OBJECTIVE: To identify the vulnerabilities of children admitted to a pediatric inpatient unit of a university hospital.METHODS: Cross-sectional, descriptive study from April to September 2013 with36 children aged 30 days to 12 years old, admitted to medical-surgical pediatric inpatient units of a university hospital and their caregivers. Data concerning sociocultural, socioeconomic and clinical context of children and their families were collected by interview with the child caregiver and from patients, records, and analyzed by descriptive statistics.RESULTS: Of the total sample, 97.1% (n=132) of children had at least one type of vulnerability, the majority related to the caregiver's level of education, followed by caregiver's financial situation, health history of the child, caregiver's family situation, use of alcohol, tobacco, and illicit drugs by the caregiver, family's living conditions, caregiver's schooling, and bonding between the caregiver and the child. Only 2.9% (n=4) of the children did not show any criteria to be classified in a category of vulnerability.CONCLUSIONS: Most children were classified has having a social vulnerability. It is imperative to create networks of support between the hospital and the primary healthcare service to promote healthcare practices directed to the needs of the child and family.

Relatar a experiência vivenciada por discentes e docentes de enfermagem na realização de atividades extensionistas para o aumento da adesão à identificação correta do paciente. Relato de experiencia acerca da importância da identificação correta do paciente realizadas em um hospital universitário do Estado do Rio de Janeiro de setembro de 2019 a março de 2020. Participaram profissionais de saúde, pacientes e acompanhantes. Para profissionais de saúde, foram desenvolvidos treinamentos interativos. Para pacientes e acompanhantes, elencou-se orientações acerca da importância da identificação do paciente. Realizados dezoito treinamentos, a maioria com técnicos de enfermagem (54 - 37,76%) e enfermeiros (23 - 16,08%). Alcançadas 2.050 orientações, sendo com 998 (48,68%) pacientes. As atividades ocorreram em 32 unidades de internação. Pode-se contribuir com atividades da Meta 1 de Segurança do Paciente, além de destacar a importância de Projetos de Extensão Universitária para melhor assistência. Descritores: Segurança do Paciente, Sistemas de Identificação de Pacientes, Capacitação em Serviço, Hospitais Universitários. Educational actions for correct patient identification: experience reportAbstract: To report the experience lived by nursing students and teachers in carrying out extension activities to increase adherence to the correct identification of the patient. Experience report about the importance of correct patient identification performed at an university hospital in the State of Rio de Janeiro from September 2019 to March 2020. The group of participants was health professionals, patients and companions. For health professionals, was developed interactive training. For patients and companions, guidelines were listed on the importance of patient identification. Eighteen training sessions were carried out, most with nursing technicians (54 - 37.76%) and nurses (23 - 16.08%). 2,050 guidelines were reached, with 998 (48.68%) patients. The activities took place in 32 inpatient units. Objective: To verify if the nursing notes improved before and after a training of short duration and the factors those determine this change. Method: We analyzed 1551 reports/nursing notes in two inpatient units and two intensive care units. There was used a form in which there were recorded the adequacies or otherwise of the annotations in function of: the course, shift, professional category, and days of hospitalization of the patient. Results: We observed that there was improvement in the records made after the training course, with an increase of the adequacies. The intensive therapy units showed higher scores for appropriateness to the wards. The time of hospitalization of the patient eventually influences the scores of adequacies. The score of appropriateness of annotations was not different between the professional categories. Conclusion: The action of training of short duration was effective in improving the nursing notes, strengthening the role of continuous education in health services.

Context: Cataract is the leading cause of poor vision worldwide. Patients who knowledgeable and skillful nurses well prepare are better prepared to engage in appropriate self-care activities post-cataract surgery. Aim: This study aimed to assess the effectiveness of nursing intervention protocol on nurses' performance and patients’ self-care after cataract surgery. Methods: The study followed a quasi-experimental, pretest-posttest design. The study was conducted in ophthalmology surgical inpatient units and the outpatient clinics at Benha University Hospital. The sample consisted of all available nurses (35) working in the ophthalmology surgical inpatient units, and the outpatient clinics who are willing and agreed to participate in the study and a convenient sample consisted of 50 patients of both genders were also included in the current study before implementing nursing intervention protocol. Three tools were used to conduct the study: A structured interview questionnaire, nurses' practice checklist, and patients’ self-care activity checklist. Results: The results showed statistically significant improvements in nurses' performance (knowledge and practice) immediately post and one-month follow-up post-nursing intervention protocol compared with pre-nursing intervention protocol implementation (p<0.001). General improvement in patients’ self-care activities (41.6±11.042) increased significantly to (64.2±13.65) after nursing intervention protocol implementation at p=0.001. However, after one month of implementing the nursing intervention protocol, a slight decline occurred after one month (52.46±10.97) compared to pre intervention level. Highly statistically significant differences were observed at a p-value ≤0.001. Conclusion: Findings of this study conclude that the nursing intervention protocol was effective in improving nurses' performance, which was reflected in improving the practice of patients' self-care activities cared for by nurses exposed to nursing intervention protocol implementation.  It is recommended that further studies are suggested to investigate the outcome of the implementing nursing intervention protocol on decreasing the occurrence of complications post-cataract surgery.

Objective: to know the perception of nurses about the application and purpose of clinical reasoning in the care of hospitalized adults with chronic diseases. Methodology: exploratory-descriptive study, with a qualitative approach. It was carried out with nurses working in adult inpatient units of a University Hospital in southern Brazil. Data collection took place from july to december 2018, through semi-structured interviews. The data were analyzed using the Thematic Content Analysis. Results: it was found that the nurses' perception of clinical reasoning was applied (in)consciously in care and management activities. Its purpose was self-reported in the qualification of care, patient comfort and prevention of complications. Conclusion: these findings are believed to be guiding instruments for questioning, analysis, synthesis, interpretation, inference, inductive and deductive reasoning, intuition, application and creativity for nurses' decision-making in care practices for adult hospitalized patients with chronic diseases.

ABSTRACT Objective: to size the Nursing staff in clinical, surgical and pediatric inpatient units. Method: a retrospective cross-sectional study, carried out in a university hospital in Midwest Brazil. The data were obtained from daily classifications of the care complexity level (n=5,808) among patients (n=882) hospitalized in the referred units, from July to October 2019, in addition to occupancy and hospital stay indicators. Descriptive statistical analysis, parameters and an equation specific to the design were used. Results: clinical hospitalization had the highest occupation, permanence, demand for Nursing hours/day (162.58) and deficit of nurses (-12), followed by the pediatric unit (-2). In the clinical and surgical units, there was the same available/real number of nurses. The general staff sized showed personnel surplus (+24), due to the high number of mid-level professionals in all the sectors. Conclusion: expected sizing inadequacy was verified, as there was a mismatch between workload and allocation of professionals in the units.

Abstract Objective This research was conducted to determine the psychometric characteristics of the “Spiritual Needs Assessment Scale of Patients with Cancer” in Turkish patients with cancer. Methods This study used psychometric methods to test the adapted tool. The participants of this study were patients with a diagnosis of cancer in the outpatients and inpatients medical oncology and hematology clinics of a university hospital in Turkey. 400 patients determined by power analysis were included in the study. The data were collected in 2018. We used the descriptive form and “Spiritual Needs Assessment Scale of Patients with Cancer.” The obtained data were evaluated using Cronbach's α reliability coefficient, Pearson's correlation coefficient, and factor analysis. Results In the present study, the factor loads of the items were sufficient and explained 39.18% of the total variance. Cronbach's α value of the scale was 0.85. Significance of results As a result, it was found that the validity and reliability of the “Spiritual Needs Assessment Scale of Patients with Cancer” were ensured and they could be used in Turkish patients.
