Computed Tomography and Ultrasonography in Parotid Tumors

To determine the most appropriate radiographic approach to parotid tumors, computed tomography (CT) was performed in 65 patients with 33 benign tumors, 19 malignant tumors, 3 abscesses and 10 other lesions with a clinical appearance of tumor. In 22 of the 65 cases CT-sialography and in 26 ultrasonography (US) were also performed. The mass was well recognized on CT without sialography in all patients, and the lesion was also well evaluated in all those examined with US. At CT-sialography the tumor was well shown, but this method made it difficult to differentiate a benign from a malignant lesion, because the character of the delineation and the border of the tumor were difficult to evaluate. It is concluded that US and CT without sialography are satisfactory for evaluating the parotid tumor, and the indication for CT-sialography is limited to those cases in which the tumor is demonstrated on US, but is unclear on conventional CT.

Objective Histology of parotid tumors determines the extent of surgery. The aim was to test ultrasound (US) contrast enhancer-kinetics to identify histologic entities, possibly being superior to qualitative morphological parameters. Study Design In a cross-sectional assessment of ultrasound diagnosis, the subjective US-classification was compared with contrast analysis with histology as gold standard. Subjects and Methods A total of 64 male and 61 female patients with a mean age of 54 years were included, with 13 malignant tumors. These were classified with US morphology, then time-dependent contrast medium analysis. Results A total of 92.8% of tumors were classified correctly as malignant or benign. The sensitivity, specificity, positive- and negative-predictive values were 66.7%, 86.3%, 60.6%, and 89.1% for differentiating Warthin tumors, but only 46.2%, 98.2%, 75%, and 94% for malignant lesions. Contrast parameters yielded significant parameters for benign tumors, not for malignant entities. Conclusion Although contrast medium analysis provided statistical criteria, these, however, do not possess the ability to improve the diagnostic prediction of tumor histology. Neither the morphologic classification nor contrast medium analysis was able to identify a malignant lesion sufficiently.

During 2011-2012, 56 patients diagnosed with parotid tumors were admitted to the Maxillofacial Surgery Department of “Lucian Blaga” University and in Euroclinic Hospital. 72% were benign tumors and 28% malignant. All patients received surgical treatment (total or partial parotidectomy). For the malignant tumors, radiotherapy was added to the modal treatment (94% of the cases). Pleomorphic adenoma was encountered in 70% of the benign cases, followed by Warthin tumor in 15%. Adenoid cystic carcinoma was noticed in 31% of the malignant cases, mucoepidermoid carcinoma in 25% of the cases, and squamous carcinoma and non-Hodgkin lymphoma on 12.5% of the malignant cases.

This chapter reviews eyelid malpositions, benign and malignant tumors, infections, and inflammatory diseases of the eyelid. Eyelid lesions can originate in any layer or structure within the eyelid and eyelid adnexa. Benign tumors of the eyelid can often be diagnosed based on their characteristic appearance. A biopsy should be performed if an eyelid lesion is not easily diagnosed based on clinical appearance. Blepharitis and meibomitis are very common disorders. Despite their prevalence, these diseases are often overlooked and misdiagnosed. Meibomian gland dysfunction is a major cause of evaporative dry eye and can occur along with aqueous deficient dry eye. Bacteria, fungi, viruses, and parasites can cause infection of the eyelids in different locations, anterior, posterior, angular, which can have different courses, acute, intermediate, or chronic. This chapter also contains photos with a rare disorder, Urbach-Wiethe disease, demonstrating the lesions of lipoid proteinosis at the lower and upper eyelids.

Context The recent recommendations of the European Endocrine Society states that the performance of computed tomography (CT) to characterize ‘true' adrenal incidentalomas (AIs) remains debatable. Objective To determine relevant thresholds for usual CT parameters for the diagnosis of benign tumors using robust reference standard among a large series of ‘true’ AIs recruited in an endocrinological setting. Design Retrospective study of 253 AIs in 233 consecutive patients explored in a single university hospital: 183 adenomas, 33 pheochromocytomas, 23 adrenocortical carcinomas, 5 other malignant tumors and 9 other benign tumors. Reference standard was histopathology in 118 AIs, biological diagnosis of pheochromocytoma in 2 AIs and size stability after at least 1 year of follow-up in 133 AIs. Methods Sensitivity, specificity and positive and negative predictive values were estimated for various thresholds of size, unenhanced attenuation (UA), relative and absolute wash-out (RPW, APW) of contrast media. 197 scans were reviewed independently in a blinded fashion by two expert radiologists to assess inter-observer reproducibility of measurements. Results Criteria associated with a 100% positive predictive value for the diagnosis of benign AI were: a combination of size and UA: 30 mm and 20 HU or 40 mm and 15 HU, respectively; RPW >53%; and APW >78%. Non-adenomatous AIs with rapid contrast wash-out were exclusively benign pseudocysts and pheochromocytomas, suggesting that classical thresholds of 60% and 40% for APW and RPW, respectively, can be safely used for patients with normal metanephrine values. Inter-observer reproducibility of all parameters was excellent (intra-class correlation coefficients: 0.96–0.99). Conclusions Our study, the largest conducted in AIs recruited in an endocrinological setting, suggests safe thresholds for quantitative CT parameters to avoid false diagnoses of benignity.

Objective. Facial nerve preservation and oncological safety are crucial in surgery of parotid tumors. An unexpected histopathologic diagnosis of a malignant parotid tumor, however, may unfavorably require a second, more radical surgery. The aim of this study was to find out whether the assessment of serological tumor markers in parotid saliva might have some diagnostic significance in the preoperative differentiation between benign and malignant parotid lesions. Study Design, Setting, Patients, and Methods. In a prospective pilot study performed at a university medical center in 28 patients with a unilateral parotid tumor, 7 serological tumor markers established in the clinical routine were quantitatively assessed in parotid saliva collected simultaneously on both sides after stimulation. The results were correlated with the histopathological diagnosis. Results. Of the 4 investigated tumors that were malignant neoplasms, 3 had a sufficient quantity of saliva available for tumor marker measurements. Carbohydrate antigen 19-9 (CA 19-9) consistently revealed high levels compared with the unaffected side in all malignant tumors, thus allowing malignant tumors to be differentiated from benign lesions. Conclusion. The results of this pilot study are encouraging, showing that preoperative tumor marker investigation in saliva from parotid glands is feasible and merits further investigation. CA 19-9 might be a valuable new diagnostic tool in the preoperative differentiation between malignant and benign parotid tumors and should be investigated in a larger number of patients.

Background The etiologies of small bowel intussusception (SBI) in adults are varied. Purpose To investigate multidetector computed tomography (MDCT) characteristics in adults with neoplastic and non-neoplastic SBI. Material and Methods Clinical data and MDCT images diagnosed with SBI in adults from January 2010 to May 2020 were retrospectively reviewed. Results The study included a total of 71 patients. Forty-two patients had a combined total of 55 neoplastic intussusceptions, including 29 patients with benign tumors and 13 patients with malignant tumors. Twenty-nine patients had a combined total of 36 non-neoplastic intussusceptions, of which the condition was idiopathic in 23 patients and cased by non-neoplastic benign lesions in six patients. There were no significant differences in patient age or sex ratio in the neoplastic and non-neoplastic groups. In the non-neoplastic group the intussusceptions were shorter in length (3.6 cm vs. 13.2 cm, P<0.05) and smaller in transverse diameter (2.8 cm vs. 4.2 cm, P<0.05), and less likely to be associated with intestinal obstruction (2 vs. 18, P<0.05). The percentage of patients with multiple intussusceptions was greater in the neoplastic group (10/42, 23.8% vs. 4/29, 13.8%). In the non-neoplastic group only one lead point was detected (in a patient with Meckel’s diverticulum), whereas lead points were detected in all 55 intussusceptions in the neoplastic group. Conclusion There are differences in the clinical and MDCT manifestations of adult neoplastic and non-neoplastic SBIs. Whether a lead point is present or not has implications with regard to deciding on the most appropriate treatment and avoiding unnecessary surgery.

ABSTRACT Objectives Salivary gland neoplasms are uncommon, but they are of much interest and debate because of their remarkable variability in structure, clinical presentation, and behavior. We have studied 56 cases of parotid tumors and shared our views on the diagnosis and management of these rare tumors. Materials and methods Fifty-six patients with parotid swelling presented to us over a period of 5 years. Preoperatively, all these patients were evaluated with fine needle aspiration cytology (FNAC). Depending on the report, they were subjected to further treatment. Results Out of 56 cases, 44 (79%) were benign and 12 (21%) were malignant. Pleomorphic adenoma being the commonest benign tumor [32 cases (72%)] and among the malignant tumors, mucoepidermoid carcinoma was the commonest. Parotidectomy is the gold standard treatment for both the benign and malignant parotid tumors. Conclusion Parotid tumors are a rare entity. Among which benign tumors are more common than malignant. An accurate diagnosis preoperatively and timely surgical management would reduce the recurrence rate and risk of complications. Objective The purpose of this study was to analyze and compare the value of fine-needle aspiration cytology (FNAC) and frozen section (FS) analysis in the assessment of parotid gland tumors. Study Design Chart review and cross-sectional analysis. Subjects and Methods FNAC and FS analysis of 110 parotid tumors, 68 malignancies and 42 benign tumors, were analyzed and compared with the final histopathologic diagnosis. Results The accuracy, sensitivity, and specificity of FNAC in detecting malignant tumors were 79 percent, 74 percent, and 88 percent, respectively. On FS analysis, the accuracy, sensitivity, and specificity in detecting malignant tumors were 94 percent, 93 percent, and 95 percent, respectively. The histologic tumor type was correctly diagnosed by FNAC and FS in 27 of 42 (64%) and 39 of 42 (93%) benign tumors, respectively, and in 24 of 68 (35%) and 49 of 68 (72%) malignant neoplasms, respectively. Conclusion The current analysis showed a superiority of FS compared with FNAC regarding the diagnosis of malignancy and tumor typing. FNAC alone is not prone to determine the surgical management of parotid malignancies.

Objectives: To show the radiological manifestations of primary pleural tumors. Patients and methods: we carried out a retrospective analysis of radiological findings in 62 patients with primary malignant tumor of pleura. Results: Study included 39 male and 23 female patients. Malignant tumors were present in 92.7% of the patients and benign ones in 7.2%. The most common malignant tumor was mesothelioma (85.4%), and solitary fibrous tumor prevailed among benign tumors (9.7%). Diffuse malignant mesothelioma manifested on computed tomography (CT) as a pleural thickening and effusion in 67.4% of the patients, tumors and effusion in11.7%, and only as an effusion in 9.8% cases. Thickening of the pleura appeared diffuse in 54% of patients and most often it had nodular pattern. Both localized malignant and all benign tumors presented as tumor-like changes with the signs of necrosis in 50%. Conclusion: The imaging methods have a key role in the diagnosis of pleural tumors. CT shows different morphologic features of pleural lesions that have been established as a useful tool for differentiating malignant from benign disease. However, magnetic resonance is preferred imaging method for assessing the extent and resectability of pleural tumors.

Background: Soft tissue tumors constitute a large and heterogenous group of neoplasms. Benign tumors out number their malignant counterparts by a ratio of about 100:1 in hospital population. FNAC has emerged as a major outpatient procedure for the diagnosis of soft tissue tumors due to low cost of the procedure, less complications, feasibility, quick results and high therapeutic efficiency with specificity and sensitivity of approximately 95%. The diagnostic accuracy of FNAC of soft tissue tumors in distinguishing benign and malignant lesion is also very high. The objectives of this study were to study the role of FNAC in the diagnosis of soft tissue tumors and its correlation with histopathology. Also, this study aimed at studying various cytomorphological patterns of soft tissue tumors and correlating cytological grading with histopathological grading.Methods: It was a hospital based study of 5 years which included 479 patients of all age groups, clinically presenting with soft tissue swellings. FNAC was performed. Smears were stained with Papanicolaou (PAP)/May Grunwald Geimsa (MGG). The cytological details of soft tissue tumors were studied and broadly classified into benign, malignant and indeterminate and suspicious. Cytomorphological subtyping and grading of tumors on FNAC was done. The cytological findings were correlated with the histopathological results, wherever available.Results: Benign tumors comprised of 423 cases (88.3%) and malignant tumors comprised 56 cases (11.69%)only. Maximum number of cases were seen in well differentiated/lipomatous group (339 cases) followed by spindle cell category (88 cases). Histopathological correlation carried out in 136 out of 479 cases (benign:111 and malignant :25) revealed that out of 111 cases diagnosed as benign by cytology, one case was malignant (liposarcoma) and among 25 malignant cases diagnosed by cytology one case was benign (myofibroblastoma). Overall sensitivity and specificity of FNAC was 96% and 99% respectively. Comparison of cytological and histopathological grading of 24 sarcomas showed overall concordance of 75%.Conclusions: FNAC is an excellent diagnostic modality in early diagnosis of soft tissue tumors. FNAC is highly reliable and obviates surgical procedures especially in high risk patients thus facilitating initiation of appropriate therapy and saving time and manpower. It is also highly sensitive in detecting benign soft tissue tumors and highly specific for malignant soft issue tumors.
