Coccidioidomycosis, also known as San Joaquin Valley Fever or Valley Fever, is mostly a pulmonary infection caused by inhalation of spores in an endemic region. Dissemination to bone, joints, meninges, and skin occurs less than one percent of the time. Skeletal involvement accounts for approximately half of the disseminated coccidioidomycosis with the vertebrae as the most common skeletal region. We present a very rare case of disseminated coccidioidomycosis with osteomyelitis and compression fracture of the lumbar vertebral body. This case depicts some of the potential issues that can arise in managing coccidioidomycosis, especially when noncompliance to initial azoles occurs, that can lead to dissemination and complicated bone infections necessitating surgical intervention along with continuous medical therapy.

Abstract Background Severe tricuspid regurgitation (TR) is a complex condition that can be difficult to treat medically, and often surgical intervention is prohibited due to the high morbidity and mortality associated with this intervention. In patients who have failed maximal medical therapy and have progressive symptoms related to their severe TR, heterotopic caval valve implantation (CAVI) offers potential for symptom relief for these patients. Case summary We present two cases of patients with severe TR with symptoms of heart failure that were refractory to medical therapy. Due to extensive comorbidities in these patient’s surgical intervention was deemed unsuitable and the decision was made to proceed with heterotopic CAVI in order to try and control their symptoms. Both patients successfully underwent the procedure and had an Edwards SAPIEN 3 valve (Edwards Lifesciences, Irvine, CA, USA) implanted in the inferior vena cava/right atrium junction. In both patients, there was improvement in the postoperative haemodynamics as measured by invasive and non-invasive methods. Successful discharge was achieved in both patients with improvement in their symptoms. Discussion Selective use of heterotopic CAVI to treat symptomatic severe TR that is refractory to medical therapy may be a viable option to improve symptoms in those patients that are unsuitable for surgical intervention.

Tumoral calcinosis is a rare diagnosis characterized by deposition of calcium salts in peri-articular soft tissue regions. It is divided into primary and secondary varieties. The primary tumoral calcinosis is further divided into two types; primary hyperphosphatemic type and primary normophosphatemic type. The secondary variety occurs in association with chronic renal failure. Biochemical assessment and typical radiographic features help in diagnosis. Mainstay of treatment for primary variety is surgical. Secondary variety is mainly treated by medical measures. Surgical intervention is reserved for patients who do not respond to medical therapy.

Mesh Hernioplasty is the gold standard for Inguinal hernia.However, it is not free of complications. Mesh migration causing intestinal obstruction, albeit rare, is a serious and complications. Timely surgical intervention is very important in the management of this condition. We report a rare case of an elderly male patient with mechanical bowel obstruction due to mesh migration 9 years after a right inguinal hernia meshplasty.

AbstractPrimary osteosarcoma arising in breast is a rare form of extra skeletal osteosarcoma, accounting for less than 1% of all breast cancers and 12.5% of sarcomas arising in breast. Fewer than 150 cases of primary osteosarcoma of breast have been reported in the medical literature in the last 50 years. It was reported to have worse outcome compared to typical skeletal osteosarcoma. Osteosarcoma of breast presents in elderly age group unlike skeletal osteosarcoma, which occurs in young people. Diagnosis is by histopathological examination of malignant osteoid and ruling out underlying skeletal involvement.

Symptomatic intracranial arterial disease is associated with a high rate of recurrent ischemic events. The management of this condition is controversial, with some advocating medical therapy as a sole means of treatment and others recommending endovascular therapy in addition to best medical management. In rare cases, surgical intervention is considered. A thorough review of the available literature was performed, and treatment recommendations based on these data are provided.
