Abstract Background Leech infestation in the nose or nasopharyngeal region is a rare occurrence. The most common known cause is drinking water from natural water sources like ponds and rivers. Its hidden location of attachment in the nasopharynx and its uncommon occurrence make it easy to miss during diagnosis. Case presentation We present a case of a 61-year-old male patient with recurrent unilateral epistaxis without any apparent cause. He was diagnosed with leech infestation in the nasopharynx on endoscopic examination. This article reports the management of nasopharyngeal leech infestation and safety measures for this animate foreign body retrieval. Conclusion A vigilant approach, thorough history, and examination are a must. Though leeches are simple to remove most of the time, certain necessary precaution should be kept in mind for better management and prevention of further complications.

Abstract We describe an extremely rare occurrence of a squamous differentiation in a sarcomatoid chromophobe renal cell carcinoma in a 45-year-old woman with nodal and lung metastasis at presentation. The tumor on histology showed all 3 components intimately admixed with each other, which to the best of our knowledge is the first such case to be reported in the literature. The renal pelvis was smooth walled and uninvolved. Kidney-specific cadherin was positive in the chromophobe renal cell carcinoma areas and negative in the sarcomatoid and squamous areas.

Plastic bronchitis is the expectoration of bronchial casts in the mold of the tracheobronchial tree. It is a rare occurrence of unknown etiology that has been primarily described in children with congenital heart disease. In this case report, we present the first reported case of plastic bronchitis in a patient with pulmonary Kaposi sarcoma and underlying HIV infection.

ABSTRACT Thyroglossal cyst is a congenital condition of the neck where the painless swelling is found in the midline of the neck in between the foramen cecum of tongue base and sternal notch. Condition is common in children. Thyroglossal fistula is either secondary to infection or drainage of a misdiagnosed abscess. Here we are presenting a case of thyroglossal fistula with its opening over the chest and a cord extending from the hyoid bone to the chest causing restriction of the neck movement. Thyroglossal fistula opening in the chest, i.e. over the sternum is very rare and not been reported in any literatures. Treatment of this type of thyroglossal fistula is same as other types, i.e. Sistrunk's operation, where tract along with part of the hyoid bone is to be removed to prevent recurrence. Gastro – intestinal duplications are usually detected in children before 2 years of age due symptoms/complications associated with the condition or during surgery in the child for some unrelated condition. In adults colonic duplication is of rare occurrence and often diagnosed during surgery. However, it may be diagnosed pre operatively due to symptoms of obstruction, volvulus or rarely due to perforation. We present an adult having tubular duplication of ascending colon, that presented with perforation. The duplication had a blind end and did not have distal communication. In addition, it had its own blood supply. The management of the case is discussed. Aetiogenesis of the anomaly is enumerated and literature reviewed. For ascending colon duplication similar meaning word accessory ascending colon has also been used in the manuscript.
