Prenatal exposure to stilbestrol and adenocarcinoma of the female genital tract: The pediatrician's responsibility

In 1970 Herbst and Scully1 reported seven cases of adenocarcinoma of the vagina in adolescents, of which six were of the clear-cell type. In a followup paper,2 an association was proposed between the occurrence of this previously rare tumor in young women and their exposure in utero to diethylstilbestrol (DES). This startling conclusion was immediately strengthened by reports of additional cases, and soon adenocarcinoma of the cervix also was linked to in utero DES exposure.3 These reports have been popularized by the media, and some obstetricians have recommended routine, repeated vaginal examination, biopsy, culposcopy, etc. for all females who were exposed in utero.4-6

ObjectivesTo assess the outcome and the fertility aspects in the particular population of cervical and/or vaginal clear cell adenocarcinoma (CCA) associated or not to previous in utero diethylstilbestrol exposure.MethodsFrom January 1970 to December 2003, data from 61 consecutive patients with cervical and/or vaginal histologically proven CCA treated with brachytherapy (BT) aimed at a conservative treatment at the Institut Gustave Roussy as a part of treatment were retrospectively analyzed.ResultsThe median follow-up was 9.4 years, ranging from 0.3 to 27.4 years. The 5-year specific overall survival rate was 79%. The median time of disease-free survival was 5.8 years. In the subgroup of 42 patients with a cervical CCA, 12 patients tried to be pregnant, 2 patients became pregnant and had miscarriages (P2M2 and P1M1). No pregnancy has been observed in 10 patients exclusively owing to anomalies of the reproductive tract: 6 patients had partial or total diaphragm, 2 patients had an anatomical alteration of the uterus, 1 patient had atrophic endometrium, and 1 patient had primary infertility. In the subgroup of 19 patients with a vaginal CCA, 7 patients tried to be pregnant. All of them had no morphological and/or functional anomalies of the genital tract. Three of 7 patients had delivered healthy babies (P1D1, P2D2, and P3M2D1), and another one had a miscarriage (P1M1). Moreover, all babies were delivered by cesarean section. The pregnancy rate was 10% (6 of 61 patients) with 3 healthy babies.ConclusionConservative approach in patients with female genital tract CCA including BT gives good results with good survival rates and an interesting global pregnancy rate.

Since in utero exposure to diethylstilbestrol (DES) is known to cause abnormalities of the female genital tract later in life, exposed male offspring were located, surveyed by mail, and compared with unexposed male offspring from the same period and medical practices. The exposed and unexposed respondents appeared comparable and did not differ in their response to most medical questions. However, a larger proportion of exposed than of unexposed boys had experienced problems in passing urine (12.9% vs. 1.8%; P = .0003) and abnormalities of the penile urethra (4.4% vs. 0%; P = .017).

We report the case of female genital tract enterobiasis. The patient is a pediatrician at a large general hospital and was suffering from nocturia, dysuria and vaginal itching. Vaginal examination showed mild inflammation and normal hematogram. The midstream urine and the culture of vaginal discharge were negative. The Gram-stained microscopic examination of the vaginal discharge showed normal numbers of galactobacilli and absence of fungus, but the microscopic examination of fresh preparation of vaginal discharge revealed 3–4 leukocytes/mm3 and E. vermicularis of about 4 mm. The patient was treated with mebendazole 100 mg PO bid for 3 days and all the symptoms disappeared. In order to prevent possible re-infection, the treatment was repeated with a further two courses. After three months the cellotape test was negative.

The seminal plasma (SP) modulates the female reproductive immune environment after mating, and microRNAs (miRNAs) could participate in the process. Considering that the boar ejaculate is built by fractions differing in SP-composition, this study evaluated whether exposure of mucosal explants of the sow internal genital tract (uterus, utero-tubal junction and isthmus) to different SP-fractions changed the profile of explant-secreted miRNAs. Mucosal explants retrieved from oestrus sows (n = 3) were in vitro exposed to: Medium 199 (M199, Control) or M199 supplemented (1:40 v/v) with SP from the sperm-rich fraction (SRF), the post-SRF or the entire recomposed ejaculate, for 16 h. After, the explants were cultured in M199 for 24 h to finally collect the media for miRNA analyses using GeneChip miRNA 4.0 Array (Affymetrix). Fifteen differentially expressed (False Discovery Rate (FDR) < 0.05 and Fold-change ≥ 2) miRNAs (11 down- versus 4 up-regulated) were identified (the most in the media of uterine explants incubated with SP from post-SRF). Bioinformatics analysis identified that predicted target genes of dysregulated miRNAs, mainly miR-34b, miR-205, miR-4776-3p and miR-574-5p, were involved in functions and pathways related to immune response. In conclusion, SP is able to elicit changes in the miRNAs profile secreted by female genital tract, ultimately depending SP-composition.
