Effect of Acupuncture at Ht7 on Heart Rate Variability: An Exploratory Study

Objective To explore the effect of acupuncture at HT7 on heart rate variability (HRV) in healthy volunteers. Methods 120 subjects were divided into four groups using a random number table. The following groups of acupuncture interventions were used: HT7 verum acupuncture; HT7 non-penetrating sham acupuncture; acupuncture at a sham point; and no acupuncture. HRV was recorded 10 min before, during and after each stimulation using an Actiheart ECG recorder. Results The HT7 verum acupuncture group had higher very-low frequency, low frequency and high frequency components of HRV compared with the control groups during but not after acupuncture. The HT7 verum acupuncture group also had higher SD of normal intervals compared with the sham needling and no acupuncture control groups. Conclusions Our preliminary study suggests, subject to limitations, that acupuncture at HT7 could affect cardiac autonomic neural regulation in healthy subjects, manifest as increased HRV, most likely via the parasympathetic system. This study is a double-blind randomized controlled trial on the effect of intradermal acupuncture on insomnia after stroke. Hospitalized stroke patients with insomnia were enrolled in the study and were randomly assigned to either a real intradermal acupuncture group (RA group) or a sham acupuncture group (SA group). The RA group received intradermal acupuncture on Shen-Men (He-7) and Nei-Kuan (EH-6) for three days, and the SA group received sham acupuncture on the same points. The effect of acupuncture on insomnia was measured using Insomnia Severity Index (ISI) and Athens Insomnia Scale (AIS) at baseline and three days after treatment. To assess the effect of acupuncture on the autonomic nervous function, the subjects' blood pressure and heart rate variability were monitored. Fifty-two subjects (27 in the RA group and 25 in the SA group) were included in the final analysis. The insomnia-related scales ISI and AIS showed greater improvement of insomnia in the RA group than in the SA group. Moreover, there is a greater reduction of the number of non-dippers and a greater decrease of the LF/HF ratio (heart rate variability) in the RA group than in the SA group. These results indicate that sympathetic hyperactivities were stabilized in the RA group. It can thus be concluded that intradermal acupuncture on Shen-Men and Nei-Kuan is a useful therapeutic method for post stroke-onset insomnia as it reduces sympathetic hyperactivities.

ABSTRACT This study aims at evaluating the effects of electrolytes, glucose and cortisol levels over heart rate (HR) and heart rate variability (HRV) in healthy newborn calves. Seventeen healthy Holstein calves were evaluated during their first month of life, and the plasma concentrations of glucose, cortisol, calcium, magnesium, inorganic phosphorus, sodium and potassium were analyzed. HRV indexes were determined in the time and frequency domains through the analysis of neonatal electrocardiogram recordings. In the first day, low blood levels of phosphorus presented a strong correlation with the HR and the increased high-frequency components of HRV. The plasma concentrations of magnesium decreased significantly throughout the 35 days, revealing a positive association with a decreasing low-frequency components of HRV at day 28. There was a strong correlation between HR, HRV indexes, some plasma electrolytes, glucose and cortisol during the studied period. Variations in the concentrations and correlations observed may be attributed to the adaptive neonatal period in calves.

BackgroundAntipsychotic medications cause a wide range of adverse effects and have been associated with sudden death in psychiatric patients.AimsTo supply power spectral analysis of heart rate variability as a tool to examine the arrythmogenic effects of neuroleptics.MethodHeart rate analysis was carried out in patients with schizophrenia on standard doses of neuroleptic monotherapy − 21 were on clozapine, 18 on haloperidol and 17 on olanzapine – and in 53 healthy subjects.ResultsPatients with schizophrenia on clozapine had significantly higher heart rate, lower heart rate variability and lower high-frequency and higher low-frequency components compared with patients on haloperidol or olanzapine and matched control subjects. Prolonged QTc intervals were more common in patients than controls.ConclusionsPatients treated with neuroleptic medications, especially clozapine, showed autonomic dysregulation and cardiac repolarisation changes. Physicians should be aware of this adverse reaction.

Heart rate variability (HRV) spectra are typically analyzed for the components related to low- (less than 0.15 Hz) and high- (greater than 0.15 Hz) frequency variations. However, there are very-low-frequency components with periods up to hours in HRV signals, which might smear short-term spectra. We developed a method of spectral analysis suitable for selectively extracting very-low-frequency components, leaving intact the low- and high-frequency components of interest in HRV spectral analysis. Computer simulations showed that those low-frequency components were well characterized by fractional Brownian motions (FBMs). If the scale invariant, or self-similar, property of FBMs is considered a new time series (x′) was constructed by sampling only every other point (course graining) of the original time series (x). Evaluation of the cross-power spectra between these two (Sxx′) showed that the power of the FBM components was preserved, whereas that of the harmonic components vanished. Subtraction of magnitude of Sxx from the autopower spectra of the original sequence emphasized only the harmonic components. Application of this method to HRV spectral analyses indicated that it might enable one to observe more clearly the low- and high-frequency components characteristic of autonomic control of heart rate.

Using a linear discriminant analysis of heart rate variability (HRV) indices, the present study sought to verify the usefulness of autonomic measurement in major depressive disorder (MDD) patients by assessing the feasibility of their return to work after sick leave. When reinstatement was scheduled, patients’ HRV was measured using a wearable electrocardiogram device. The outcome of the reinstatement was evaluated at one month after returning to work. HRV indices including high- and low-frequency components were calculated in three conditions within a session: initial rest, mental task, and rest after task. A linear discriminant function was made using the HRV indices of 30 MDD patients from our previous study to effectively discriminate the successful reinstatement from the unsuccessful reinstatement; this was then tested on 52 patients who participated in the present study. The discriminant function showed that the sensitivity and specificity in discriminating successful from unsuccessful returns were 95.8% and 35.7%, respectively. Sensitivity is high, indicating that normal HRV is required for a successful return, and that the discriminant analysis of HRV indices is useful for return-to-work screening in MDD patients. On the other hand, specificity is low, suggesting that other factors may also affect the outcome of reinstatement.

We hypothesized that spontaneous movements (postural adjustments and ideomotion) disturb analysis of heart rate and blood pressure variability and could explain the discrepancy between studies. We measured R-R intervals and systolic blood pressure in nine healthy sitting subjects during three protocols: 1) no movement allowed, 2) movements allowed but not standing, 3) movements and standing allowed. Heart rate and blood pressure were not altered by movements. Movements with or without standing produced a twofold or greater increase of the overall variability of R-R intervals and of the low-frequency components of spectral analysis of heart rate variability. The spectral exponent β of heart rate variability (1.123 at rest) was changed by movements (1.364), and the percentage of fractal noise (79% at rest) was increased by standing (91%, coarse-graining spectral analysis). Spontaneous movements could induce a plateau in the correlation dimensions of heart rate variability, but they changed its nonlinear predictability. We suggest that future studies on short-term cardiovascular variability should control spontaneous movements.
