AF is a worldwide epidemic, affecting approximately 33 million people, and its rising prevalence is expected to account for increasing clinical and public health costs. AF is associated with an increased risk of MI, heart failure, stroke, dementia, chronic kidney disease and mortality. Preserving sinus rhythm is essential for a better outcome. However, because of the inherent limits of both pharmacological and interventional methods, rhythm strategy management is reserved for symptom and quality-of-life improvement. While ‘classical’ antiarrhythmic drug therapy remains the first-line therapy for rhythm control, its efficacy and safety are limited by empirical use, proarrhythmic risk and organ toxicity. Ablative techniques have had an impressive development, but AF ablation still failed to demonstrate a significant impact on hard endpoints. Understanding of the complex mechanisms of AF will help to develop new vulnerable targets to therapy. Promising molecules are under development, intended to fill the gap between the current pharmacological treatment aimed at maintaining sinus rhythm and the expectations from rhythm strategy.

With the continued economic downturn, coupled with the slowdown of domestic economic development and fierce market competition, the development of small and medium-sized enterprises has become more and more difficult. Because the financial background and operating strength of small and medium-sized enterprises are far inferior to large-scale enterprises, coupled with the influence of global economic integration, many small and medium-sized enterprises have gradually closed down. Therefore, exploring the entropy coupling algorithm is of great significance to the role of corporate leadership strategy management. This paper studies the coupling conditions and coupling process of corporate strategy and business model, builds a new coupling model, and goes deep into the coupling model to study the cooperation mechanism between its internal modules and initially builds the company to break through the two major dilemmas at the same time. This paper uses entropy theory to evaluate corporate leadership strategy, constructs an evaluation index system based on entropy, determines the weight of each index, and calculates the entropy value. This paper uses the alpha coefficient to test the reliability of the questionnaire. The value range of α coefficient is [0, 1], and different values represent different reliability. Large enterprises as a whole are mostly in the highly coupled (41.80%) and moderately coupled (27.34%) stages; medium-sized enterprises as a whole are mostly in the highly coupled (39.50%) and moderately coupled (31.50%) stages; small enterprises as a whole are mostly in the moderately coupled (39.50%) and moderately coupled (40.72%), low coupling (33.20%), and high coupling (25.9%) stages; microenterprises as a whole are mostly in the low coupling (43.70%), moderate coupling (36.41%), and high coupling (30.51%) stages. The results show that the entropy coupling algorithm can improve the deficiencies in the leadership strategy and provide a practical and reliable path for carrying out leadership development projects.

ABSTRACTBackgroundData regarding critical care for patients with severe COVID-19 are limited. We aimed to describe the clinical course, multi-strategy management, and respiratory support usage for the severe COVID-19 at the provincial level.MethodsUsing data from Sichuan Provincial Department of Health and the multicentre cohort study, all microbiologically confirmed COVID-19 patients in Sichuan who met the national severe criteria were included and followed-up from the day of inclusion (D1), until discharge, death, or the end of the study.FindingsOut of 539 COVID-19 patients, 81 severe cases (15.0%) were identified. The median (IQR) age was 50 (39-65) years, 37% were female, and 53.1% had chronic comorbidities. All severe cases were identified before requiring mechanical ventilation and treated in the intensive care units (ICUs), among whom 51 (63.0%) were treated in provisional ICUs and 77 patients (95.1%) were admitted by D1. On D1, 76 (93.8%) were administered by respiratory support, including 55 (67.9%) by conventional oxygen therapy (COT), 8 (9.9%) by high-flow nasal cannula (HFNC) and 13 (16.0%) by non-invasive ventilation (NIV). By D28, 53 (65.4%) were discharged, three (3.7%) were deceased, and 25 (30.9%) were still hospitalized. COT, administered to 95.1% of the patients, was the most commonly used respiratory support and met 62.7% of the respiratory support needed, followed by HFNC (19.3%), NIV ventilation (9.4%) and IV 8.5%.InterpretationThe multi-strategy management for severe COVID-19 patients including early identification and timely critical care may contribute to the low case-fatailty. Preparation of sufficient conventional oxygen equipment should be prioritized.Trial registration numberChiCTR2000029758.
