A disability is a physical or mental impairment that has an adverse long-term impact on someone's ability to carry out normal day-to-day activities. This is often thought of in terms of medical conditions with clearly defined symptoms. This chapter, however, argues that it is these symptoms that can be considered to be the impairments and that in the right environment can in fact be advantageous. Someone may be have a medical diagnosis but not be symptomatic due to medication, for instance. In this chapter, a set of symptoms is made up from a number of different scales, called Social-Behavioural Traits (SBTs), which are affected by a person's Serotonergic-Dopamagenic Asynchronicity (SDA). This chapter discusses the role of affective computing in achieving harmony between people with various personality differences, including a model for explaining how technology can influence SDA and SBTs for the better.

This review surveys technological disruption in banking, examining its impact on competition and its potential to increase efficiency and customer welfare. It analyzes the possible strategies of the players involved—incumbents and FinTech and BigTech firms—and the role of regulation. The industry is facing radical transformation and restructuring, as well as a move toward a customer-centric platform-based model. Competition will increase as new players enter the industry, but the long-term impact is more open. Regulation will decisively influence to what extent BigTech will enter the industry and who the dominant players will be. The challenge for regulators will be to keep a level playing field that strikes the right balance between fostering innovation and preserving financial stability. Consumer protection concerns rise to the forefront.

AbstractIn November 1876, two Oneida Indians, Abram Elm and Lewis Doxtator, were arrested for voting illegally in the twenty-third congressional district election in New York. Their trial was held the next year in a federal court in the Northern District of New York, the same venue where Susan B. Anthony had been tried and convicted on a similar charge four years earlier. This essay focuses on the significance of the historically neglected United States v. Elm case, its origins, why the decision was rendered, and its short-term and long-term impact. Importantly, United States v. Elm has cast a long shadow over Supreme Court decisions—from the time of Elk v. Wilkins in 1884 right up to City of Sherrill v. Oneida Indian Nation in New York in 2005. In going to the polls, the two Native Americans were not trying to deny their Oneida identity; they saw themselves as dual citizens advocating a different course of resistance.

Background Symptomatic congenital cytomegalovirus (CMV) infection has an impact mainly on neurological sequelae, including sensorineural deafness. Because of the long-term impact, early treatment of CMV infection is mandatory. However, predictive factors for hearing function improvement in CMV infection therapy remain unexamined. Objective To evaluate potential predictive factors for hearing improvement in pediatric CMV infection therapy. Methods All medical record data of patients aged 0-6 years with CMV infection who completed a 6-week course of ganciclovir therapy or a combination of a 4-week course of ganciclovir and a 2-week course valganciclovir from January 2013 to December 2017 were collected. Age at onset of therapy, gender, gestational age, nutritional status, multi-organ involvement, and neurological symptoms were studied as potential predictive factors of hearing improvement in CMV therapy. The effectiveness of CMV infection therapy on improving hearing function was measured with the brainstem evoked response audiometry (BERA) test. Results BERA tests proportion in the right, left, and best ear showed significant improvement after therapy. All variables analyzed were not statistically significant as predictive factors for hearing improvement in CMV infection therapy. Conclusion Ganciclovir/valganciclovir therapy in CMV infection patients accounted for the improvement of hearing impairment. However, none of the assessed factors were considered predictive for improving hearing function in CMV infection therapy.

Abstract: Regardless of age, malignant diseases and deaths can occur even in children. About 100 of one million children suffering from cancer every year. Indonesian laws do not allow the children to give any decision or refusal about the medical treatment directed to them. Moreover, there are posibilities that they do not get any information from the medical staff. The children know that something wrong is going to happen to them although their parents and doctors tell them nothing. Lack of information make them frightened and anxious. These feelings can give a long term impact if they finally being cured from their diseases. Medical staffs and family members have very important roles in giving the right information to the children concerning the current problem and the worst possibility, and also in giving physicological support.

This article explores older owner occupiers in lower value properties who, having acquired their home through the Right to Buy (RTB) in the 1980s, are now experiencing housing-related challenges in older age. This article outlines the views and perceptions of older owner occupiers, social landlords, voluntary groups and housing organisations to explore the legacy of the RTB. Current and future policy challenges in the area include the differentiation of home owners, difficulties of selling property with low equity in older age and the relationship between health and housing. This article calls to widen the analysis of the long-term impact of the RTB to owner occupiers in lower value properties and notes that ‘ageing in place’ goes beyond looking at people’s current house to the linked housing choices available to them. We recommend that policy support be extended to older home owners to increase housing choice in older age.

Post-traumatic stress disorder (PTSD) has been shown to be a significant problem for both patients and relatives after critical illness. For patients the recall of delusional memories from the period in ICU can be a powerful trigger for the development of PTSD. Such memories are described by patients as very vivid and difficult to separate from reality. Early recognition and treatment of PTSD, where needed, can reduce the long term effects. Chronic PTSD, where symptoms have been present for three months after the traumatic event, is associated with a number of long term health problems such as chronic pain. It can also have profound effects on relationships, financial status and overall wellbeing. The provision of an ICU diary has been shown to reduce the incidence of PTSD in patients and reduce the level of PTSD-related symptoms in family members. For the majority of patients this relatively simple intervention helps them to fill in memory gaps and combat any delusional memories they may recall.
