Abstract Introduction Pregnancy is associated with disrupted slow-wave sleep (SWS) and a high prevalence of sleep disordered breathing (SDB), which may further exacerbate the decrease of deep sleep. Reduced slow wave sleep may impair glucose homeostasis, contributing to Gestational Diabetes Mellitus (GDM). Studies investigating EEG markers of deep and light sleep, and their associations with SDB and GDM are lacking. In this study, we measured associations of EEG Delta-power with objective SDB measures assessed in late-pregnancy to determine if changes in these bands are associated with GDM risk. Methods 74 women (24-36 weeks pregnancy) underwent overnight polysomnography. Spectral profiles for Delta relative power were created for NREM and REM sleep after removing epochs with movements or muscle artifacts. The association of Delta power with SDB, assessed by the Apnea Hypopnea-Index (AHI) and AHI-based SDB severity (none, mild, moderate, severe) was tested by multivariate linear regression including demographic variables with bivariate correlations (p<0.2) versus Delta-power. Conditional-regression was used to explore relationships between Delta-power and GDM, controlling for covariates. Results Obstructive Sleep Apnea (OSA, AHI>5) was present in 14% of subjects (8 GDM-cases and 3 controls). In bivariate analyses, AHI, AHI-severity categories and OSA were associated with Delta-power in NREM (all p<0.2) and AHI was associated with Delta relative-power in REM (p=0.18). However, these associations did not remain significant after adjusting for covariates. Delta relative-power in NREM was significantly associated with decreased risk of GDM (OR:0.50, 95%CI-0.25,0.91), but, in REM sleep, was not associated with GDM risk (OR:1.25, 95%CI-0.79,1.97). Conclusion These analyses failed to demonstrate an association between OSA or OSA severity and EEG Delta power. However, lower levels of SWS, characterized by low Delta power were associated with increased GDM risk. Support NIH-R00-NR013187

Headaches occurring during sleep are one of the most common types of night time pain complaints, along with back pain. Sleep-related headaches can be a manifestation of both primary headaches (migraine, cluster headache, chronic paroxysmal hemicrania, hypnic headache) and secondary headaches associated with somatic pathology (anemia, hypoxemia), neurological disorders (brain tumors, arteriovenous malformations), psychiatric (depressive, anxiety) and sleep disorders (obstructive sleep apnea). The relationship between headaches and sleep depends on the patients age, frequency and severity of the headaches, provoking factors (excessive sleep, sleep deprivation, overuse of painkillers), the stage of sleep (REM sleep or slow-wave sleep) and possible genetic predisposition (hemiplegic migraine). The connections between sleep and headaches are complex and interrelated. Sleep can both provoke and relieve headaches. On the other hand, headaches can cause sleep disorders, which are typical for a severe type of cephalgia with the development of chronic daily headache syndrome, medication overuse, and psychiatric comorbidity. General anatomical structures, neurochemical and neurophysiological mechanisms involved in sleep and headache regulation are assumed. According to polysomnography data, objective changes in the structure of night sleep were detected in patients with a sleep-related headache: a reduction in the sleep duration and a decrease in the slow-wave sleep representation. Most nighttime headache attacks are linked with the REM sleep phase. Management of patients with sleep-related headaches should include the diagnosis and treatment of both headache and sleep disorder, which will significantly improve the results of treatment or even cure headaches in some cases.

AbstractThis study aimed to investigate the clinical characteristics and predictors of increased rapid eye movement (REM) sleep or slow wave sleep (SWS) in patients with obstructive sleep apnea (OSA) following positive airway pressure (PAP) therapy. The study retrospectively analyzed data from patients with OSA who underwent both diagnostic polysomnography (PSG) and pressure titration PSG at the Tangdu Hospital Sleep Medicine Center from 2011–2016. Paired diagnostic PSG and pressure titration studies from 501 patients were included. REM rebound was predicted by a higher oxygen desaturation index, lower REM proportion, higher arousal index, lower mean pulse oxygen saturation (SpO2), higher Epworth sleepiness score and younger age (adjusted R2 = 0.482). The SWS rebound was predicted by a longer total duration of apneas and hypopneas, lower N3 duration, lower SpO2 nadir, lower REM proportion in diagnostic PSG and younger age (adjusted R2 = 0.286). Patients without REM rebound or SWS rebound had a high probability of comorbidities with insomnia and mood complaints. Some parameters (subjective and objective insomnia, excessive daytime sleepiness, age and OSA severity) indicate changes in REM sleep and SWS between diagnostic and titration PSG tests. Treatment of insomnia and mood disorders in patients with OSA may helpful to improve the use PAP.
