Aims and methodTo assess how well psychiatric teams follow up abnormal results from physical health monitoring of patients in a secondary care clozapine clinic compared with follow-up in a specialist secondary care physical health clinic, using an audit/re-audit method of comparison. Prevalence data for cardiovascular risk factors within the clinic population were also obtained.ResultsSubstantial and statistically significant (P < 0.01) improvements in follow-up of abnormal results were made following the introduction of a specialist clinic compared with treatment as usual. Prevalence of all cardiovascular risk factors among the patients on clozapine was very high compared with the general population.Clinical implicationsPatients on clozapine have significant and multiple cardiovascular risk factors, which may be best managed within a specialist secondary care physical health clinic that can follow up abnormal results and coordinate care across primary and secondary care.

Penelitian di bidang Interaksi Manusia dan Komputer khususnya aspek usability untuk aplikasi pemantauan kesehatan penyakit cardiovascular (CVD) pada negara berkembang telah menjadi fenomena yang belum diteliti sepenuhnya. Pada era digitalisasi, pasien penyakit jantung dapat menggunakan aplikasi pemantauan kesehatan CVD pada perangkat selulernya untuk memantau kondisi penyakitnya. Agar aplikasi dapat digunakan sesuai dengan keperluan pasien CVD, maka aspek usability seperti ease of use (mudah digunakan), learnability (mudah dipelajari), effectiveness (efisiensi), flexibility (fleksibilitas), dan attitude of users (sikap pengguna) sangat penting dalam menentukan kegunaan aplikasi. Penelitian ini meneliti integrasi aspek usability ke dalam aplikasi pemantauan kesehatan CVD oleh usability engineers dan system developers. Penelitian ini menggunakan metode Systematic Literature Review (SLR) dan mengusulkan model konseptual yang disebut dengan Usability of Cardiovascular Health Monitoring Application Model (uCALM). Model ini bertujuan untuk meningkatkan aspek usability untuk aplikasi pemantauan kesehatan CVD untuk manfaat pasien dan dokter spesialis jantung mereka.

Survival rates for patients following liver transplantation exceed 90% at 12 months and approach 70% at 10 years. Part 1 of this guideline has dealt with all aspects of liver transplantation up to the point of placement on the waiting list. Part 2 explains the organ allocation process, organ donation and organ type and how this influences the choice of recipient. After organ allocation, the transplant surgery and the critical early post-operative period are, of necessity, confined to the liver transplant unit. However, patients will eventually return to their referring secondary care centre with a requirement for ongoing supervision. Part 2 of this guideline concerns three key areas of post liver transplantation care for the non-transplant specialist: (1) overseeing immunosuppression, including interactions and adherence; (2) the transplanted organ and how to initiate investigation of organ dysfunction; and (3) careful oversight of other organ systems, including optimising renal function, cardiovascular health and the psychosocial impact. The crucial significance of this holistic approach becomes more obvious as time passes from the transplant, when patients should expect the responsibility for managing the increasing number of non-liver consequences to lie with primary and secondary care.

This study looks at secondary care utilisation metrics as an effectiveness indicator of Clozapine initiation in 89 patients over the following year compared to the year prior to initiation. It was found that there was an overall reduction of outpatient use and crisis activity, with a marginal drop in bed usage. Medical outpatient use increased, as did physical health monitoring. Overall, compared with costs over the year preceding Clozapine initiation, an estimated saving per patient of GB Pounds 1800 was estimated over the first year post initiation, increasing to GB Pounds 3100 per patient if those with premorbid dissocial and substance misuse traits were excluded, raising the issue of better treatment targeting.
