Aim. This study was aimed to determine the prevalence of Tannerella forsythia in subgingival plaque samples of chronic periodontitis patients having different level of periodontal destruction and to assess the effect of scaling and root planing (SRP) on prevalence of T. forsythia. Materials and Methods. Study included 3 groups: group 1 were healthy individuals, group 2 had periodontitis with probing depth ≤ 5 mm, and group 3 had periodontitis with probing depth > 5 mm. Subjects in groups 2 and 3 exhibited both healthy and diseased periodontal sites. Prevalence of T. forsythia was determined using polymerase chain reaction. Subjects in groups 2 and 3 received SRP and were reevaluated three months after SRP. Results. T. forsythia was not detected in group 1. It was found in diseased sites in 40% and 73.33% of patients from groups 2 and 3, respectively. It was also found in healthy sites in 6.67% and 13.33% of patients from groups 2 and 3, respectively. The detection frequency of T. forsythia after SRP was 6.67% and 13.33% in groups 2 and 3, respectively. Conclusion. The results indicate a possible association between periodontal disease and presence of T. forsythia. Also, the detection frequency of T. forsythia was reduced after SRP.

Recently discovered interleukin 29 (IL-29) has antiviral properties and its production is induced by herpes viruses. This study was aimed at analyzing the effect of non-surgical periodontal treatment on IL-29 levels in gingival crevicular fluid (GCF) of chronic and aggressive periodontitis patients. A total of 60 participants were divided into healthy group (group 1;n= 20), chronic periodontitis group (group 2;n= 20), and aggressive periodontitis group (group 3;n= 20). GCF samples collected from each subject at baseline and 6–8 weeks after scaling and root planing were quantified for IL-29 levels using ELISA. The mean IL-29 concentration in GCF was found to be highest in group 3 (92.37 pg/μl). The mean IL-29 level in group 1 and group 2 was 36.88 pg/μl and 69.35 pg/μl respectively. After scaling and root planing, the mean concentration of IL-29 in GCF was increased to 85.99 pg/μl in group 2 and to 114.64 pg/μl in group 3. Results of the present study indicate that antiviral IL-29 level was highest in GCF of aggressive periodontitis patients and least in subjects with healthy periodontium, while that of chronic periodontitis lying in between. After non-surgical periodontal therapy, IL-29 levels increased both in chronic and aggressive periodontitis patients and deserve further investigation as a potential therapeutic agent in treating periodontitis.

Background. Conventional mechanical debridement alone cannot eliminate bacteria and their products from periodontal pockets. Adjunctive therapies improve tissue healing through detoxification and bactericidal effects. Photodynamic therapy (PDT) is a non-invasive treatment procedure that involves the use of a dye as a photosensitizer to attach to the target cell and be activated by a photon of an appropriate wavelength. This study aimed to assess the effectiveness of PDT in treating periodontitis as an adjunct to scaling and root planing. Methods. Fifteen subjects with chronic periodontitis were treated randomly with scaling and root planing (SRP), followed by a single PDT (test) or SRP (control) episode alone. Full-mouth plaque index (PI), sulcus bleeding index (SBI), probing depth (PD), and clinical attachment level (CAL) were assessed at baseline and 1-month and 3-month intervals. Microbiological evaluation of Porphyromonas gingivalis (Pg) in subgingival plaque samples was performed using a commercially available real-time polymerase chain reaction. Results. The results revealed a significant difference in PI, SBI, PD, CAL, and microbiological parameters between the groups one and three months after treatment. Conclusion. A combination of PDT and SRP gave rise to a significant improvement in clinical and microbiological parameters in patients with chronic periodontitis.

Abstract Recent studies have shown that obesity is not a homogeneous condition and that there is a subgroup of people with obesity, but without metabolic disturbance. This phenotype of obesity is called “metabolically healthy obesity” (MHO) [1]. More and more data are appearing in the scientific literature, indicating that quantitative and qualitative changes in the gut microbiota (GM) can be a trigger in the development of obesity and metabolic disorders [2]. In order to study the role of GM in the development of various types of obesity, 37 patients were examined, divided into 3 groups: group 1 (n = 11) - healthy people without obesity and overweight (control), group 2 (n = 13) - patients with MHO, group 3 (n = 13) - patients with metabolically unhealthy obesity (MUHO). The basic metabolic parameters were determined for all of them and a quantitative assessment of the condition of the GM was performed using the Real-time PCR method. Results: 1. In people from the control group (group 1) in the GM, compared with formal normative indicators, the number of Lactobacillus spp., Bifidobacterium spp., B. thetaiotaomicron was reduced (p<0.05) and indicators of the total bacterial mass and Enterobacter spp./Citrobacter spp. were increased (p<0.05). 2. In subjects with MHO (group 2), GM changes similar to group 1 were observed. However, in comparison to group 1, Klebsiella spp. and Proteus spp. were recorded in feces in quantities exceeding the formal regulatory. In patients with MUHO (group 3), in addition to changes detected in group 2, C. difficile was found in feces, as well as a significant (p <0.05) decrease in F. prausnitzii and an increase (p <0.05) in the detection frequency of banal E.coli, as well as the more diverse composition of the microbiota. Thus, the data obtained as a result of a pilot study certainly indicate changes in the GM in people with different phenotypes of obesity and in healthy ones. Background. Thisstudy aimed to compare the effect of one and two sessions of antimicrobial photodynamic therapy (aPDT) as an adjunct to scaling and root planing (SRP) on clinical and microbial parameters in patients with chronic periodontitis. Methods. This study was conducted on 20 patients. The dental quadrants of patients were randomly assigned to SRP at baseline (group 1), SRP at baseline and one month (group 2), SRP plus aPDT at baseline (group 3) and SRP plus aPDT at baseline and one month (group 4). Probing depth (PD), clinical attachment level (CAL) gain, and bleeding on probing (BoP) were measured at baseline, and one and three months later. F. nucleatum counts were determined by PCR. ANOVA was used for the comparison of these variables between the groups. Results. In all the groups, PD reduction and CAL gain increased significantly at 1- and 3-month intervals compared to baseline (P=0.001). At three months, the difference in PD between groups 1 and 3 was statistically significant (P=0.014). CAL gain between groups 2 and 4 at one month (P=0.016) and three months (P=0.001) wasstatistically significant. Reduction in F. nucleatum counts was not significant between the four study groups (P>0.05). Conclusion. A combination of two sessions of aPDT and SRP could improve CAL gain; however, further long-term studies are necessary in this regard.

AbstractThe current understanding of the pathogenesis of periodontal disease has resulted in adjunctive use of various pharmacologic agents in periodontal therapy. The objective of this investigation was to assess the efficacy of atorvastatin and simvastatin (because of their pleiotropic properties) as an adjunct to dental scaling and root planing (SRP) by local delivery, i.e. placing them subgingivally, in the treatment of chronic periodontitis.Local delivery systems for atorvastatin and simvastatin were prepared in sodium alginate suspension to be administered with calcium chloride solution. Patients diagnosed with chronic periodontitis were grouped as group 1, receiving SRP only (control), group 2, receiving SRP with subgingival delivery of 1.2% simvastatin, and group 3, receiving SRP with subgingival delivery of 1.2% atorvastatin. Clinical parameters and interleukin (IL) 1α levels in the gingival crevicular fluid (GCF) were assessed.All three groups showed significant reductions in clinical parameters and IL-1α levels in the GCF (p<0.05). However, the test groups did not show any statistically significant difference when compared with control.Subgingivally delivered atorvastatin and simvastatin as an adjunct to SRP is efficacious but did not demonstrate any added benefit as compared with SRP alone.

AbstractThe present clinical trial aimed to assess the effectiveness of antimicrobial photodynamic therapy versus Aloe vera gel as an adjunct to scaling and root planing on periodontal and microbial outcomes in patients with periodontitis. Eligible patients undergoing nonsurgical periodontal treatment were divided into 3 groups: group 1: antimicrobial photodynamic therapy; group 2: Aloe vera gel application; and group 3: scaling and root planing only. Clinical periodontal variables included the assessment of plaque scores, bleeding on probing, probing depth, and clinical attachment level gain. Plaque samples were collected to estimate microbial counts of Tannerella forsythia (T. forsythia) and Porphyromonas gingivalis (P. gingivalis). All measurements were recorded at baseline, 3 mo, and 6 mo. Statistical analysis of the given data was performed using the chi-squared test and ANOVA for clinical periodontal and microbiological data. Eighty-seven patients completed the trial. Bleeding on probing showed a significant reduction in group 2 compared with the other groups (p < 0.001). Group 1 showed a statistically significant reduction in probing depth and gain in clinical attachment level when compared to group 2 and group 3 (p < 0.05). Group 1 showed a statistically significant reduction in the counts of T. forsythia and P. gingivalis over a period of 3 mo (p < 0.05). The reduction was seen for T. forsythia only following 6 mo (p < 0.05). Group 2 showed a significant reduction for only T. forsythia at 3 mo (p < 0.05). Both antimicrobial photodynamic therapy and Aloe vera gel helped in reducing periodontal inflammation. Aloe vera gel showed additional benefit in reducing bleeding scores, while antimicrobial photodynamic therapy showed additional enhanced activity against periodontal pathogens and periodontal attachment level gain.

Purpose. To evaluate the histopathological condition of the pulp in teeth with different levels of chronic periodontitis in humans. Methods. Twenty-five single-root nondecayed teeth were divided into three groups as follows: group 1, clinical attachment level (CAL) 3 to 4 mm and alveolar bone loss (BL) from 4 to 6 mm without reaching the tooth apex; group 2, CAL≥5 mm and BL>6 mm without reaching the tooth apex; group 3, CAL≥5 mm and BL>6 mm up to the tooth apex. Histological analyses were accomplished after laboratorial processing. Results. The mean of CAL was 3.2±0.7 mm in group 1, 7.6±2.0 mm in group 2, and 12.1±2.8 mm in group 3, while for BL it was 4.8±0.9 mm, 7.6±2.2 mm, and 11.9±2.1 mm, respectively. Histopathological data in the pulpal chambers were similar among the three groups showing normal aspects, and, the radicular pulps showed variable levels of reactive dentin, fibrosis, dystrophic mineralizations, atrophy, and mononuclear inflammatory infiltrate. Conclusions. Gradual progression of the chronic periodontitis led to changes in the histopathological aspects of the radicular pulp with progressive involvement.

Background: SDF-1α may be involved in the immune defense pathway activated during periodontal disease. Upon development of disease, SDF-1α levels increase and may recruit host defensive cells into sites of inflammation. This suggests that SDF-1α may be a useful biomarker for the identification of periodontal disease progression. Previous studies suggested that diode laser using as an adjunct to SRP in clinical and biochemical benefits. Aims and objectives: The aim of the present study was to assess the effect of diode laser on gingival crevicular fluid stromal derived factor – 1 alpha (SDF-1α) level in chronic generalized periodontitis patients.  Materials and methods: A total of 30 patients were divided in two groups based on inclusion and exclusion criteria as follows: Group 1:  15 chronic generalized periodontitis patients who received treatment with scaling and root planing. Group 2: 15 chronic generalized periodontitis patients who received treatment with scaling and root planing and diode laser. Each GCF samples were collected at baseline, 15 days and 21 days after treatment. Clinical parameters like gingival index, probing pocket depth and clinical attachment level were recorded and GCF samples also collected. Results: All the clinical parameters (GI, PPD, CAL) and the concentration of SDF-1α level in GCF were found to be significantly reduced in the group 2 compared to group 1 after 21 days. The mean values of GI, PPD, CAL parameters at different time intervals was found statistically significant in both the groups (p<0.001). Conclusion: Within the limitations of the present study, we conclude that diode laser used as an adjunct to SRP provides greater improvement in clinical and biochemical benefits over conventional mechanical treatment during medium term of observation.

Visfatin is a pleiotropic mediator which acts as growth factor, cytokine, enzyme involved in energy including nicotinamide adenine dinucleotide metabolism and has been recently demonstrated to exert several pro-inflammatory functions. The purpose of this study is to evaluate the Visfatin concentration in gingival crevicular fluid (GCF) and serum in patients with chronic periodontitis, and to evaluate the effect of non-surgical periodontal therapy on the GCF and serum visfatin concentration. 30 subjects (age range: 25 to 52 years) were selected and divided into two groups based on the gingival index, probing depth, periodontal attachment level, and radiologic parameters (bone loss): group 1 (15 subjects with healthy periodontium), group 2 (15 subjects with chronic periodontitis), while, Group 2 patients after 8 weeks of the treatment (scaling and root planning, SRP) constituted group 3. GCF samples (by microcapillary pipettes) and serum samples (by venipuncture) were collected to estimate the levels of Visfatin using enzyme linked immunosorbent assay kit. The mean Visfatin concentration in GCF and serum was observed to be the highest in group 2 and lowest in group 1. While concentration in group 3 was similar to group 1. The concentration of Visfatin in GCF and serum decreased after SRP. The Visfatin concentration in GCF and serum found to be highest in chronic periodontitis group and decreases after treatment. Hence Visfatin values can be considered as an “inflammatory marker” can be explored in future as a potential therapeutic target in the treatment of periodontal disease.
