Limited understanding of the psychological challenges experienced by individuals with chronic critical illness hampers efforts to deliver quality care. We used an interpretive description approach to explore sources of distress for individuals with chronic critical illness in residential care, wherein we interviewed six residents, 11 family members, and 21 staff. Rather than discuss physical symptoms, sources of distress for residents were connected to feeling as though they were a patient receiving medical care as opposed to an individual living in their home. The tension between medical care and the unmet need for a sense of home was related to care beyond the physical being overlooked, being dependent on others but feeling neglected, frustration with limited choice and participation in decision making, and feeling sad and alone. Efforts to refine health care for individuals with chronic critical illness must foster a sense of home while ensuring individuals feel safe and supported to make decisions.

Abstract Objective: to analyze the care and management demands of Long-Term Care Facilities for the Elderly (LTCFs) in Brazil. Method: an integrative review of literature was carried out, organized into six stages: a) elaboration of a guiding question; b) online search of LILACS, SciELO, PubMed, the CAPES Portal and the Brazilian Society of Geriatrics and Gerontology databases; c) article selection, following the exclusion and inclusion criteria, with the sample composed of 17 articles; d) commented analysis of the selected articles; e) deliberation on the results obtained, formulated from the synthesis and interpretation of the selected studies; f) presentation of the results of the review. Results: the care demands identified are related to the process of caring and assume a working team with geriatric and gerontological knowledge, while the management demands include the means and resources needed so the care can be provided effectively. However, the LTCFs were found to rely on professionals who are unprepared to provide care or to assume an organizational management role, meaning care for the elderly is restricted to the essentials for their basic needs. Conclusion: the care demands were easily identified in the analyzed publications, however, there is a lack of research that evaluates management demands in a broader and more in-depth manner. It is suggested that studies aiming to broaden theoretical knowledge of the care and management demands of LTCFs are carried out, to stimulate effective and positive actions in the practices of these institutions, seeking to offer top quality care to elderly persons that live in these facilities, that responds to the real needs of their current stage of life.

The development of general medical practice as a scientific specialty and practice and the structuring of medical care according to the levels of its delivery are characterized by shifting the emphasis on long-term care for patients with chronic dermatoses to the level of general practitioners. This paper describes the scope of treatment and prophylactic measures at the stage of primary medical care for 75 patients with eczema of different severity. It was revealed that the completeness of the use of non-medicated means in the period between the exacerbations of eczema prescribed by general practitioners was at the level of 56.1 ÷ 60.2% according to the generalized quality index and was characterized by insufficient use of autotraining techniques with elements of psychological correction in 76.7 ± 5,0%, herbal ointments in 72,7 ± 5,1%, as well as insufficient physiotherapeutic corrections in 57,3 ± 5,7%. Adequate use of pharmacological medicines prescribed by general practitioners for the periods between the exacerbation of eczema makes up 53,1 ÷ 53,9% according to the generalized indicator of quality and is characterized by low use of magnesium-containing products in 76,0 ± 4 9% of cases and adrenal gland stimulants in 74,7 ± 5,0% of cases, as well as detoxification agents (53,3 ± 5,8)% of cases.  The analysis of general indicators of the quality of therapeutic and prophylactic process in the periods between exacerbations of chronic eczema has shown that general practitioners prefer to prescribe non-medicinal means, underestimating the importance of using immunomodulatory drugs and vitamins (A, E, B and C).

The Continental Europe regional outlook presents a comparative assessment of the historical development of the healthcare system, health politics, and selected health-related indicators for Austria, Belgium, France, Germany, Luxembourg, the Netherlands, and Switzerland. In terms of health financing, these countries rely on the compulsory contributory insurance and corporatist administrative practices characteristic of Bismarckian health systems, to which the Netherlands and Switzerland have added mandated private insurance. Health outcomes are very positive, with high life expectancy, low infant mortality, and comparatively low levels of health inequality. Unmet need is the lowest in Europe, and satisfaction is among the highest. Since at least 2002, healthcare has tended to be a highly salient issue for Germany and the Netherlands, but not so for Belgium and Luxembourg, the two countries with the highest levels of public satisfaction with the health system. The key issues in Continental Europe have been cost containment, patient rights, and the introduction of long-term care, as well as some discussion of the role of private insurance and the efficiency of corporatist institutions.

Advances in medical care has reduced the rate of mortality from strokes, but the incidence of stroke has remained stable while the incidence of ministrokes has increased. Most stroke victims require long-term care, imposing a heavy financial and emotional burden on families while incurring a heavy cost to society. Thus, strokes are a key issue in the context of health care in Taiwan. This paper proposes using VBA (Visual Basic for Applications) to build a system for assessing Brunnstrom stages based on the observation of several obvious rehabilitation features The system calculates features for accelerometer readings, which are then used as input parameters for a fuzzy algorithm to obtain the Brunnstrom action level. Experimental results show the proposed approach effectively assesses Brunnstrom level, and that the approach can be used to assist physical therapists in performing longitudinal assessments of stroke victim progress, thus improving evaluation efficiency.
