Abstract Background: The under-five mortality rate is the probability that a newborn will die before reaching the age of five years. It is usually expressed as a rate per 1000 live births. Nearly 7 million children worldwide die before their fifth birthdays, with almost all of such death occurring in developing countries Objective: To assess the trend and factors associated with under-five mortality in Ethiopia further analysis of 2000-2016 Ethiopian demographic and health survey: a decomposition analysis Methods: A secondary serial cross-sectional analysis was conducted utilizing data from four rounds of EDHS administered in 2000, 2005, 2011 and 2016. The Ethiopian health and demographic survey in 2000-2016 used a two-stage stratified sampling technique to select a nationally representative sample. In the first stage the total number of enumeration areas was 539 in 2000, 540 in 2005, 624 in 2011 and 645 EAs in 2016 were selected with probability proportional to EA size and with independent selection in each sampling stratum. In the second stage of selection, a fixed number of 28 households per cluster were selected with an equal probability systematic selection from the newly created household listing. Result: Looking at the overall trend, Ethiopia has shown a significant decrease in under-five mortality over the study period from 166 per 1000 births in 2000 to 67 per 1000 birth in 2016 with annual rate of reduction of 3.7%. The trends in under-five mortality showed a variation according to their characteristics. Among home delivery, the largest decrement was observed around 6.5% decrease from 2000 to 2016 and in health facility delivery decreased by 8.3% The decomposition analysis shows that 100.74% of the decrease in under-five mortality was accredited to the difference in the effects of characteristics. Conclusion: Under-five mortality decreased significantly over the last one and a half decades. One of the remarkable findings from the decomposition analysis is the effect of education, place of delivery and maternal age. the government of Ethiopia shall do all daughters will be educated and to build health facilities the whole place of the country Keywords: Under-five mortality decomposition analysis, trend

Background. Underweight is one of the paramount major worldwide health problems, and it traces a big number of populations from infancy to old age. This study aimed to analyze the trends and predictors of change in underweight among children under five years in Ethiopia. Method. The data for this study were accessed from three Ethiopian Demographic and Health Survey data sets 2005, 2011, and 2016. The trend was examined separately for the periods 2005–2011, 2005–2016, and 2011–2016. Multivariate decomposition analysis of change in underweight was employed to answer the major research question of this study. The technique employed the output from the logistic regression model to parcel out the observed difference in underweight into components, and STATA 14 was utilized for data management and analysis. Result. Perceiving the overall trend, the rate of underweight was decreased from 38% in 2005 to 24% in 2016. The decomposition analysis results revealed that, about 12.60% of declines in underweight have been explained by the difference in population characteristics or endowments (E) over the study period. The size of the child at birth, husband’s education, women’s education, and household wealth index contributed significantly to the compositional decline in underweight. Conclusion. The magnitude of underweight among children under five years indicates a remarkable decline over the last ten years in Ethiopia. In this study, two-twelfth of the overall decrease in underweight among children under five years over the decade was due to the difference in characteristics between 2005 and 2016. Continuing to educate the population and boost the population’s economy is needed on the government side in Ethiopia.

Background. Long-acting contraceptive methods, subdermal implants, and intrauterine devices are reliable, safe, and cost-effective family planning methods. However, these methods are not widely used in Ethiopia despite government effort to increase access. The study is aimed at assessing the rate of utilization of long-acting contraceptive methods among married women and associated factors. Method. We analyzed the 2016 Ethiopian Demographic and Health Survey dataset. A total of 2045 married women of reproductive age group, who were using any modern contraceptive method at the time of the survey, were included in the study. Descriptive statistics were computed to characterize the study participants. Bivariate and multivariable logistic regression analyses were performed to identify associated factors, reporting odds ratios (ORs) and 95% confidence intervals (95% CIs). Result. The multivariable analyses showed that women educational status, parity, religion, previous history of abortion, desire for more child, and region where the respondents reside were significantly the factors that determine the utilization of long-acting contraceptive. There is a significant regional disparity in long-acting contraceptive utilization. Compared to women residing in Tigray region, those who live in other regions (Afar-Somali, Oromia, Amhara, Benishangul Gumz-Gambela, and Southern Nations Nationalities and People) have low likelihood of using long-acting contraceptive methods. Conclusion. Utilization of long-acting family planning method is low in Ethiopia. There is a significant regional disparity in utilizing these methods. Policy makers should promote culture-sensitive and tailored interventions to improve the utilization of long-acting family planning methods.

Abstract Background Though child mortality has dropped remarkably, it is considerably high in South Asia. Across the globe, 5.2 million children under 5 years of age died in 2019, and India accounts for a significant portion of these deaths. Common childhood illnesses are the leading cause of these deaths. Seeking care from formal providers can reduce these avoidable deaths. Inequity is a crucial blockage in optimum utilization of medical treatment for children. Hence, the present study analyzes the inequalities and horizontal inequities in utilizing the medical treatment for diarrhea, fever, acute respiratory infection (ARI), and any of these common childhood illnesses in India and across the Indian states. The study also attempts to locate significant contributors to these inequalities. Methods The study used 0 to 59 months children’s data sourced from the Demographic and Health Survey, India (2015–16). Concentration Index (CI) and Erreygers Corrected Concentration Index (EI) were used to measure the inequalities. The Horizontal Inequity Index (HII) was deployed to estimate inequity. The decomposition method introduced by Erreygers was applied to determine the significant contributors of inequalities. Results The EI in medical treatment-seeking for common childhood illnesses was 0.16, while the HII was 0.15. The highest inequality was perceived in the utilization of medical treatment for ARI (0.17). The primary contributing factors of these inequalities were continuum of maternal care (18.7%), media exposure (12%), affordability (9.3%), place of residence (9.1%), mother’s education (8.5%), and state groups (8.8%). The North-Eastern states showed the highest level of inequality across the Indian states. Conclusion The study reveals that the horizontal inequity in medical treatment utilization for children in India is pro-rich. The findings of the study suggest that attuning the efforts of existing maternal and child health programs into one seamless chain of care can bring the inequalities down and improve the utilization of child health care services. The spread of health education through different media sources, reaching out to rural and remote places with adequate health personnel, and easing out the financial hardship in accessing medical treatment could be the cornerstone in accelerating the utilization level amongst the impoverished children.
