbow) is a major cause of musculoskeletal pain involving common extensor origin of the forearm. This study was done to determine the effects of platelet-rich plasma on 15 patients with lateral epicondylitis. Selected patients were given intralesional platelet-rich plasma injection, activity of daily living instructions and paracetamol. Patients were assessed every 14 days interval by visual analogue scale, and the patient rated tennis elbow evaluation. Treatment response according to visual analogue scale and patient rated tennis elbow evaluation tool, the difference of improvement was found in respect to time, from pretreatment W1 (just before 1st Intervention) score to W11 score in every alternate week (p<0.005). This indicates that intralesional platelet-rich plasma is effective in the patients with lateral epicondylitis of elbow.

<b><i>Objective:</i></b> To investigate the effect of platelet-rich plasma (PRP) injection in patients with atrophic rhinitis. <b><i>Methods:</i></b> Prepared PRP was injected into the inferior turbinate bilaterally, and nasal bacterial cultures were conducted. Improvement of symptoms was assessed with the Nasal Obstruction Symptom Evaluation (NOSE) and the Sino-Nasal Outcome Test-22 (SNOT-22). Nasal mucociliary clearance was assessed using the saccharin transit time (STT). <b><i>Results:</i></b> In the PRP-injected group (group A), NOSE (throughout the study) and SNOT-22 (1 month after injection) scores were significantly decreased during the study. However, the saline spray group (group B) showed no significant nasal symptom improvement during the study period. In group A, the STT was improved until 3 months after the injection. In contrast, group B showed STT improvement after 2 months that was maintained throughout the study. <b><i>Conclusion:</i></b> PRP injections can improve nasal symptoms and nasal mucociliary function in patients with atrophic rhinitis.
