Motivational interviewing (MI) is a collaborative communication style that can be integrated into everyday practice to improve conversations and serve as a catalyst for behavior change. This article reviews the fundamental principles and basic skills of MI. It discusses how MI has evolved from a therapeutic modality to a conversational style, applicable in the school setting and specifically for school nurses. The article provides an overview of how MI can be used by school nurses, challenges they may face, and resources to support implementation.

Background Clinical research investigating effective intervention strategies for adolescents to improve health behaviors has shifted to the application of motivational interviewing (MI). Evidence indicates that MI is an effective intervention for improving health behaviors as related to diet, exercise, and diabetes among adolescents. However, there is a lack of understanding about the mechanisms through which MI works and the contextual factors impacting MI effectiveness. The purpose of this review was to understand how, for whom, and under what circumstances MI works for adolescent health behavior change, which will inform future implementation of this intervention. To provide this in-depth understanding, a realist-informed systematic review was conducted in order to synthesize the evidence on the use of MI for health behaviors. Self-determination theory (SDT) was chosen as the candidate theory for testing in the present review. Methods Databases including PsycINFO, Healthstar, Cochrane, and PubMed were searched for articles published until March 2017. The search strategy included studies that examined or reviewed the effectiveness or efficacy of MI to change health behaviors among adolescent populations. The search identified 185 abstracts, of which 28 were included in the review. The literature was synthesized qualitatively (immersion/crystallization) and tested SDT as the candidate theory. Results Based on SDT, three mechanisms were found within reviewed studies, including competence, relatedness, and autonomy. The following contexts were found to impact mechanisms: school setting, clinician MI proficiency, parental involvement, and peer involvement. Conclusions This realist-informed systematic review provides advances in understanding the mechanisms involved in MI for adolescent health behavior change. Additionally, it provides important practical information as to which contexts create the conditions for these mechanisms to occur, leading to health behavior change. The results can inform future MI interventions for adolescent health behavior change. Future research should continue to test this realist theory and also examine mechanism variables not extensively documented in order to improve our understanding of MI in this population.

Students presenting with varying degrees of respiratory symptoms and distress occur commonly in the school setting. It is important to develop a differential diagnosis for respiratory distress, to initiate stabilization of the student with life-threatening symptoms, and to triage these students to an appropriate level of care (back to the classroom, home with their guardian with follow up at their primary health care provider’s office, or directly to the closest emergency department via Emergency Medical Services). This article describes the initial assessment and management of a student presenting with respiratory distress.

Tooth avulsions occur when a tooth is displaced from its socket. Tooth avulsions are common dental injuries that may occur before, during, or after school. Therefore, it is essential that school nurses be well prepared to intervene when such a dental emergency arises. It is also imperative that school nurses and school personnel are fully equipped to manage a tooth avulsion. Time is of the essence when attempting to save an avulsed tooth. The goal is to successfully reinsert the avulsed tooth and to preserve the periodontal ligament. The school nurse is responsible for implementing the appropriate actions needed to save the avulsed tooth.

Abstract Background The relationship between a clinician and their client—the “therapeutic alliance” is a robust predictor of outcome in healthcare settings; yet, few interventions to improve alliance have been tested. Motivational interviewing is a client-centered approach that embodies many principles and strategies consistent with a strong therapeutic alliance. Purpose To examine whether alliance is enhanced by training dietitians to deliver a motivational interviewing informed health behavior change intervention (“Eating as Treatment”; EAT) as part of routine consultations with patients with head and neck cancer. The predictive ability of motivational interviewing techniques was also assessed. Methods A secondary analysis of the EAT stepped-wedge cluster-randomized controlled trial was conducted. Patients with head and neck cancer undergoing radiotherapy (n = 307) were treated by radiotherapy dietitians (n = 29) during the control (Treatment as Usual) or intervention (EAT) phase. Alliance was rated during the first and final weeks of radiotherapy, and again 4 and 12 weeks post-radiotherapy. Dietetic sessions were audiotaped. Week one sessions were objectively rated for dietitians’ use of motivational interviewing techniques. Results Generalized linear-mixed effects regressions found no effect of EAT on dietitian-rated alliance (p = .237). After excluding outliers, patient-rated alliance was 0.29 points lower after EAT training (p = .016). Post hoc analyses revealed lower patient ratings on perceived support and dietitian confidence. Hierarchical multiple regressions found that no specific motivational interviewing techniques predicted patient-rated alliance. Dietitian acknowledgment of patient challenges was related to dietitian-rated alliance (β =.15, p =.035). Conclusions Patient and dietitian ratings of alliance were high after EAT training, but not significantly improved. Further research is needed to better understand the differential impact of intervention training and delivery on patient and clinician ratings of therapeutic alliance. This article provides a conceptual definition of the concept trigger within the context of health behaviors and applies it to the highly significant health issue of obesity. Healthy behaviors are essential to life and happiness, but they do not just happen. They are triggered, and an inner drive keeps them alive. To help patients gain and retain optimal health, nurses must understand the triggers of healthy behaviors. Walker and Avant’s (2011) method of concept analysis is used as the basis for defining the concept of trigger. The antecedents, defining attributes, and consequences of trigger are identified. Findings suggest that nurses can play a role in triggering health behavior change through simple motivational efforts.

This paper aims at finding verbal communication used by an interviewer and interviewee in Oprah Winfrey Show. The theory used in this research was Levine�s and Adelman theory. The data were taken from two different videos of Oprah Winfrey Show. This study used descriptive qualitative method. Based on the result of the research, in the first video, it was found 81 of high involvement and 129 of high considerateness used by Oprah and 54 of high involvement and 125 of high considerateness used by Thich, 21 directness and 20 indirectness used by Oprah and 18 directness and 21 indirectness used by Thich, 3 bowling conversational style applied by Oprah and Thich. In the second video, there were109 high involvement and 66 of high considerateness used by Oprah, 131 high involvement and 69 of high consideratness used by Alanis, 26 directness and 7 indirectness used by Oprah and 26 of directness and 7 of indirectness used by Alanis, and 1 ping-pong conversation used by Oprah and Alanis. In addition, there were the changes of communication style occured in the conversation between an interviewee and an interviewer in Oprah Winfrey Show that indicate that there are the influences of cultural background towards communication style.Keywords: Analysis, Verbal Communication, Oprah Winfrey Show

Helping animal owners to recognise and manage obesity in their animals is a particularly complex area of communications in veterinary medicine. Several studies have outlined the difficulty veterinary professionals face in such discussions, including frustration with the client, embarrassment (particularly if the owner is also overweight), and a sense of inevitable failure. However, obesity continues to be a serious and prevalent welfare problem in dogs, cats, and probably other companion animals as well — hence those discussions will only continue. This review considers veterinary surgeon–client interactions around obesity from the perspective of behaviour change psychology and motivational interviewing, in order to determine how veterinary surgeons and nurses can best assist owners. We consider how an approach based on a supportive and empathetic conversational style could be best suited to these discussions, leading to tailored weight management solutions. Nurses are ideally placed to work with owners in this way.

In this chapter, the authors present the project “WITE 2.0.” This project is at the crossroads of various issues related to mobility (Urry, 2007) and use of Information and Communication Technologies. WITE 2.0 is a part of the designing process of a collaborative communication tool: “a virtualized and unified platform.” The authors define scenarios of teleworking practices, “equipped” by ICTs, and use these scenarios to better specify the platform. The project started at the end of 2010 and continued for a period of 18 months. The analysis is based on several complementary methodologies: a qualitative study (47 semi-structured interviews) and an experimentation of the platform. They present the main results of the interview survey through the following themes: remote management, skills, articulation of private and professional spheres, and the maturity of technologies. The authors also describe how these elements help the understanding of the evolution of workers’ practices.
