Advanced heart failure is an entity where irreversible structural heart disease is associated with persistent, refractory symptoms and quantitative decrease in cardiopulmonary capacity. Despite advanced and comprehensive medical therapy, patients are at high risk of death due to cardiogenic shock or cardiac arrest. Heart transplantation was developed as a surgical intervention to replace the failing recipient heart with a healthy heart from a recently deceased donor. In this article, we discuss the current state of cardiac transplantation, more than 5 decades after the first human cardiac transplantation was performed. Apart from an historical overview of the development of surgical techniques, we focus on appropriate patient selection, pretransplant evaluation, and recognition and treatment of post-transplant complications.

Cardiac transplantation has extended and improved the lives of patients suffering from severe heart failure over many decades. Despite advances in medical therapy, cardiac transplantation remains the definitive treatment for end-stage heart disease. Surgical techniques for organ procurement and implantation, development of appropriate methods for preserving the heart, and understanding the immunological challenges associated with transplantation were among the many areas which required focused investigation. In the current era, heart transplantation is associated with a low operative mortality and excellent long-term survival, however, the major obstacle of shortage of suitable donor organs remains. In the following chapter, recipient selection and management, donor organ procurement and preservation, and surgical techniques of heart transplantation are described in detail.

SummaryIntra-arterial thrombolytic therapy has replaced systemic intravenous infusion of thrombolytic agents as a treatment modality for arterial occlusion in the limbs. Several catheter-guided techniques and various infusion methods and schemes have been developed. At present there is no scientific proof of definite superiority of any agent in terms of efficacy or safety but clinical practice favours the use of urokinase or alteplase. Studies which compared thrombolysis to surgical intervention suggest that thrombolytic therapy is an appropriate initial management in patients with acute occlusion of a native leg artery or a bypass graft. Underlying causative lesions are treated in a second step by endovascular or open surgical techniques. Severe bleeding is the most feared complication: the risk of hemorrhagic stroke is 1-2%.

The current situation in the sphere of district heating is analysed on the basis of use of the cognitive approach. The presence of closed chains of cause-effect relationships of negative factors and conflicts of target settings of the subjects in the field of district heating is shown. The conceptual model of energy efficient modernization of district heating systems is proposed. This model includes indicators of the current status of heat sources, networks and heat consumers, energetic and economic models, restrictions, procedure of forming and analysis of the mutual influence of the recommended projects. The quantitative data on indicators of the current state of district heating systems of the cities of Ukraine are presented. The interrelation between indicators of the current state and projects of energy efficient modernization of district heating systems is shown. Assessment of energy self-sufficiency of municipal district heating systems on condition of thermal modernization of buildings is carried out. The creation of energy management systems at the district heating enterprises is proposed. Bib. 6, Fig. 7, Tab. 5.

Given the progress of endoscopic surgical techniques detailed knowledge of nasosinusal local anatomy is an essential clinical request for surgeons. In this paper, the aim of the authors is to illustrate the surgical endoscopic technique of sphenoidotomy and to provide a step-by-step description of the main stages  of the surgical intervention.

The International conference “Flora and conservation in the Caucasus: history and current state of knowledge” dedicated to the 130-year anniversary of the Perkalsky Arboretum took place at 22–25 of May 2019 in Pyatigorsk (Stavropol Territory) on the base of the Pyatigorsk Museum of local lore and natural history. The participants were from 11 cities of Russia and 7 Republics of the Caucasus and represented 14 institutions. Proceedings of the conference were published by the beginning of the meeting the book of abstracts includes 49 papers on the study of vascular plants, bryophytes, lichens and fungi, plant communities, as well as the protection of rare and endangered species, unique plant communities, and ecological problems in the Caucasus. The following geobotanical topics were highlighted in 13 papers: forest communities (3 reports), meadow and steppe vegetation (2), xeric open forests (2), communities of ecotone areas (1), structure of populations of rare plant species (3), as well as the history and current status of nature protected areas (2). The great emphasis has been focused on the study of floristic composition and plant populations. Thus, the conference showed that very few studies от vegetation are currently carried out in the Caucasus, and a lot of districts are not affected by the research. The greatest attention is paid to forest vegetation while meadow, steppe, alpine heath and xerophytic communities are studied rather poorly. Besides, there are “white spots” — mire, floodplain and aquatic vegetation. In nowadays, when the anthropogenic impact on the plant cover of the Caucasus is intensively increasing, it is especially important to study natural undisturbed communities preserved in protected natural areas. Another important issue is the conservation of the unique vegetation cover of the whole Caucasus. Thus, the study of vegetation of this region opens a wide field for researchers using various methods of modern plant science.

Radiofrequency ablation (RFA) of liver cancers can be performed safely using percutaneous, laparoscopic, or open surgical techniques, and much of the impetus for the use of RFA has come from cohort series that have provided an evidence base for this technique. Here, we give an overview of the current status of radiofrequency ablation (RFA) for hepatocellular carcinoma (HCC), including its physical properties, to assess the characteristics that make this technique applicable in clinical practice. We review the technical development of probe design and summarize current indications and outcomes of reported clinical use. An accurate evaluation of treatment response is very important to secure successful RFA therapy since a sufficient safety margin (at least 0.5 cm) can prevent local tumor recurrences. We also provide a profile of side effects and information on the integration of this technique into the general management of patients with HCC. To minimize complications of RFA, physicians should be familiar with each feature of complication. Appropriate management of complications is essential for successful RFA treatment. Moreover, adjuvant therapy, such as molecular targeted therapies following curative therapy, is expected to further improve survival after RFA.

 Objective: Cardiovascular diseases are the major cause of morbidity and mortality in the modern era. Myocardial infarction is a condition where there is a significant decrease or block in the blood (oxygen) supply to the part of heart, leading to degeneration of a portion of the myocardium which triggers a cascade of cellular, inflammatory and biochemical events, leading eventually to the irreversible death (necrosis) of heart muscle cells. Various therapeutic interventions, including lifestyle modification, pharmacological treatment options, and surgical techniques are available. The present review focus on the plants that have been evaluated for cardioprotective activity against isoproterenol-induced myocardial infarction.Method: The current status of Cardioprotective plants was obtained from a literature search of electronic databases such as Google Scholar, Pubmed and Scopus up to 2017 for publications on medicinal plants used against isoproterenol-induced myocardial infarction. Isoproterenol, Isoprenaline, myocardial infarction, cardioprotective were used as keywords for the searching.Result: A total of 117 different plant parts and their extracts have till now been published to possess cardioprotection against isoproterenol-induced myocardial infarction. Isoproterenol a beta-adrenergic receptors agonist causes severe stress in myocardium resulting in the infarct-like lesion and produced cardiotoxic effects by elevating the levels of cardiac biomarkers and causing changes in ECG. Plant-based medicines with their antioxidant, antiapoptotic, antihyperlipidemic, platelet antiaggregatory, anti-lipid peroxidation property provide substantial evidence for the management of Ischemia.Conclusion: This review, therefore, provides a useful resource to enable a thorough assessment of the profile of plants that have cardioprotective activity against isoproterenol-induced myocardial infarction.
