The Pediatric Alliance for Coordinated Care: Evaluation of a Medical Home Model

Objectives. The American Academy of Pediatrics recommends a medical home for children with special health care needs (CSHCN). In the Pediatric Alliance for Coordinated Care (PACC), 6 pediatric practices introduced interventions to operationalize the medical home for CSHCN. The intervention consisted of a designated pediatric nurse practitioner acting as case manager, a local parent consultant for each practice, the development of an individualized health plan for each patient, and continuing medical education for health care professionals. The objectives of this study were 1) to characterize CSHCN in the PACC, 2) to assess parental satisfaction with the PACC intervention, 3) to assess the impact on hospitalizations and emergency department episodes, and 4) to assess the impact on parental workdays lost and children’s school days lost for CSHCN before and during the PACC intervention. Methods. A total of 150 CSHCN in 6 pediatric practices in the Boston, Massachusetts, area were studied. Participants were recruited by their pediatricians on the basis of medical/developmental complexity. Physicians completed enrollment information about each child’s diagnosis and severity of condition. Families completed surveys at baseline and follow-up (at 2 years), assessing their experience with health care for their children. Results. A total of 60% of the children had >5 conditions, 41% were dependent on medical technology, and 47% were rated by their physician as having a “severe” condition. A total of 117 (78%) families provided data after the intervention. The PACC made care delivery easier, including having the same nurse to talk to (68%), getting letters of medical necessity (67%), getting resources (60%), getting telephone calls returned (61%), getting early medical care when the child is sick (61%), communicating with the child’s doctor (61%), getting referrals to specialists (61%), getting prescriptions filled (56%), getting appointments (61%), setting goals for the child (52%), understanding the child’s medical condition (56%), and relationship with the child’s doctor (58%). Families of children who were rated “severe” were most likely to find these aspects of care “much easier” with the help of the pediatric nurse practitioner. Satisfaction with primary care delivery was high at baseline and remained high throughout the study. There was a statistically significant decrease in parents missing >20 days of work (26% at baseline; 14.1% after PACC) and in hospitalizations (58% at baseline; 43.2% after PACC). The approximate cost per child per year of the intervention was $400. Conclusions. The PACC medical home intervention increases parent satisfaction with pediatric primary care. Those whose needs are most severe seem to benefit most from the intervention. There are some indications of improved health as well as decreased burden of disease with the intervention in place. The PACC model allows a practice to meet many of the goals of serving as a medical home with a relatively small financial investment.

Abstract Background Integrated primary care teams are ideally positioned to support the mental health care needs arising during the COVID-19 pandemic. Understanding how COVID-19 has affected mental health care delivery within primary care settings will be critical to inform future policy and practice decisions during the later phases of the pandemic and beyond. The objective of our study was to describe the impact of the COVID-19 pandemic on primary care teams’ delivery of mental health care. Methods A qualitative study using focus groups conducted with primary care teams in Ontario, Canada. Focus group data was analysed using thematic analysis. Results We conducted 11 focus groups with 10 primary care teams and a total of 48 participants. With respect to the impact of the COVID-19 pandemic on mental health care in primary care teams, we identified three key themes: i) the high demand for mental health care, ii) the rapid transformation to virtual care, and iii) the impact on providers. Conclusions From the outset of the COVID-19 pandemic, primary care quickly responded to the rising mental health care demands of their patients. Despite the numerous challenges they faced with the rapid transition to virtual care, primary care teams have persevered. It is essential that policy and decision-makers take note of the toll that these demands have placed on providers. There is an immediate need to enhance primary care’s capacity for mental health care for the duration of the pandemic and beyond.

The increase in population of the United States is occurring at a much more rapid rate than the increase in medical and nursing personnel available to maintain health services at an optimum level. Unless the pattern of furnishing health care, particularly to lower socioeconomic groups in both urban and rural areas, is drastically improved, these groups will suffer from increasingly inadequate health supervision. This paper describes an educational and training program in pediatrics for professional nurses (the “pediatric nurse practitioner” program), which prepares them to assume an expanded role in providing increased health care for children in areas where there are limited facilities for such care.

The recent letter to Pediatric Nurse Associates and members of the American Academy of Pediatrics reporting the division between the AAP and American Nurses Association on certification, is an unfortunate and deplorable happening. Since the onset of the first Pediatric Nurse Practitioner Program in 1965, more than 1,000 nurses have graduated from over 45 programs adding a new dimension to care for children. Although touted by many as the answer to manpower problems for child health care, evidence over the past five years indicates that a new dimension has been added to pediatric care.

Summary Background: Primary care delivers patient-centred and coordinated care, which should be quality-assured. Much of family practice now routinely uses computerised medical record (CMR) systems, these systems being linked at varying levels to laboratories and other care providers. CMR systems have the potential to support care. Objective: To achieve a consensus among an international panel of health care professionals and informatics experts about the role of informatics in the delivery of patient-centred, coordinated, and quality-assured care.Method: The consensus building exercise involved 20 individuals, five general practitioners and 15 informatics academics, members of the International Medical Informatics Association Primary Care Informatics Working Group. A thematic analysis of the literature was carried out according to the defined themes. Results:The first round of the analysis developed 27 statements on how the CMR, or any other information system, including paper-based medical records, supports care delivery. Round 2 aimed at achieving a consensus about the statements of round one. Round 3 stated that there was an agreement on informatics principles and structures that should be put in place. However, there was a disagreement about the processes involved in the implementation, and about the clinical interaction with the systems after the implementation. Conclusions: The panel had a strong agreement about the core concepts and structures that should be put in place to support high quality care. However, this agreement evaporated over statements related to implementation. These findings reflect literature and personal experiences: whilst there is consensus about how informatics structures and processes support good quality care, implementation is difficult.

The health-care acquired infections (HCAIs) occur world-wide among persons undergoing medical attention in health institutions and result in unexpected long-term stay, disability and financial loses. The most predominant infections are catheter associated urinary tract, central line associated, surgical site and ventilator associated pneumonia infections. The patients are prone to infections during hospitalization from varied environmental sources, hands of health-care professionals, medical equipment and other infected patients. The frequent factors affecting patients on admission are improper hand hygiene, contact with infected patients, adverse drug events and surgical complications. Patients under health-care delivery can acquire infection disseminated from food, water, aerosols and hospital wastes. The application of personal protective equipment, routine educational interventions are common approaches that can help stop HCAIs and save lives, decrease death rate and health delivery expenses. In buttressing this, the World Health Organization (WHO) enunciated guidelines to enhance hand washing practices, infection prevention and control programme, monitored use of antibiotics and its resistance. The other measures included global adoption of efficient surveillance system and the impact of relevant stakeholders in health sectors needed to prevent and control hospital acquired infections.

The Patient-Centered Medical Home (PCMH) involves an organizing set of principles for the delivery of primary care that has become widely accepted by primary care medical associations, policymakers, businesses, and insurance companies. The PCMH places the patient at the center of the delivery of care. One physician directs the patient’s care. The PCMH emphasizes a whole-person orientation and coordinated care, including integration of physical health with behavioral health. Care is coordinated across the health care system. Use of a PCMH enhances access to medical care and ensures quality and safety. This chapter reviews the history of the PCMH and discusses joint principles of the PCMH; models of care, space, and payment; and the newer concept of the “medical neighborhood.”

Managing chronic illness is highly complex and the pathways to access health care for the patient are unpredictable and often unknown. While multidisciplinary care (MDC) arrangements are promoted in the Australian primary health care system, there is a paucity of research on multidisciplinary collaboration from patients’ perspectives. This exploratory study is the first to gain an understanding of the experiences, perceptions, attitudes and potential role of people with chronic illness (asthma) on the delivery of MDC in the Australian primary health care setting. In-depth semi-structured interviews were conducted with asthma patients from Sydney, Australia. Qualitative analysis of data indicates that patients are significant players in MDC and their perceptions of their chronic condition, perceived roles of health care professionals, and expectations of health care delivery, influence their participation and attitudes towards multidisciplinary services. Our research shows the challenges presented by patients in the delivery and establishment of multidisciplinary health care teams, and highlights the need to consider patients’ perspectives in the development of MDC models in primary care.

Involvement in and reflections upon nine years of change in nursing and health care provide the framework for this commentary on the article, "Nurse Practitioners for Children—Past and Future" by McAtee and Silver.1 My earlier association with Silver as a co-director of the first pediatric nurse practitioner project at the University of Colorado makes these comments, hopefully, like conversations and challenges between colleagues. My remarks address those issues concerned with establishing priorities in the preparation of teacher-practitioners, the development of interdisciplinary collaboration, the need for studies of effectiveness of nurse practitioners, and an opinion on the recommendation to prepare "assistant nurse practitioners."
