We tested the effects of seeding date and weed control during switchgrass establishment in a field experiment that was conducted in central Pennsylvania in 2007 and repeated in 2008. Switchgrass was no-till seeded in early May, late May, and mid-June, and three postemergence weed management treatments were evaluated, including Mow (only a single mowing), Broadleaf (2,4-D + dicamba), and Broad Spectrum (2,4-D + dicamba + atrazine + quinclorac). Switchgrass density increased at later seeding dates, except in 2008, when the middle seeding date had the lowest density. In both years, weed biomass in late summer was lowest in the last seeding date of the Broad Spectrum treatment. In contrast, switchgrass biomass in late summer was greatest in the first seeding date of the Broad Spectrum treatment in both years. In the year after establishment (production year), plots were split to test the effects of supplemental weed control, composed of metsulfuron + 2,4-D applied in May, on total aboveground yield. Supplemental control in the production year increased total aboveground yield in the Mow treatment only, indicating that effective weed control during the establishment year might reduce the need for weed control in the following year. Although maximum aboveground yield was achieved when switchgrass was seeded in May and herbicides were used, results from our experiment suggest that seeding switchgrass at a relatively high seeding rate in June in our study region and mowing annual weeds to reduce competition and prevent seed production could be an effective strategy if minimizing herbicide use is a priority.

The aim of this paper is to stay a very rare umbilico-inguinal fistula (UIF) resulting from a delayed suture reaction after the use of silk suture to repair an inguinal hernia. A 3-year-old boy presented with persistent umbilical discharge. The initial diagnosis was omphalitis and he was treated with broad-spectrum antibiotics but a UIF was subsequently diagnosed. Surgery was performed to ascertain the cause of the UIF. This case demonstrates that silk suture used in inguinal hernia repair can lead to a UIF, which should be considered in the differential diagnosis of a patient presenting with persistent umbilical discharge.

We present a case of acute hemolytic anemia, renal failure, andClostridium perfringensbacteremia in a patient with acute myelogenous leukemia. The high fatality ofC. perfringensbacteremia requires that clinicians recognize and rapidly treat patients at risk for this infection. Although other hemolytic processes are in the differential diagnosis of these events, the presence of high fever, chills, and rapidly positive blood cultures may help narrow the diagnosis. Most cases ofC. perfringensbacteremia have a concomitant coinfection, which makes broad spectrum empiric therapy essential. There is a high mortality rate ofC. perfringensinfections associated with leukemia.

Ocular auscultation is a commonly neglected step of routine physical examination. An adequate ocular auscultation can be helpful in discovering an ocular bruit, which is an important diagnostic finding for a broad spectrum of pathologic conditions, some of which are potentially fatal. In this article, we present a literature review on the physical exam maneuver of ocular auscultation, as well as the pathophysiology and differential diagnosis of ocular bruits. We also included a description of the adequate auscultation technique and a discussion about the applicability of ocular auscultation in clinical practice

Although simple liver cysts are common, complex cystic liver lesions are infrequent and represent a diagnostic and therapeutic challenge. The differential diagnosis of complex cystic liver lesions can be grouped into neoplastic, infectious or inflammatory, and miscellaneous pathologic entities. Clinicians should remember to consider mucinous cystic neoplasm and echinococcal cysts in the differential, which are uncommon etiologies for liver lesions but may expose unique challenges. We present a case of a 49-year-old female who was referred for evaluation of a new complex cystic liver lesion. The following brief review describes how radiologic imaging and pathologic testing can help distinguish between the broad spectrum of diseases that may produce cystic liver lesions.

Abstract Unilateral laterothoracic exanthem (ULE), or asymmetric periflexural exanthem of childhood (APEC), is an uncommon skin eruption that usually occurs in childhood, with unilateral distribution and self limiting course. The etiology of ULE is unknown, but viral cause is suspected. We report a case of ULE in a 4-year-old girl, that was associated with parvo virus B19 infection, and a brief selected literature review. The patient presented with unilateral maculopapular rash on the left side of the body which was asymptomatic and resolved spontaneously within 5 weeks. The clinical diagnosis of ULE may be precise, ruling out a broad spectrum of differential diagnosis, and prevent unnecessary examinations, whereas the patient is informed about the benign self-limiting nature of ULE.
