Objectives: To quantify differences in waste and cost of disposable equipment between different tonsillectomy techniques. Methods: Prospective study of waste attributable to disposable waste produced by tonsillectomy surgery. Disposable equipment required for tonsillectomy using cold, monopolar electrocautery (ME), and coblation techniques was measured; and differences in mass, volume, and cost of equipment between the 3 techniques were quantified. Results: Cold technique was found to produce the least waste and have the lowest cost attributable to disposable surgical equipment. Projected single-case savings in mass and volume of waste resulting from using cold technique compared to ME were 1.272 kg and 1.013 L, respectively, and 1.043 kg and 1.723 L compared to coblation. Projected single-case savings in cost of disposable equipment for cold technique compared to ME were US$9.35 and US$185.05 compared to coblation. Discussion: Using cold technique for adult tonsillectomy reduces waste and cost of disposable equipment compared to ME and coblation. Implications for Practice: Surgeons desiring to reduce cost and waste associated with tonsillectomy surgery may consider transitioning to cold technique.

The cause of multiple sclerosis (MS) remains unknown. It is largely regarded as being an inflammatory autoimmune disease and has been reported in association with other inflammatory/autoimmune diseases. We performed a prospective study in 658 consecutive patients diagnosed with MS attending our outpatient MS management clinic between June 2002 and June 2003. Prevalence of associated conditions in these patients was calculated and compared with values from population studies using chi-square tests, odds ratios and confidence intervals. The MS population had significantly increased rates of asthma, inflammatory bowel disease, type I diabetes mellitus, pernicious anaemia, autoimmune thyroid disease, uveitis, seronegative spondyloarthropathies, bipolar disorder and melanoma compared to the general population. Both T helper type 1 (Th1)-mediated and T helper type 2 (Th2)-mediated diseases were significantly increased compared to the general population. There were also interesting associations seen with polyglandular autoimmune syndrome and rare single case associations. MS is associated with several other conditions. This work does not give evidence for the hypothesis that MS and atopy, reflecting Th1 and Th2 polarization, respectively, are mutually exclusive. Further work, ideally with a matched control population, is indicated.

Objective: To report preliminary findings of a phase II study exploring the clinical outcomes of moderate hypofractionated radiotherapy performed with helical tomotherapy (HT) using computed tomography–magnetic resonance imaging–based planning for localized prostate cancer. Methods: The phase II prospective study received ethics approval from our institutional ethics committee. A dose of 60 Gy/20 fractions for low–intermediate risk prostate cancer by means of HT was explored. Primary endpoints of the study were acute and late gastrointestinal (GI) and genitourinary (GU) toxicities. Secondary endpoints were quality of life and biochemical-free survival. Results: A total of 35 patients were included in this interim report. At the time of the analysis, median follow-up was 36 months (range, 13–62). Acute GI toxicity was recorded as follows: grade 1 in 34% and grade 2 in 14%; acute GU toxicity was grade 1 in 71% and grade 2 in 11%. For the entire population of the study, no acute toxicities ⩾ grade 3 occurred. A single case of late grade 3 GU toxicity was registered, whereas no late GI toxicity ⩾grade 3 was recorded. At the time of the final assessment, no biochemical failure was detected. Conclusions: The preliminary results of the present phase II trial, using HT for moderate hypofractionation in localized prostate cancer, are optimal. In fact, HT guaranteed an acceptable tolerability profile with low rates of GU and GI side effects and, more specifically, no acute severe adverse events were recorded. Long-term findings are warranted.

In the current financial climate the NHS faces budget cuts. A good knowledge of the costs of the equipment used in theatres will allow more cost-efficient allocation of resources. The equipment used is dependent on the consultant surgeon leading the operation, with individual surgeons having their own preferences. For the efficient running of a department one would assume that the surgeons would have a working knowledge of the cost of equipment. This study looked at the awareness of surgeons of the cost of disposable equipment. It aimed to highlight the difference between the estimated and actual cost of disposable items, providing feedback to the surgeons so they become more aware of the real cost of their choices in order to help optimise use of equipment. This will hopefully lead to more cost-effective theatres.

Introduction. Retinopathy of prematurity is a disease of the eye, i.e. the retinal blood vessels, which occurs exclusively in premature infants. The level of blindness in one country depends on the level of development of neonatal care and the opportunities to implement screening. The aim of this study was to examine the possibilities of changing screening criteria, provided that not a single child was left out from the survey. Material and Methods. A two-year prospective study, which was carried out in the period from January 1st 2007 to December 31st 2008, included 191 premature infants who were treated at the Institute for Child and Youth Healthcare of Vojvodina. Results. Different inclusion criteria regarding body mass and gestational age were applied for screening retinopathy of prematurity and we assessed the coverage of the sample if certain screening criteria were applied. According to the results of the research, when the applied screening criterion was 37/2000, there was not a single case of a blind, prematurely born baby. Discussion. Great migrations of population as well as big differences in characteristics of premature infants together with underlying multi-factor diseases besides retinopathy of prematurity send a warning signal to be very cautious. Conclusion. Although this study has given ground to shift the limits of screening, we will adhere to broad screening criteria.

<p class="abstract"><strong>Background:</strong> Tibial plateau fractures are complex injuries of the knee. The tibial plateau is one of the most critical load-bearing areas in the human body. Early detection and appropriate treatment of these fractures are essential in minimizing patient's disability in range of movement, stability and reducing the risk of documented complications. The aim of the present study is to study the outcome of tibial plateau fractures and their management.</p><p class="abstract"><strong>Methods:</strong> This is prospective study which comprises of 50 patients with displaced tibial plateau fracture and were treated between January 2018 to December 2018 with minimal invasive percutaneous plate osteosynthesis and cortico-cancellous screw fixation. Statistical analysis was done by Chi-square test and IBM SPSS software.<strong></strong></p><p class="abstract"><strong>Results:</strong> In our study we included 50 cases, treated with surgical procedure, 30 cases gave excellent result, 16 cases came out with good result, fair in 3 cases and 1 case had poor result. High velocity injuries have poor outcome than low velocity injuries. A single case of malunion was noted in study. Four cases had knee joint stiffness.</p><p class="abstract"><strong>Conclusions:</strong> We conclude that functional outcome is good in operatively treated patients. Early physiotherapy plays key role in preventing knee stiffness, use of bone grafts and good fixation important for successful outcome.</p>

Recently environmental problem becomes a highlight, and bought out to the surface, mostly about plastic waste and how to deal with it. When plastic waste dissolved they released some kind of toxic substances that will harm the environment and bring damage to the ecosystem. In order to reduce the mass volume of plastic waste, the Indonesian Government made some regulations. One of the regulation stated that the customer had to pay for the plastic bag each time they had transactions. But this program didn’t last a long time because of some pros and cons about the regulation. PT. Lion Super Indo, or known as Super Indo Supermarket is one of the brands that applying sustainability movement as their main strategy. One of their programs is to create a competition for designing Fashion Reusable Bag in order to educate Indonesian people to get used to bring and use reusable bag everytime they shop. This research using literature studies and using descriptive qualitative as a method, divided into two phase. The first phase is the literature studies for the indicators used in this research, the elements of design and the judging aspects of the competitions. The Judging aspects are Uniqueness, Brand, Fashion, and Practicality. The next phase is a single case study with an implementation of Design Elements and Judging Aspects. Hopefully, the findings and the result of this research could help to take apart bringing more impact of the sustainability movement and give some contribution in order to reduce plastic waste volume in Indonesia.

Background: Skin adnexal tumours are a category of rare tumours. They usually present as painless nodules and papules. The diagnosis of these tumours poses a challenge, because there is usually a discrepancy in clinical and histopathological diagnosis. Aims: To study the skin biopsies received in our department and compare their clinical and histopathological diagnosis. Material and methods: Prospective study of skin biopsies received in our department over a period of 1 year. The biopsies were processed and stained with haematoxylin and eosin and studied under the light microscope. Results: Out of the 26 cases studied, 25 were found to be benign tumours with a single case of keratoacanthoma. The most common tumour encountered was pilomatricoma and the least common was syringoma. Only four cases showed clinical correlation with histopathological diagnosis. Conclusion: It can be therefore concluded that skin adnexal tumours are rare tumours which need histopathological examination for their accurate diagnosis.
