IntroductionCurrent HIV guidelines recommend differentiated service delivery (DSD) models that allow for fewer health centre visits for clinically stable people living with HIV (PLHIV). Newly diagnosed PLHIV may require more intensive care early in their treatment course, yet frequent appointments can be burdensome to patients and health systems. Determining the optimal parameters for defining clinical stability and transitioning to less frequent appointments could decrease patient burden and health system costs. The objectives of this pilot study are to explore the feasibility and acceptability of (1) reducing the time to DSD from 12 to 6 months after antiretroviral therapy (ART) initiation,and (2) reducing the number of suppressed viral loads required to enter DSD from two to one.Methods and analysesThe present study is a pilot, unblinded trial taking place in three health facilities in Kigali, Rwanda. Current Rwandan guidelines require PLHIV to be on ART for ≥12 months with two consecutive suppressed viral loads in order to transition to less frequent appointments. We will randomise 90 participants to one of three arms: entry into DSD at 6 months after one suppressed viral load (n=30), entry into DSD at 6 months after two suppressed viral loads (n=30) or current standard of care (n=30). We will measure feasibility and acceptability of this intervention; clinical outcomes include viral suppression at 12 months (primary outcome) and appointment attendance (secondary outcome).Ethics and disseminationThis clinical trial was approved by the institutional review board of Albert Einstein College of Medicine and by the Rwanda National Ethics Committee. Findings will be disseminated through conferences and peer-reviewed publications, as well as meetings with stakeholders.Trial registration numberNCT04567693.

In this paper we present counter-intuitive examples for the multiclass queueing network, where each station may serve more than one job class with differentiated service priority and each job may require service sequentially by more than one service station. In our examples, the network performance is improved even when more jobs are admitted for service.

Wireless Sensor Network finds its extensive use in healthcare applications for the transfer of time-critical data through wireless connectivity. The primary cause of network failure is the transfer of time-critical multimedia data. The article presents a new differentiated service modelsupported (DSM) cluster-based routing in wireless sensor networks (WSNs) that overcomes the above issue. DSM prioritizes the transfer of different flow types based on packet type and packet size. The employment of computational offloading minimizes delay for critical and small-sized data packets and by carrying out data reduction of large-sized packets at proxy server. It outperforms the existing protocols in terms of energy efficiency, throughput, and reliability by prioritizing the transfer of time-critical health application data

Purpose The conceptualization of service sabotage failed to adequately tap the domain of interest. Phenomena like turnover and service sabotage are difficult to measure and are not suitable for individual-level study. However, “intention” is suitable for individual-level or management-oriented studies. The paper aims to discuss these issues. Design/methodology/approach A new scale (eight items) to measure the intention to sabotage was developed and tested using a sample of bank (n=313) and insurance (n=258) employees in Nigeria. Cynicism and the desire for justice are the roots of sabotage. As such, the inability to stabilize institutionalized work processes and procedures may cause employees to be overcome with the intention to sabotage service, prior to the actual sabotage. Drawing on conservation of resources theory, this paper investigates the impact of employee cynicism on intention to sabotage as moderated by procedural justice. Findings The analyses suggest that employee cynicism is related to the intention to sabotage, and procedural justice moderates the relationship between employee cynicism and intention to sabotage. The findings endorse the model of interest, and implications of this study for research and practice are discussed. Originality/value The study differentiated service sabotage from intention to sabotage, and developed and tested a scale to measure the intention to sabotage.
