I should like to offer the following comments about the recent article by Gardner, and others, on the use of racemic epinephrine in croup. 1. Unfortunately the racemic epinephrine was not delivered by IPPB, using a respirator, as by those who have reported impressive successes, albeit uncontrolled, with that method. 2. Based on their initial positive experience with postanesthetic croup, Jordan et al. took the battle directly to the surgeons and won many surgical converts by initially performing the racemic epinephrine procedure only on patients about to undergo tracheotomy.

I was fascinated by your decision to publish the recent article "Monitoring for Anthracycline Cardiotoxicity" by Dr Lipshultz et al. (Pediatrics. 1994;93:433-437). The article appears to be nothing more than a somewhat delayed letter to the Editor in response to the previously published "Guidelines for Cardiac Monitoring of Children During and After Anthracycline Therapy: Report of the Cardiology Committee of the Children's Cancer Study Group" (CCSG) (Pediatrics. 1992;89:942-949). In attempting to refute the recommendations of a national panel of experts, the authors present their opinions supported by no new data.

Dr. Todd has written a thoughtful commentary on our recent article. All tests, alone or in combination, including the medical history and physical examination, have false-positive and false-negative errors. The balance between these errors is, as Dr. Todd mentions, determined by the cutoff points we choose. Although other cutoff points for WBC count and ESR were studied, a WBC count of 15,000/cu mm or more and an ESR of 30 mm/hr or more demonstrated the best balance of specificity and sensitivity for bacteremia or pneumonia in young children who had a temperature of 4°C or more.

As practicing pediatricians, we read with great interest the recent article by Baraff et al1 regarding the management of febrile illnesses in children. The authors took great care to recommend an approach to this common problem which might be practically followed in both practitioners' offices and hospital emergency departments. Previous studies have documented the well-recognized disparity which exists between pediatric offices and hospital emergency departments regarding the laboratory evaluation performed in situations in which young children have fever without an apparent source.
