An Open-Label Trial of Methylphenidate Treating Sluggish Cognitive Tempo, Inattention, and Hyperactivity/Impulsivity Symptoms Among 6- to 12-Year-Old ADHD Children: What Are the Predictors of Treatment Response at Home and School?

Objective: This study investigated the effects of sluggish cognitive tempo (SCT), other psychiatric symptoms, age, dose, and pretreatment ADHD severity on methylphenidate (MPH) treatment response among ADHD children in both home and school. In addition, the predictors of the MPH-SCT treatment response were examined. Methods: One hundred eighty-five (6–12 years old) ADHD children who were treated with MPH included in the study. Results: MPH improved SCT total and SCT-Daydreaming scores both at home and school while improved SCT-Sluggish scores in only school. Higher pretreatment Daydreaming score predicted lower treatment response for inattention ( B = .301, p = .002), and higher Daydreaming-Sluggish scores predicted lower treatment response for total ADHD symptoms at school ( B = .456, p = .006; B = .888, p = .04, respectively). Also higher oppositional defiant disorder symptoms have negative effects on MPH treatment response in ADHD. Older age positively affected the MPH-SCT treatment response in paternal and teacher ratings. Conclusion: SCT symptoms have negative effects on MPH treatment response at school.

Objective: This study evaluated which Sluggish Cognitive Tempo (SCT) factors (i.e., Slow, Sleepy, Daydreamer) are most strongly associated with homework motivation, and whether homework motivation mediates the path between SCT and academic impairment. Method: Participants were 285 middle school students (boys 209) in Grades 6 to 8 (ages 10-15 years) who were comprehensively diagnosed with ADHD. Results: Parent- and self-report of SCT Slow behaviors predicted homework motivation above and beyond symptoms of ADHD, oppositional defiant disorder (ODD), anxiety, depression, and intelligence. The mediation models tested were multi-informant and cross-rater (parent-report of SCT to self-report of motivation to teacher-report of homework problems), and suggest that low motivation may help explain the associations between SCT and functional impairment. Conclusion: SCT and motivation are significantly associated constructs. Clinically, youth with ADHD and comorbid SCT may be more likely to present with low motivation, placing them at risk for academic failure. The manuscript discusses potential clinical implications of these findings.

Objective: To examine the validity of sluggish cognitive tempo (SCT) and ADHD–inattention (ADHD-IN) symptoms in children from Chile. Method: Mothers and teachers rated SCT, ADHD-IN, ADHD–hyperactivity/impulsivity (ADHD-HI), oppositional defiant disorder (ODD), anxiety, depression, academic impairment, social impairment, and peer rejection (teachers only) in 652 Chilean children (55% boys) aged 6 to 14 years. Results: For both mother and teacher ratings, the eight SCT symptoms and nine ADHD-IN symptoms showed substantial loadings on their respective factors (convergent validity) along with loadings close to zero on the alternative factor (discriminant validity). ADHD-IN showed a uniquely stronger relationship than SCT with ADHD-HI and ODD whereas SCT showed a uniquely stronger relationship than ADHD-IN with anxiety and depression. Although ADHD-IN uniquely predicted academic impairment and social difficulties, SCT did not. Conclusion: This study provides the first evidence for the validity of SCT among children outside of North America or Western Europe.

Objective: The aims of the present study were to examine the presence of sluggish cognitive tempo (SCT) symptoms in children; associations of the symptoms with sociodemographic characteristics of the children; and relationships between SCT symptoms and symptoms of ADHD, dyslexia, academic performance, and behavioral problems. Method: We evaluated Catalan schoolchildren aged 7 to 10 years in Barcelona, 2012-2013. Parents filled out the SCT–Child Behavior Checklist (SCT-CBCL), the Strengths and Difficulties Questionnaire (SDQ), and a questionnaire concerning sociodemographic characteristics. Teachers completed the ADHD criteria of Diagnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV) (ADHD- DSM-IV), a list of dyslexia symptoms, and evaluated the children’s academic performance. SCT symptoms were studied as a continuous and dichotomous variable. Results: In all, 11% of the children in our sample scored above the clinical cut-off on the SCT-CBCL scale. We observed a higher rate of SCT symptoms in boys, children whose father was unemployed, those whose maternal educational level was lower, children with a high socioeconomic vulnerability index at home, those who reported maternal smoking during pregnancy and current second-hand smoke exposure at home, and children with an ADHD diagnosis. More SCT symptoms were associated with inattention symptoms, symptoms of dyslexia, academic problems, and emotional and peer relationship problems. Conclusion: We observed a higher prevalence of SCT symptoms in our sample than expected in the general population. While girls are less prone to SCT symptoms, some socioeconomic indicators, dyslexia, and inattention symptoms as well as exposure to smoking at home increase the risk of SCT and must be taken into account during assessments.

Purpose The purpose of this paper is to analyze the nutritional quality of dinnertime meals eaten out of home (OH) vs those eaten at home by male Korean workers. Design/methodology/approach The study included 1,634 male Korean workers aged between 19 and 64 years among 15,508 individuals who participated in a 24-hour dietary recall through the Sixth Korea National Health and Nutrition Examination Survey from 2013 to 2014. The study participants were classified and analyzed according to the place where dinner had been prepared: out-of-home group (OHG) (n=659) and at-home group (AHG) (n=975). Findings Young male white-collar workers who are unmarried with a higher level of education and income were more likely to eat OH at dinner. The OHG consumed more energy, fat, and sodium, but less carbohydrate at dinner than the AHG. The contribution of dinner to daily energy and macronutrient intakes, except for carbohydrate, was higher in the OHG. Additionally, the study results suggested that the OHG was less likely to consume a traditional Korean meal at dinner. Overall, the nutritional quality of dinnertime meals eaten OH had greater potential to lead to negative effects on nutrition and health. Originality/value This study highlights OH eating among male Korean workers as an important arena in which strategies for healthier eating can be deployed when establishing worksite health promotion or related national nutrition policies.

Objective: This study evaluated whether sluggish cognitive tempo (SCT) is separable from ADHD–inattention (IN) and uniquely associated with internalizing dimensions in preschool children in South Korea. Method: Mothers of 172 preschool children (ages 4-6 years; 52% girls) rated children’s SCT, ADHD-IN, ADHD–hyperactivity/impulsivity (HI), oppositional defiant disorder (ODD), aggression, emotional reactivity, anxiety/depression, somatic complaints, withdrawal, and sleep problems. Results: Eight of 10 SCT symptoms showed convergent and discriminant validity with ADHD-IN. ADHD-IN remained significantly positively associated with ADHD-HI, ODD, and aggressive behavior after controlling for SCT, whereas SCT was no longer positively associated with these externalizing behaviors after controlling for ADHD-IN. Both SCT and ADHD-IN were uniquely associated with greater emotionally reactivity, anxiety/depression, and withdrawal. Only SCT was uniquely associated with somatic complaints, and only ADHD-IN was uniquely associated with sleep problems. Conclusion: Findings replicate results with children and adolescents, thus expanding evidence for the validity of SCT in early development.

Interest in symptoms of sluggish cognitive tempo (SCT) has led to a number of studies evaluating how these symptoms respond to treatment commonly utilized in youths with symptoms of ADHD. No study to date, however, has examined the extent to which symptoms of SCT predict behavioral treatment response in youths across multiple domains of functioning. The current preliminary investigation integrates a number of methodological (e.g., direct observations) and analytic (e.g., Poisson regression) refinements to evaluate the extent to which symptoms of SCT predict treatment responses across multiple domains including behavioral (e.g., interruptions, rule violations), social (e.g., social skills, negative verbalizations), and severe behavioral difficulties (e.g., intentional aggression) above and beyond other demographic characteristics (e.g., symptom severity, Full Scale Intelligence Quotient [FSIQ]). A relatively small sample of 37 children, aged six to 12 years ( M = 8.03, SD = 1.83, 35 males: 2 females) attending an eight week multi-component intensive behavioral treatment program for youths with behavioral difficulties participated in the current study. Baseline parental perceptions of SCT were collected prior to the initiation of treatment. Results from this preliminary investigation revealed that pre-treatment SCT symptoms only predicted a less robust treatment response to time out which was associated also with parent’s perceptions of underlying working memory problems. Results revealed also that pre-treatment SCT symptoms failed to predict paraprofessional counselor’s and teacher’s improvement ratings of both rule following and social skills following treatment. Notably, other potential predictors (e.g., symptom severity, FSIQ) also largely failed to predict behavioral treatment response.

Research has yet to determine how much of the variance in sluggish cognitive tempo (SCT) symptom ratings is consistent across occasions, sources, and settings versus specific to occasion, source, and setting. Our first objective was to determine the amount of variance in SCT ratings that was consistent ( trait consistency) across three occasions of measurement over 12 months versus specific to the occasion ( occasion-specificity) with ratings by mothers, fathers, primary teachers, and secondary teachers of 811 Spanish children. Our second objective was then to determine the convergent validity for trait consistency and occasion-specificity variance components within and across settings. SCT ratings reflected mostly trait consistency for mothers, fathers, and primary teachers (less so for secondary teachers) with the convergent validity for trait consistency also being strong for mothers with fathers and for primary teachers with secondary teachers. Across home and school, however, convergent validity for trait consistency was low and even lower for occasion-specificity. SCT appears to be more trait-like rather than state-like, with similar levels of trait consistency across occasions and convergent validity within settings as attention-deficit/hyperactivity disorder (ADHD) symptoms in a prior study. However, SCT symptoms had slightly weaker convergent validity for trait consistency across settings relative to ADHD symptoms.
