ABSTRACT:Objective:To describe a case of Pasteurella multocida meningitis associated with acute disseminated encephalomyelitis (ADEM).Case report:A 33-year-old woman employed in a dog pound presented herself to hospital with fever and meningismus and was found to have culture positive Pasteurella multocida meningitis. Despite appropriate antibiotic treatment her clinical course was characterized by a persistent fever and worsening encephalopathy, which prompted further neurological investigation. Spinal fluid exam and serial MRI scans as well as her one-year clinical course were found to be compatible with ADEM.Conclusion:Persistent fever and worsening encephalopathy in meningitis may indicate a para-infectious immune process such as ADEM, and may serve as indications for further neurological investigation.

Seven children with cystic fibrosis (CF) have been treated for at least one year with intravenously administered soya oil emulsion. In all, an improvement of at least one biochemical abnormality in character with the disease appeared. The children's clinical course remains benign. This course is remarkably better than that of other children with CF treated without Intralipid in Auckland in the same period, though a placebo effect cannot be discounted. It is postulated that intravenous supplementation with essential fatty acid in CF may in turn partially correct an error of metabolism of prostaglandins present in the disease.

The outcomes of hemorrhagic fever with renal syndrome (HFRS) were analyzed in 55 patients who were under the supervision of a physician – infectious within one year after the disease. This issue has been studied in more detail by the example of Hantaan virus infection in the Far East, the effects of Puumala infection aren’t well understood, vigilance among physicians concerning the formation of chronic kidney disease in patients is lacking. However, it is proved that the peculiarities of the clinical course of HFRS are caused by a specific serotype of the virus. It is revealed that the leading clinical manifestation in patients in early reconvalescence period of HFRS was astheno vegetative syndrome. Long-term prognosis of patients with HFRS, isn’t always favorable, it may be associated with prolonged disorders of the kidneys. Renal pathology includes lumbar – pain, eyelid swelling, dysuric phenomena, polyuria, nycturia, proteinuria, hematuria, cylindruria, reposotory. It is established that the disease is accompanied by a prolonged dysfunction of the nervous system, cardiovascular system, hepato-biliary system, this suggests the possibility of development of chronic pathology of different organs and the need for surveillance of HFRS patients, not only the infectious, but the nephrologist, cardiologist, neurologist.

MAGNEZIX® (Syntellix AG, Hanover, Germany) is a biodegradable magnesium-based alloy (MgYREZr) which is currently used to manufacture bioabsorbable compression screws. To date, there are very few studies reporting on a limited number of elective foot surgeries using this innovative implant. This case report describes the application of this screw for osteochondral fracture fixation at the humeral capitulum next to a loose radial head prosthesis, which was revised at the same time. The clinical course was uneventful. Degradation of the magnesium alloy did not interfere with fracture healing. Showing an excellent clinical result and free range-of-motion, the contour of the implant was still visible in a one-year follow-up.

In recent years, there has been a rise in the number of invasive infections caused by Streptococcus pyogenes. The reasons for the observed increased prevalence of the disease are not entirely clear. The present study describes the cases of 6 children hospitalised over a course of one year with a diagnosis of invasive infection due to this pathogen. Three patients were diagnosed with acute post-streptococcal glomerulonephritis and pneumonia (complicated by pleural empyema in 1 patient). Two patients had erysipelas (including one case of recurrent erysipelas), and 1 patient was found to have an abscess in the border area between the femur and soft tissues. The study analyses the clinical course of infection, and highlights the importance of diagnostic tests, particularly the antistreptolysin O titre test, in confirming the infection triggered by Streptococcus pyogenes.

Objective of this study is to see the clinical course of decompensated cirrhosis especially with reference to differences in presentation, age, sex and disease complications regarding the type of virus involved i.e., HCV or HBV. It is a prospective. randomized hospital based study. The patients were selected from inpatient and outpatient department of North medical Ward. The patients were followed up from October 1998 to April 2003. Twenty five patients suffering from decompensated cirrhosis were studied. The patients were followed up till their death or till the last visit. The maximum follow up was for 4 years and 7 months. The minimum follow up was three months and 10 days. Eleven patients died during the study. The average duration of follow up was one year and 4% months. The usual cause of death w as Variceal bleed or hepatic encephalopathy. There is a difference in presentation, age, sex and severity of disease in patients suffering from HCV or HBV. When there is coinfection of HCV and HBV in a patient the prognosis is worse and the patient is having rapid downhill course.

We report a 16-year-old girl with a multiple primary rhabdomyosarcoma of right upper extremity who developed contralateral breast metastasis in her clinical course. She was diagnosed to have multiple primary rhabdomyosarcoma of the right upper extremity with lung metastasis one year prior to finding a mass in her left breast. The excisional biopsy of the breast mass confirmed metastatic rhabdomyosarcoma. Despite aggressive chemotherapy and subcutaneous total mastectomy, she developed a widespread bone and lung metastasis in few months and expired. Metastatic rhabdomyosarcoma of the breast is very rare but it should be considered in adolescent females with primary alveolar rhabdomyosarcoma, specially located on an extremity.

The clinical and neurological study in four neonates infants with cerebral infarction are reported. The purpose of this study is to call attention for the clinical course, cranial ultrasound, computed tomography and laboratories tests, in order to evaluate the neurological sequelae. A careful evaluation has be taken in order to determine the significance of clinical and laboratory tests for syndromic, topographic and etiologic diagnosis after one year ambulatorial follow-up.
