Use of a nitinol stent to treat dysphagia post-laryngopharyngectomy

Permanent dysphagia occurring after laryngectomy or laryngopharyngectomy is uncommon, and when it does occur, can usually be treated by periodic dilatation under general anaesthesia. Occasionally, however, conservative treatment is insufficient, and patients require long-term feeding via a gastrostomy or jejunostomy tube. We describe the case of a man with an anastamotic stricture post-pharyngectomy who underwent insertion of a nitinol stent across the stricture for treatment of dysphagia. The patient’s swallowing was significantly improved in the short-term, however, ultimately, florid granulation tissue formation led to obstruction of the stent and a disappointing long-term result. This, to our knowledge, is the first documentation of the use of a stent to treat dysphagia in a patient post-laryngectomy or pharyngolaryngectomy.

Introduction: Sequelae of chronic foreign body aspiration include occlusive granulation tissue formation, which can cause delayed fixed airway stenosis. The aim of this study is to highlight this complication, which should be diagnosed early and treated proactively. Methods: We present a case series of areca nut aspiration, an organic foreign body commonly chewed in Southeast Asia, complicated by delayed airway stenosis and examine the peculiar nature of areca nut that predisposes to this complication. Results: Long-term sequelae of areca nut aspiration are granulation tissue formation, potentially complicated by airway stenosis due to the mechanical and chemical irritant nature of areca nut. Discussion: Organic impacted foreign bodies such as areca nut can cause delayed complications of airway stenosis. Both the sharp edges of the areca nut and chemical irritation caused by alkaloids released by the nut in constant contact with the airway mucosa can predispose to this long-term complication. Early diagnosis and appropriate therapeutic interventions such as balloon bronchoplasty can help restore airway patency and prevent lung damage.

The authors indicated that pronounced deformities of the ducts of the large salivary glands occur in 70% of patients with chronic sialadenitis. Due to the fact that conservative treatment of ductal sialadenitis, especially pronounced, does not give long-term results, the authors proposed various methods of surgical treatment of ductal sialadenitis, which have a low level of trauma of collateral tissues and provides a stable long-term result.

Idiopathic avascular necrosis (AVN) of the capitate is rare. A 44-year-old woman presented with chronic pain in her dominant wrist without a history of trauma. Clinical and standard radiological examination were initially inconclusive, while MRI was diagnostic. Mid-carpal arthrodesis gave a satisfactory short term result, and the long term result is awaited. Idiopathic AVN of the capitate should be included in the differential diagnosis of chronic wrist pain.

Os tumores vesicais ocorrem mais frequentemente entre a quinta e sétima décadas de vida, sendo raros antes dos 40 anos de idade. O rabdomiossarcoma é um tumor embrionário raro originado do tecido muscular esquelético, que, quando localizado no trato geniturinário, se manifesta com hematúria e sintomas urinários obstrutivos. É um tumor que acomete principalmente a faixa etária infantil. Por se tratar de um tumor muito agressivo geralmente necessita de tratamento cirúrgico, com ressecções parciais e até mesmo remoção completa de grande parte do trato geniturinário baixo. Será relatado um caso de rabdomiossarcoma vesical em criança de 5 anos, cujo tratamento sem cirurgia foi curativo e que com acompanhamento a longo prazo não houve recorrência tumoral. Palavras chaves: Bexiga, rabdomiossarcoma, tratamento conservador  ABSTRACTThe bladder tumors occur most often between the fifth and seventh decades of life, being rare before forty. The embryonal rhabdomyosarcoma is a rare tumor originating from the skeletal muscle tissue, which, when located in the genitourinary tract, manifested with haematuria and urinary obstruction. This tumor is more common in the childhood. It’s a very aggressive tumor and usually requires surgical treatment which partial resection and even the complete removal of much of the lower urinary tract. This is a report of one case of bladder rhabdomyosarcoma in a 5 years old child, whose treatment was curative without surgery, and in the long-term following there was no tumor recurrence. Keywords: Bladder, rhabdomyosarcoma, conservative treatment

ABSTRACTOBJECTIVESCentral airway obstruction (CAO) is caused by various malignant and benign processes. Surgery is a preferred option for CAO, but if not possible, bronchoscopic treatment could be performed. Recently, bronchoscopic treatments have been improved. Particularly in airway stents, new attempts are being made to overcome the existing shortcomings of stents (migration, mucostasis, and granulation tissue formation). We recently developed a new silicone airway stent (GINA stent). The GINA stent has anti-migration design, dynamic structure enabling reduction of stent cross-sectional area, and radiopaqueness. We sought to evaluate mechanical characteristics and performance of our novel GINA stent in a pig tracheal stenosis model.METHODSAll tests were performed by comparing GINA stent [outer diameter (OD, mm) 14, length (L, mm) 55] with Dumon stent (OD14L50). Mechanical tests were done using digital force gage to determine the anti-migration force, expansion force, and flexibility. Short-term (3 weeks) performance was evaluated after stent implantations [GINA (n = 4) vs. Dumon (n = 3)] in a pig model of tracheal stenosis.RESULTSMechanical properties outcomes for GINA vs. Dumon: anti-migration force [18.4 vs. 12.8 Newton (N)]; expansion force (11.9 vs. 14.5 N); flexibility (3.1 vs. 4.5 N). Short-term (3 weeks) GINA vs. Dumon performances: mucus retention (0/4 vs. 0/3); granulation tissue formation (0/4 vs. 0/3); migration (1/4 vs. 2/3).CONCLUSIONSGINA stent demonstrated better mechanical properties than Dumon stent with a stent performance not inferior to Dumon stent.

Patella tendinopathy condition is often resistant to conservative treatment. We report a case of patella tendinopathy treated by distalisation of the tibial tubercle with excellent long-term result.

Bilateral choanal atresia (CA) is a rare congenital malformation frequently associated with other anomalies. CHARGE association is closely linked to bilateral CA. The aim of this study was to describe the outcomes of the endoscopic repair in bilateral CA, and to assess the role of postoperative nasal stenting in two cohorts of CHARGE-associated and non-syndromic CA. Thirty-nine children were retrospectively analyzed (16 patients had CHARGE-associated CA). The rate of postoperative neochoanal restenosis was 31.3% in the CHARGE population, and 47.8% in the non-syndromic CA cohort. Data on postoperative synechiae and granulation tissue formation, need for endonasal toilette and dilation procedures, and number of procedures per patient were presented. Stent positioning led to a higher number of postoperative dilation procedures per patient in the non-syndromic cohort (p = 0.018), and to a higher rate of restenosis both in the CHARGE-associated, and non-syndromic CA populations. Children with CHARGE-associated and non-syndromic bilateral CA benefitted from endonasal endoscopic CA correction. The postoperative application of an endonasal stent should be carefully evaluated.
