Cystic lymphangiomas are benign tumors originating mainly in the head and neck of the pediatric population. The authors report a rare case of cystic lymphangioma in the right elbow of an 8-month-old baby treated successfully by complete surgical resection.

Abstract Background Primary cardiac tumours are extremely rare. Most of them are benign. Sarcomas account for 95% of the malignant tumours. Prognosis of primary cardiac angiosarcoma remains poor. Complete surgical resection is oftentimes hampered when there is extensive tumour involvement into important cardiac apparatus. We report a case of cardiac angiosarcoma of the right atrium and ventricle, infiltrating the right atrioventricular junction and tricuspid valve. Case presentation Initially, a 22-year-old man presented with dyspnoea. One year later, he had recurrent pericardial effusion. Afterwards, echocardiography revealed a large mass in the right atrium, expanding from the roof of the right atrium to the tricuspid valve. The mass was causing compression on the tricuspid valve, and another mass was seen in the right ventricle. Complete resection of the tumour was impossible. The mass was resected with the biggest possible margins. The right atrium was reconstructed using heterologous pericardium. The patient’s postoperative course was uneventful. Postoperative echocardiography showed a small mass remaining in the right side of the heart. Histopathology and immunohistochemistry confirmed the diagnosis of angiosarcoma. The patient underwent adjuvant chemotherapy and radiotherapy later on. He survived for 1 year and 5 days after the surgery. After a diagnosis of lung and brain metastases, he ended up on mechanical ventilation for 48 h and died. Conclusions Surgical resection combined with postoperative chemotherapy and radiotherapy is feasible even in patients with an advanced stage of cardiac angiosarcoma when it is impossible to perform complete surgical resection.

lymphangiomas are benign tumors caused by the congenital proliferation of lymphatic tissue, causing an obstruction in the lymphatic ducts, producing fluid filled cysts (1). Histologically they are polycystic, with cysts divided by thin septas lined with endothelial cells. Most common presentation of cystic lymphangiomas are in the neck (75%) and axillary region (20%) and only 5% are intra-abdominal, while retroperitoneal lymphangiomas account for only 1% of cases (2). It is more common in children with males affected more than women, while women have shown to encounter pancreatic lymphangiomas more than men (3). Often retroperitoneal lypmhangiomas are asymptomatic but due to their large size they might cause a wide variety of symptoms from mild abdominal distention/discomfort and back pain to duodenal obstruction, sepsis and DIC (4). Diagnostic techniques are ultrasound, CT scan and MRI but often it is found accidentally and definitive diagnosis is made post-operatively. Treatment of choice is complete surgical resection with cystectomy

Lymphangiomas are uncommon benign tumours and occur mainly in children with the most common sites being head and neck followed by the axilla and mediastinum. Intra-abdominal lymphangiomas are rare. Clinical presentation is diverse ranging from incidentally discovered abdominal mass to symptoms of acute abdomen. Life threatening complications are more likely to develop in children. We here present a rare case of giant mesenteric cystic lymphangioma causing intra-abdominal catastrophe in an adult.

Background. Teratomas in the pediatric population are most commonly found in the sacrococcygeal region. Pediatric intraspinal teratomas, however, are an exceedingly rare central nervous system (CNS) neoplasm. The clinical presentation of these intraspinal neoplasms can vary significantly and thus can be difficult to identify in infants less than one year of age where verbal expression and motor development are still lacking. Case Description. A 7-month-old, previously healthy male presented with a thoracic scoliosis and an asymptomatic right midupper thoracic spinal prominence present since birth. MRI revealed an extensive heterogenous mass in the right epidural space from T5-T6 and the right paravertebral space, resulting in severe spinal stenosis. Outcome. Complete resection of the tumor, including a three-level neurotomy, was achieved by posterior decompression/laminectomy. The final tumor was consistent with a mature teratoma. The surgical resection was performed without any immediate complications. Conclusions. Extramedullary epidural teratomas are exceptionally rare tumors in the pediatric population. Clinical presentation can be ambiguous, particularly in an infant. MRI was useful in suggesting a teratoma as a potential diagnosis and for postoperative surveillance for recurrence. However, histopathological analysis remains the gold standard for definitive diagnosis. Surgical resection is the mainstay of treatment, especially in the setting of cord compression and progressive loss of motor function. Close follow-up is crucial to monitor for progressive spinal deformity or recurrence.

Purpose: To report an unusual case of frontal sinus keratocyst. Case Report: A 31 year old woman presented with a history of swollen right eye and visual field impairment. The ophthalmological examination revealed right superior eyelid edema and some degrees of proptosis. CT scan with iodine injection evidenced the opacification of the right frontal sinus with superior orbital wall lysis. Complete endoscopic surgical resection of a mass containing foul smelling keratin material was performed via Draf type III. The patient became symptomatic after 6 years. Imaging studies showed neo- osteogenesis in the far lateral end of right frontal sinus. Frontal trephination was done and the remnant of keratocyst was removed and the septum interfacing the cell and the right frontal sinus was walled down entirely. Conclusion: Although frontal sinus keratocyst is benign but can spread to the surrounding structures leading to several complications. Therefore, complete surgical resection is necessary to avoid recurrence.

Juvenile xanthogranuloma (JXG) is a relatively uncommon, benign, histiocytic proliferative cutaneous disorder that typically affects children, with the head and neck being the most common sites. The present case report describes an isolated subcutaneous JXG in a 4-year-old girl who presented with a circumscribed oval mass located in the lower eyelid of the right eye. This lesion was histologically diagnosed as JXG after a surgical resection of the mass.

Abstract Background Tailgut cysts arise from the remnants of the tailgut during the embryonic period. Although malignant transition of tailgut cysts is very rarely observed in the clinic, this congenital condition should be carefully monitored for early diagnosis and appropriate treatment, especially when the tailgut cysts are malignant. Case presentation Here, we report the case of a 33-year-old man with retrorectal adenocarcinoma originating from the tailgut cysts. Magnetic resonance imaging (MRI) showed many cystic masses in the posterior rectal space, the largest of which was approximately 100 mm × 59 mm × 53 mm in size and compressed the rectum. The patient underwent surgical resection of the masses located in the retrorectal and anterior sacral spaces. Histological and immunohistological examinations confirmed adenocarcinoma transition of the tailgut cysts. The patient recovered well and was discharged 10 days after surgery. Conclusions We have reported a rare case of retrorectal adenocarcinoma originating from tailgut cysts. MRI, histological, and immunohistological examinations are vital for the diagnosis of tailgut cysts. Complete surgical resection of the tumor should be better performed.

Hemangiomas of the head and neck region comprise about 60 to 70% of all benign tumors. Intramuscular hemangioma is a rare, slow-growing, angiomatous tumor. We report a rare case of an Intramuscular Hemangioma of Right sternocleidomastoid muscle in a six years old girl presenting for four years and with extensive involvement necessitating excision. Microscopic excision reduces the risk of recurrence.

We present a case of cystic lymphangioma of the mediastinum complicated with situs inversus totalis. The 70-year-old man underwent thoracoscopic resection of a mediastinal cystic tumor, which was diagnosed as cystic lymphangioma. Cystic lymphangiomas are congenital cystic abnormalities of the lymphatic system. The head and neck area is often involved while the mediastinum is rarely affected. The rarity of this case is further attributed to the coexistence of situs inversus totalis.
