In December 2001, the North Carolina Division of Public Health established Public Health Regional Surveillance Teams (PHRSTs) to build local public health capacity to prevent, prepare for, respond to, and recover from public health incidents and events. Seven PHRSTs are colocated at local health departments (LHDs) around the state.The authors assessed structural capacity of the PHRSTs and analyzed the relationship between structural capacity and the frequency of support and services provided to LHDs by PHRSTs. Five categories of structural capacity were measured: human, fiscal, informational, physical, and organizational resources. In addition, variation in structural capacity among teams was also examined.The most variation was seen in human resources. Although each team was originally designed to include a physician/epidemiologist, industrial hygienist, nurse/epidemiologist, and administrative support technician, team composition varied such that only the administrative support technician is common to all teams. Variation in team composition was associated with differences in the support and services that PHRSTs provide to LHDs.Teams that reported having a medical doctor or a doctor of osteopathic medicine (χ2 = 9.95; p 0.01) or an epidemiologist (χ2 = 5.35; p 0.02) had larger budgets and provided more support and services, and teams that housed a pharmacist reported more partners (χ2 = 52.34; p 0.01). Teams that received directives from more groups (such as LHDs) also provided more support and services in planning (Z = 21.71; p 0.01), communication and liaison (Z = 12.11; p 0.01), epidemiology and surveillance (Z = 5.09; p 0.01), consultation and technical support (Z = 2.25; p = 0.02), H1N1 outbreak assistance (Z = 10.25; p 0.01), and public health event response (Z = 2.19; p = 0.03).In the last 10 years, significant variation in structural capacity, particularly in human resources, has been introduced among PHRSTs. These differences explain much of the variation in support and services provided to LHDs by PHRSTs.

AbstractThe 2009 Pinelake Health and Rehab Center shooting in Carthage, North Carolina, presents a unique case study for examining the specific considerations for mass violence events in senior living facilities. A variety of factors, including reduced sensory perception, reduced mobility, and cognitive decline, may increase the vulnerability of the populations of senior living facilities during mass violence events. Management of response aspects such as evacuation, relocation, and reunification also require special consideration in the context of mass violence at senior living facilities. Better awareness of these vulnerabilities and response considerations can assist facility administrators and emergency managers when preparing for potential mass violence events at senior living facilities. A survey of newly trained local Sudden Infant Death Syndrome (SIDS) counselors was undertaken by the North Carolina Sudden Infant Death Syndrome Project to evaluate the use of the autopsy report as a counseling tool. The counselors, most of whom were public health nurses, reported that sharing the autopsy report with bereaved parents was a valuable part of the counseling process. Most parents wanted to see copies of the report. Although not all counselors were comfortable going over the report, there was a significant increase in comfort among counselors whose training included sharing a home visit with an experienced counselor.

Since the onset of the COVID-19 pandemic, the North Carolina Local Food Council has strengthened its role as a cohesive and effective organization during a public-health crisis to share challenges, devise solutions, and build resilience across local food systems in North Carolina. The Council includes representatives from 21 organizations working across the state, as well as three representatives from regional local food councils. The Council’s response to the pandemic addressed three key areas of action: (1) Coordinate responses across multiple sectors; (2) Enhance collaboration across the food supply chain; and (3) Facilitate data collection and public messaging. This paper describes the positive impacts the Council has had across North Carolina on consumers and producers of local food as a result of this collaborative network and long-established relationships across the state. Now, more than ever, the relationships and collaborative efforts of statewide organizations and partners are needed. The Council’s crisis response has been strong because of the long-standing relationships of its members and its ability to share resources quickly, allowing it to work toward coordinated responses. The work of the North Carolina Local Food Council can serve as a model for other states that have state-level local food coun¬cils or want to develop them. In addition, the Council’s work demonstrates how collaborations among statewide partners can foster resilience within local food systems, particularly during a public health crisis.

This paper provides an account of the development of the North Carolina History and Fiction Digital Library (http://www.lib.ecu.edu/ncc/historyfiction/) from an idea into a worldwide resource in two years. The principal investigator for the project introduces the Web site and gives practical and technical information that can be used as a model in other digital projects. The paper includes use statistics; reactions to the site; suggestions for how students, historians, genealogists, and other researchers can use the site; and plans for enhancements.
