Atopic Dermatitis has long been a controversial entity in regard its relationship to food allergy. Indeed, inter-discipline disparity in the way dermatologists and allergologists perceive the food allergy/atopic dermatitis interplay, hampers the design of concise therapeutic strategies and conveys conflicting messages to the patients. Within this conceptual frame, food exclusion regimes are rendered a contentious option. On the basis of this acknowledgment, we opted to put the emphasis on the discrepant perceptions surrounding such therapeutic regimes and to share our view pertaining to their appropriate implementation.

Tacrolimus ointment is an agent approved for the treatment of atopic dermatitis; however, tacrolimus has been expected also as one of the promising therapeutic strategies for other T-cell mediated inflammatory skin disorders. Recent progress have demonstrated that topical tacrolimus shows beneficial effects for psoriasis depending on sites. In particular, facial, intertriginous, and genital psoriasis respond to topical tacrolimus dramatically in a short period. Further, topical tacrolimus is tolerable also for child psoriasis. Because topical tacrolimus does not induce either skin atrophy or telangiectasia different from corticosteroids, it is recommended to be a first choice. The most proper way of topical tacrolimus therapy at present is to use this ointment intermittently after the remission was once obtained, paying attention to its adverse effects.

Patients with Gorham–Stout disease (GSD) present progressive destruction and resorption of bone. Typical bone-related symptoms include swelling, pain and functional impairment in the region involved. The three aspects of GSD etiopathology are osteoclasts, angiogenesis/lymphangiogenesis and osteoblast function. Multi-targeted pharmacological approach includes innovative options and represent milestones of treatment, sometimes associated with radiotherapy. Surgery is mainly used to treat complications: pathologic/impending fractures, spinal instability or deformities and chylothorax. In this narrative review, we highlight current standards in diagnosis, clinical management and therapeutic strategies.

There are presented data on the participation of vitamin D in the formation of various forms of allergic pathology in children. The spectrum of knowledge about the biological properties of vitamin D has recently expanded significantly. There are described peculiarities of metabolism of vitamin D and its participation in the leading links of the pathogenesis of atopic dermatitis (AD): in the regulation of the immune response, violations of the integrity of the epidermal barrier and the realization of allergic inflammation in the skin. The pathogenetic significance of vitamin D deficiency in the formation of bronchial asthma (BA) is discussed. Possibilities of the use of vitamin D and its analogues in AD and BA patients are indicated. The authors believe the compensation for vitamin D (hormone) D deficiency in allergic pathology to be an important preventive and curative task within the framework of therapeutic strategies aimed at improving the quality of life of sick children.

The literature review covered clinical, pathogenic and therapeutic aspects of comorbidity of atopic dermatitis (AD) and bronchial asthma (BA) in children. On the basis of data on pathogenesis of diseases, characteristics of AD in children with BA it is justified that there is a need to use common therapeutic strategies in data management of comorbid patients, which can reduce pharmacological load and improve treatment efficiency.
