Abstract. Many fields, such as drought-risk assessment or reservoir management, can benefit from long-range streamflow forecasts. Climatology has long been used in long-range streamflow forecasting. Conditioning methods have been proposed to select or weight relevant historical time series from climatology. They are often based on general circulation model (GCM) outputs that are specific to the forecast date due to the initialisation of GCMs on current conditions. This study investigates the impact of conditioning methods on the performance of seasonal streamflow forecasts. Four conditioning statistics based on seasonal forecasts of cumulative precipitation and the standardised precipitation index were used to select relevant traces within historical streamflows and precipitation respectively. This resulted in eight conditioned streamflow forecast scenarios. These scenarios were compared to the climatology of historical streamflows, the ensemble streamflow prediction approach and the streamflow forecasts obtained from ECMWF System 4 precipitation forecasts. The impact of conditioning was assessed in terms of forecast sharpness (spread), reliability, overall performance and low-flow event detection. Results showed that conditioning past observations on seasonal precipitation indices generally improves forecast sharpness, but may reduce reliability, with respect to climatology. Conversely, conditioned ensembles were more reliable but less sharp than streamflow forecasts derived from System 4 precipitation. Forecast attributes from conditioned and unconditioned ensembles are illustrated for a case of drought-risk forecasting: the 2003 drought in France. In the case of low-flow forecasting, conditioning results in ensembles that can better assess weekly deficit volumes and durations over a wider range of lead times.

Objective: Ability of the brain to recover after an acute ischemic stroke (AIS) is linked to the pre-stroke burden of white matter hyperintensity (WMH), a radiographic marker of brain health. We sought to determine the excessive WMH burden in an AIS population and investigate its association with 3-month stroke outcomes. Data: We used 2,435 subjects from the MRI-GENIE study. Three-month functional outcomes of 872 subjects among those subjects were measured by 90-day modified Ranking Scale (mRS). Methods: We automatically quantified WMH volume (WMHv) on FLAIR images and adjusted for a brain volume. We modeled a trend using the factor analysis (FA) log-linear regression using age, sex, atrial fibrillation, diabetes, hypertension, coronary artery disease and smoking as input variables. We categorized three WMH burden groups based on the conditional probability given by the model (LOW: lower 33%, MED: middle 34%, and HIGH: upper 33%). The subgroups were compared with respect to mRS (median and dichotomized odds ratio (OR) (good/poor: mRS 0-2/3-6)). Results: Five FA components out of seven with significant relationship to WMHv (p<0.001) were used for the regression modeling (R 2 =0.359). The HIGH group showed higher median (median=2, IQR=2) mRS score than LOW (median=1, IQR=1) and MED (median=1, IQR=1). The odds (OR) of good AIS outcome for LOW and MED were 1.8 (p=0.0001) and 1.6 (p=0.006) times higher than HIGH, respectively. Conclusion: Once accounted for clinical covariates, the excessive WMHv was associated with worse 3-month stroke outcomes. These data suggest that a life-time of injury to the white matter reflected in WMH is an important factor for stroke recovery and an indicator of the brain health.

AbstractGrowing evidence suggests dementia incidence can be reduced through prevention programs targeting risk factors. To accelerate the implementation of such prevention programs, a new generation of brain health services (BHS) is envisioned, involving risk profiling, risk communication, risk reduction, and cognitive enhancement. The purpose of risk communication is to enable individuals at risk to make informed decisions and take action to protect themselves and is thus a crucial step in tailored prevention strategies of the dementia incidence. However, communicating about dementia risk is complex and challenging.In this paper, we provide an overview of (i) perspectives on communicating dementia risk from an ethical, clinical, and societal viewpoint; (ii) insights gained from memory clinical practice; (iii) available evidence on the impact of disclosing APOE and Alzheimer’s disease biomarker test results gathered from clinical trials and observational studies; (iv) the value of established registries in light of BHS; and (v) practical recommendations regarding effective strategies for communicating about dementia risk.In addition, we identify challenges, i.e., the current lack of evidence on what to tell on an individual level—the actual risk—and on how to optimally communicate about dementia risk, especially concerning worried yet cognitively unimpaired individuals. Ideally, dementia risk communication strategies should maximize the desired impact of risk information on individuals’ understanding of their health/disease status and risk perception and minimize potential harms. More research is thus warranted on the impact of dementia risk communication, to (1) evaluate the merits of different approaches to risk communication on outcomes in the cognitive, affective and behavioral domains, (2) develop an evidence-based, harmonized dementia risk communication protocol, and (3) develop e-tools to support and promote adherence to this protocol in BHSs.Based on the research reviewed, we recommend that dementia risk communication should be precise; include the use of absolute risks, visual displays, and time frames; based on a process of shared decision-making; and address the inherent uncertainty that comes with any probability.

Abstract. Haze has been severely affecting the densely populated areas in China during recent years. While many of the pilot studies have been devoted to investigate the contributions from local anthropogenic emission, limited attention has been paid to the influence from long-range transport. In this study, we use simulations from 6 participating models supplied through the Task Force on Hemispheric Transport of Air Pollution Phase 2 (HTAP2) exercise to investigate the long-range transport impact of Europe and Russia/Belarussia/Ukraine on the surface air quality in East Asia, with special focus on their contributions during the haze episodes over China. The impact of 20 % anthropogenic emission perturbation from the source region is extrapolated by a factor of 5 to estimate the full impact. We find that the full impacts from EUR and RBU are 0.99 µg/m3 (3.1 %) and 1.32 µg/m3 (4.1 %) respectively during haze episodes, while the annual averaged full impacts are only 0.35 µg m3 (1.7 %) and 0.53 µg/m3 (2.6 %) respectively. By estimating the aerosol response within and above the planetary boundary layer (PBL), we find that long-range transport within the PBL contributes to 22–38 % of the total column density of aerosol response. Comparison with the HTAP Phase 1 (HTAP1) assessment reveals that from 2000 to 2010, the long-range transport from Europe to East Asia has decreased significantly by a factor of 2–10 for surface aerosol mass concentration due to the simultaneous emission reduction in source region and emission increase in the receptor region. By investigating the visibility response, we find that the long-range transport from the Europe and RBU region increases the number of haze events in China by 0.15 % and 0.11 % respectively, and the North China Plain and southeast China receives 1–3 extra haze days. This study is the first investigation into the contribution of long-range transport to haze in China with multiple model experiments.
