Modeling Vehicle Collision Injury Severity Involving Distracted Driving: Assessing the Effects of Land Use and Built Environment

This paper presents the findings of the vehicle occupant injury severity model, particularly focusing on the collisions involving distracted driving. The study develops a latent segmentation-based logit model for analyzing crash injury severity utilizing police-reported collision data from 2007 to 2011 in Nova Scotia, Canada. A segment allocation model is estimated to capture latent heterogeneity based on individual victims’ and drivers’ profiles, and collision attributes including vehicle type, vehicle trajectory, collision object, and collision type. The segment allocation model results suggest the existence of high-risk and low-risk injury severity segments. This study extensively tests the effects of built environment characteristics. The model results suggest that rain, curved road, freeway, and mid-block collisions aggravate vehicle occupant injury severity; whereas, higher land use mix, longer length of sidewalk, and higher population density mitigate injury severity. Significant heterogeneity is found across the high- and low-risk segments. For instance, straight road alignment is found to yield higher injury severity in the high-risk segment and lower severity in the low-risk segment. Moreover, the model unveils the interplay between built environment and distraction type. Driver distraction by communication device increases injury severity at a curved road intersection. Additionally, distraction because of inattentiveness increases injury severity. The findings of this study assist road safety engineers and planners to identify effective countermeasures and awareness programs for reducing the crash injury severity or consequences for vehicle occupants.

Klaten Regency is located in Central Java Province, Indonesia, ranked as 19th most susceptible area in Indonesia. Among of many disasters those take place in Klaten are floods, landslides, and earthquake, which cause damages and loss of lives. Unfortunately, some areas in Klaten Regency are also very vulnerable to the disasters that often contribute severe damage and loss. This paper presents result of risk analysis due to floods, landslides and earthquake disaster at Klaten Regency. Several parameters or criteria are utilized to describe the level of the disaster intensities. The flood susceptibility parameters are the Topographic Wet Index (TWI), Normalized Difference Vegetation Index (NDVI), permeability and roughness, as proposed by Kafira, et al. (2015). The landslide susceptibility are the geology, slope, elevation, distance from fault, distance from rivers, rainfall and land use, as suggested by Thearith (2009) whereas the earthquake susceptibility was referred to FEMA P-154 by using the Ss and S1. The vulnerability and risk analysis are carried out by referring to the parameters as stipulated by the Chief Regulation of the National Board of Disaster Management No.2 Year 2012 (Perka Badan Nasional Penanggulangan Bencana- BNPB), concerning the parameters being used for the vulnerability analysis, i.e. population density, poverty ratio, land use, and level of Gross Regional Domestic Product. Further spatial analysis of the risk performs the multi-disaster risk map as a combination between the floods, landslides and earthquake disaster risk in Klaten Regency. The established multi-disaster risk map shows the risk level in the Klaten Regency, i.e., 16.31% at very low risk, 33.01% at low risk, 34.49% at medium risk, at 14.22% high risk and 1.97% at very high risk.

Collisions between heavy trucks and passenger cars are a major concern because of the severity of injuries. This research has two objectives. One is to examine the impact of various factors on injuries to passenger car occupants involved in such collisions. Due to the complex interaction of factors influencing injury levels in truck-car collisions, the ordered probit model is used to identify specific variables significantly influencing levels of injury in two-vehicle rear-end involvements on divided roadways. Another objective is to demonstrate the use of the ordered probit in this complex highway safety problem. A set of vehicle, occupant, roadway, and environmental factors expected to influence injury severity was developed. Given two-vehicle passenger car-truck rear-end collisions, the variables that increase passenger vehicle occupant injury severity include darkness; high speed differentials; high speed limits; grades, especially when they are wet; being in a car struck to the rear (as opposed to being in a car striking a truck to the rear); driving while drunk; and being female. The interaction effects of cars being struck to the rear with high speed differentials and car rollovers were significant. Variables decreasing severity include snowy or icy roads, congested roads, being in a station wagon struck to the rear (as opposed to a sedan), and using a child restraint. With injuries ordered in five classes from no injury to fatalities, the marginal effects of each factor on the likelihood of each injury class are reported.

Fragestellung: Die Bedeutung von Lymphknotenbefall bei papillärem Schilddrüsenkarzinom und die optimale Lymphknotenchirurgie werden kontrovers beurteilt. Methodik: Retrospektive Langzeitstudie eines Operateurs (n = 159), prospektive Dokumentation, Nachkontrolle 1-27 (x = 8) Jahre, Untersuchung mit Bezug auf Lymphknotenbefall. Resultate: Staging. Bei 42 Patienten wurde wegen makroskopischem Lymphknotenbefall (cN1) eine therapeutische Lymphadenektomie durchgeführt, mit pN1 Status bei 41 (98%) Patienten. Unter 117 Patienten ohne Anhalt für Lymphknotenbefall (cN0) fand sich okkulter Befall bei 5/29 (17%) Patienten mit elektiver (prophylaktischer) Lymphadenektomie, und bei 2/88 (2.3%) Patienten ohne Lymphadenektomie (metachroner Befall) (p < 0.005). Lymphknotenrezidive traten (1-5 Jahre nach kurativer Primärtherapie) bei 5/42 (12%) pN1 und bei 3/114 (2.6%) cN0, pN0 Tumoren auf (p = 0009). Das 20-Jahres-Überleben war bei TNM I + II (low risk) Patienten 100%, d.h. unabhängig vom N Status; pN1 vs. pN0, cN0 beeinflusste das Überleben ungünstig bei high risk (>= 45-jährige) Patienten (50% vs. 86%; p = 0.03). Diskussion: Der makroskopische intraoperative Lymphknotenbefund (cN) hat Bedeutung: - Befall ist meistens richtig positiv (pN1) und erfordert eine ausreichend radikale, d.h. systematische, kompartiment-orientierte Lymphadenektomie (Mikrodissektion) zur Verhütung von - kurablem oder gefährlichem - Rezidiv. - Okkulter Befall bei unauffälligen Lymphknoten führt selten zum klinischen Rezidiv und beeinflusst das Überleben nicht. Wir empfehlen eine weniger radikale (sampling), nur zentrale prophylaktische Lymphadenektomie, ohne Risiko von chirurgischer Morbidität. Ein empfindlicherer Nachweis von okkultem Befund (Immunhistochemie, Schnellschnitt von sampling Gewebe oder sentinel nodes) erscheint nicht rational. Bei pN0, cN0 Befund kommen Verzicht auf 131I Prophylaxe und eine weniger intensive Nachsorge in Frage.

Background: We performed a critical overview of published systematic reviews (SRs) of chemotherapy for advanced and locally advanced pancreatic cancer, and evaluated their quality using AMSTAR2 and ROBIS tools. Materials and Methods: PubMed and Cochrane Central Library were searched for SRs on 13th June 2020. SRs with metaanalysis which included only randomized controlled trials and that had assessed chemotherapy as one of the treatment arms were included. The outcome measures, which were looked into, were progression-free survival (PFS), overall survival (OS), and adverse events (AEs) of grade 3 or above. Two reviewers independently assessed all the SRs with both ROBIS and AMSTAR2. Results: Out of the 1,879 identified records, 26 SRs were included for the overview. Most SRs had concluded that gemcitabine-based combination regimes, prolonged OS and PFS, but increased the incidence of grade 3-4 toxicities, when compared to gemcitabine monotherapy, but survival benefits were not consistent when gemcitabine was combined with molecular targeted agents. As per ROBIS, 24/26 SRs had high risk of bias, with only 1/26 SR having low risk of bias. As per AMSTAR2, 25/26 SRs had critically low, and 1/26 SR had low, confidence in the results. The study which scored ‘low’ risk of bias in ROBIS scored ‘low confidence in results’ in AMSTAR2. The inter-rater reliability for scoring the overall confidence in the SRs with AMSTAR2 and the overall domain in ROBIS was substantial; ROBIS: kappa=0.785, SEM=0.207, p<0.001; AMSTAR2: kappa=0.649, SEM=0.323, p<0.001. Conclusion: Gemcitabine-based combination regimens can prolong OS and PFS but also worsen AEs when compared to gemcitabine monotherapy. The included SRs have an overall low methodological quality and high risk of bias as per AMSTAR2 and ROBIS respectively.

This study furthers existing research on the link between the built environment and travel behavior, particularly mode choice (auto, transit, biking, walking). While researchers have studied built environment characteristics and their impact on mode choice, none have attempted to measure the impact of zoning on travel behavior. By testing the impact of land use regulation in the form of zoning restrictions on travel behavior, this study expands the literature by incorporating an additional variable that can be changed through public policy action and may help cities promote sustainable real estate development goals. Using a unique, high-resolution travel survey dataset from Denver, Colorado, we develop a multinomial discrete choice model that addresses unobserved travel preferences by incorporating sociodemographic, built environment, and land use restriction variables. The results suggest that zoning can be tailored by cities to encourage reductions in auto usage, furthering sustainability goals in transportation.
