The success of dental implant therapy in the esthetic zone requires not only functional osseointegration but also a satisfactory esthetic outcome. To establish harmony, balance, and continuity of gingival architecture between an implant restoration and the adjacent natural dentition is challenging. Immediate implant placement and provisionalization following tooth extraction have been documented as a predictable treatment modality, with fewer surgical interventions needed, to replace a missing tooth in the esthetic zone. This case report illustrates immediate implant placement and provisionalization to replace a failing maxillary right central incisor while maintaining optimal gingival esthetics. The maxillary right central incisor was extracted without flap elevation to minimize soft and hard tissue trauma. Immediately afterwards, the implant was installed using a surgical stent and restored with a provisional crown that had no occlusal contacts. During healing, no significant adverse effects were observed clinically or radiographically. This proposed treatment modality provided the patient with immediate esthetics, function, and comfort without any complications during a follow-up period of 6.5 years.

INTRODUCTION: Immediate dental implants have greatly reduced the treatment time and the number of surgical interventions. Recently it has been noted that this treatment modality can be used in aesthetically demanding cases especially the anterior maxilla. CASE REPORT: In the present case report a 24 year old male patient reported to our institution with fractured upper front teeth. After careful examination and treatment planning immediate implant treatment was initiated. The teeth were extracted atraumatically. We placed three implants into the extraction sockets. The prosthetic rehabilitation was done with metal ceramic crowns. CONCLUSION: It was found that the immediate implant therapy has several advantages such as reduced treatment length, preservation of soft and hard tissues surrounding the implant and reduced number of operations. Immediate implant treatment therefore has a great future in the treatment of aesthetic zones.

Background: Immediate implant placement and restoration (IPR), is a reliable treatment modality. Purpose: This historical prospective study evaluated the medium-term outcomes of hard tissue after IPR in the anterior maxilla with simultaneous hard tissue augmentation. Methods: Seventy-three patients treated with single-implant IPR in the anterior maxilla were followed for 1-8 years. Treatment involved, atraumatic extraction, immediate implant placement and abutment adaptation, followed by simultaneous augmentation with mineralized freeze-dried bone allograft (FDBA) particles to fill the gaps and restore the ridge. The surgical site was stabilized with a resorbable collagen membrane, followed by the connection of an acrylic provisional restoration. Results: All implants osseointegrated during the follow-up period (mean, 34 ± 22 months). Radiographic evaluation of the distance between the implant shoulder (IS) and crestal bone level (CBL) was of 0.86 ± 0.86 mm and 0.8 ± 0.84 mm mesially and distally, respectively. Splitting the results into up to 3 years and 3–8 years of follow-up data, the corresponding values were 0.90 ± 0.83 and 0.68 ± 0.88 for the mesial aspect and 0.99 ± 0.87and 0.74 ± 0.83 for the distal aspect, respectively. Mean peri-implant probing depth was 3.63 mm (SD ± 1.06) and 16 implants (22%) presented at least one bleeding pocket of ≥5 mm (peri implant mucositis). Conclusions: The immediate replacement of a single maxillary tooth by implants combined with guided bone regeneration is a predictable treatment modality with favorable peri-implant bony response.

Traumatic oral injuries present their own unique restorative challenges to the clinician due to their variable clinical presentation. In such cases, achieving optimum aesthetics and function in the maxillary aesthetic zone is challenging. This case report describes the rehabilitation of a patient with immediate implant placement after suffering trauma to the oral cavity resulting in a complete loss (avulsion) of a permanent central incisor tooth. In order to preserve the existing soft and hard tissue and to achieve predictable and aesthetically pleasing results, we decided to place an immediate implant as opposed to replantation of the avulsed tooth. The placement of immediate implants in the posterior sector is a widespread procedure where the success and survival rates are similar to those of traditional protocols. It has several anatomical challenges, such as the presence of interradicular bone septa that hinder a correct three-dimensional positioning of the implant and may compromise primary stability and/or cause damage of neighboring structures. The aim of this article is to present the treatment and the one-year clinical follow-up of a patient who received immediate implant placement using an interradicular bone-drilling technique before the molar extraction.

Summary Implant placement in the esthetic zone is a complex procedure and requires a restoration-driven approach. Proper selection of patients and implant together with individual assessment of the risk of esthetic complications are very important. Correct 3D-implant positioning and sufficient bone volume should provide long-term esthetic and function. Esthetic region is a zone in which expectations and possibilities collide. Clinician should bring the important decision on the appropriate time of implant placement. Immediate implant placement is particularly challenging in the esthetic zone. Patient desire for reduced treatment time should be weighed against the possible risk factors. Protocol of immediate implant placement in conditions of unfavourable gingival biotypes, the lack of bone or soft tissue in patients with a high smile line lead to esthetic failure which is very important in the esthetic region.

Background. The incisive canal located at the midline, posterior to the central incisor, is an important anatomic structure of this area to be considered while planning for immediate implant placement in maxillary central incisor region. The purpose of the present study is to assess incisive canal characteristics using CBCT sections. Materials and Methods. CBCT scans of 79 systemically healthy patients, with intact maxillary incisors, were evaluated by two calibrated and independent examiners. Assessments included (1) mesiodistal diameter, (2) labiopalatal diameter, (3) length of the incisive canal, (4) shape of incisive canal, and (5) width of the bone anterior to the incisive foramen. Results. The mean width of the foramen labiopalatally and mesiodistally was 3.12 ± 0.94 mm and 3.23 ± 0.98 mm, respectively. Mean canal length was 18.63 ± 2.35 mm and males have significantly longer incisive canal than females. The mean width of bone anterior to the incisive canal was 6.32 ± 1.43 mm. As age of the subjects increased, incisive foramen diameter and incisive canal length were found to be increased. Cylindrical shaped incisive canals were seen in most of the individuals followed by funnel shaped and hour-glass shaped canals, and banana-like canal is least prevalent type. Conclusion. The findings from the present study suggest that the diameter and length of incisive canal vary among different individuals and presence of very thin bone anterior to the canal would suggest that a pretreatment CBCT scan is a valuable tool to evaluate anatomic variations, morphology, and dimensions of incisive foramen before immediate implant placement in maxillary central incisor region.

SUMMARYObjective: The aim of this study was to evaluate the outcome of immediate postextraction implant placement. Immediate placement of dental implants have been claimed of the potential advantages such as reductions in the number of surgical interventions, a shorter treatment time, an ideal 3-dimensional implant positioning, the presumptive preservation of alveolar bone at the site of the tooth extraction and soft tissue aesthetics.Method: In this case series we reported to extract 15 teeth in 12 patients (8 males; 4 females, mean age: 46.08 years) and replace the teeth with implants immediately.Results: There were no signs of inflammation or infection and none of the patients had complaints subsequently. All implants were osseointegrated at the time of abutment connection. Postoperative healing was uneventful in all of the patients. No complications were observed.Conclusion: Within the limits of the present study, immediate implant placement was a predictable treatment.
