The objective: optimization of treatment of clinical symptoms and prevention of relapse infetsii virus (HSV, HPV) using immunomodulatory therapy alloferon. Patients and methods. The study included 50 patients aged 20 to 42 years with clinical manifestations of HPV in the form of warts of vulva, vagina, cervix, cervical dysplasia. Patients were divided into two groups according to the treatment received – the main group received Allokin-Alpha 1ml subcutaneous 6 injections, the comparison group – a-2b interferon 3 million 10 injections. Results. The group of patients treated with Allokin-аlfa, showed better clinical performance treatment of genital kondilomas lesions, decrease in the number of lesions in 6–12 days after initiation of therapy, decrease in the degree of dysplastic changes in the cervix, and the absence of relapse (in the control group 32% of patients had a relapse). No side effects as a result of Allokin-alpha has not been registered, while 76% of patients receiving a-interferon-2b expressed flu-like symptoms were observed, requiring additional purpose of detoxification drugs. Test results: in the main group had the elimination of HPV in 92% of cases, 78% in the control group; herpes – elimination of 96% of the cases in the main group and 75% in the comparison group. Conclusion The medicine Allokin-alpha is highly effective in the treatment of HPV and HSV infection, demonstrating good results of therapy, no relapse of infection. The recommended regimen – 6 injections of the drug prior to the electrosurgical treatment. Key words: human papillomavirus infection, herpes, Allokin-alpha, a-interferon-2b.

The article presents the results of a comparative randomized study, the purpose of which was to evaluate the effectiveness of the use of aminodihydrophthalasindione sodium (Galavit, LLC SELVIM, Russia) in the treatment of patients undergoing an abortion. Included in the study, 48 women were divided into two groups, 24 patients of the main group in addition to the standard rehabilitation were treated with aminodihydrophthalasindione sodium in the comparison group – 24 patients underwent only standard rehabilitation. In this study, all patients (100%) of the main group who were treated with aminodihydrophthalasindione sodium in addition to the standard therapy marked reduction of the clinical symptoms of the disease and positive dynamics was observed at ultrasound. In the control group, the full clinical effect of treatment was observed only in 10 patients (52.6%). 9 women (47,4%) required repeated therapy. Ultrasound studies in 12 patients (63.2%) showed changes equivalent to endometritis.

Goal. Improving the scheme of treatment and prevention of periodontal diseases in workers of the woodworking industry of Chernivtsi region, who in the process of professional activity have long-term contact with unfavorable factors of the production environment. Materials and methods. 70 workers of the woodworking industry with diagnosed chronic generalized periodontitis of the I degree were treated (35 people – the main group (A); 35 people – comparison group (B)), age – from 25 to 45 years. To compare the results of laboratory studies, an additional survey of 25 healthy individuals of the same age with an intact periodontium were conducted. They formed the control group (C). Clinical examination of patients was performed according to standart methods: subjective (complaints, medical history, life history) and objective (examination, periodontal indices, determination of the level of gingival attachment). Laboratory methods of research included determination of urease and lysozyme activity in saliva, degree of dysbiosis of oral cavity. As maintenance therapy, patients of the main group were prescribed the proposed composition DEPE. Results. After the treatment, a significant improvement in the hygienic status of patients in both groups was observed, but difference between groups A and B in the indicators of oral hygiene after treatment was no statistically significant (pA2–B2 > 0.05). There was a significant improvement in periodontal indices after treatment in patients of both groups (PMA index according to C. Parma, bleeding index according to Muhlemann in Cowell I. modification, Russell periodontal index, PSR-test), but the indicators in patients of the main group were significantly better – pA2–B2 < 0.05. In patients of the main group (A) the level of attachment loss decreased by 1.83 times, comparison group (B) – by 1.71 times. The difference in levels of attachment loss after treatment between groups A and B is statistically significant (pA2–B2 < 0.05). The activity of the enzyme urease in patients of the main group (A) and comparison group (B) after treatment was significantly reduced (pA1–A2 < 0.001, pB1–B2 < 0.001), but only in the main group it reached the level in the control group (C) (pA2–C > 0.05). The level of lysozyme after treatment was significantly increased in patients of both observation groups (pA1–A2 < 0.001, pB1–B2 < 0.001), and reached the level of lysozyme in patients of the control group (C) (pA2–C > 0.05, pB2–C > 0.05). The degree of oral dysbiosis in patients of the main group (A) was significantly reduced by 5.43 times (pA1–A2 < 0.001), and its difference from the degree of dysbiosis in patients of the control group (C) is statistically insignificant (pA2–C > 0.05 ). The degree of oral dysbiosis in patients of the comparison group (B) was significantly reduced by 3.04 times (pB1–B2 < 0.001), but still remained significantly different from oral dysbiosis in patients of the control group (C) (pB2–C < 0.05). Conclusion. The proposed pharmacological composition DEPE is an effective antiseptic solution and can be used in the phase of maintenance therapy to improve the treatment and prevention of periodontal disease in workers of woodworking industry. The variability of clinical course and frequent exacerbations of chronic rhinosinusitis make it necessary to improve conservative therapy. According to current literature and EPOS 2020 data the main trends are aimed to improve drug treatment, while physiotherapy is rarely applied. The aim of the study was to increase clinical effectiveness of chronic rhinosinusitis exacerbations treatment by using low-frequency ultrasonic cavitation. Clinical examination and treatment of 57 patients with exacerbation of chronic rhinosinusitis and formation of two groups was carried out. The main group (34 patients), in addition to the conventional treatment, received a course of low-frequency ultrasonic cavitation irrigation of the nasal cavity (frequency – 29 kHz, amplitude – 25 µm), and the control group (23 patients) received a course of standard therapy in accordance with clinical recommendations. The treatment resulted in improvement of clinical symptoms and reduced disease recurrence rates in 31 (91,2%) patients in the main group and 16 (69,4%) patients in the control group. A satisfactory result was obtained in 3 (8,8%) patients in the main group and 6 (27,6%) in the control group. An unsatisfactory result was noted in one case in the control group.

The aim of the pilot study of a group of adolescents with H. pylori infection was to study the preliminary data obtained on the rs602662 locus of the FUT2 gene and to establish its role in the realization of clinical manifestations of chronic gastritis, gastric ulcer and duodenal ulcer associated with H. pylori.Methods: The study included 91 patients. The study for the presence of the polymorphic locus rs602662 of the FUT2 gene was carried out by the standard TaqMan PCR method on a Real-Time CFX96 Touch amplifier. The duration of the study was 6 months.Results: The main group included 25 adolescents aged 16 to 17 years 11 months, the control group included 20 patients. Patients infected with H. pylori more often noticed symptoms of dyspepsia - in 36%, compared with the control group - 9.7%. The presence of a family history in the main group for associated diseases had a significant difference, χ2 = 4.97, p <0.05.To assess the contribution of the genotype of the rs602662 locus of the FUT2 gene to the risk of clinical manifestations in H. pylori infection, the main group was divided into subgroups. In the distribution of alleles in these groups, statistically significant differences were revealed.Allele “A” has a protective effect against the onset of clinical symptoms of dyspepsia. The odds ratio (OR) with the carriage of allele “A” (genotypes A / A and G / A versus G / G) to have clinical symptoms with a positive H. pylori status was 0.175 (CI = [0.049-0.625] chi2 = 7.79 p = 0.0053).Conclusion. As a result of the study, we were unable to identify a significant association of alleles and genotypes of the rs602662 locus of the FUT2 gene with clinical manifestations of H. pylori infection. At the same time, carriers of the A allele have a pronounced association with the absence of clinical symptoms in patients with a positive H. pylori infection status of 0.175 (C.I. = [0.049-0.625] chi2 = 7.79 p = 0.0053).

Objective. The purpose the of the study is to improve the fluid therapy regimens for burn sepsis patient. Materials and methods. The study is based on the observation 52 patients with burn sepsis aged 23 to 65 years, with a total body surface area more than 35 % at the Kyiv City Clinical Hospital № 2 during 2016-2019. Patients were divided into two groups: the main group – 30 patients were treated with crystalloids, plasma, albumin and Rheosorbilact (RSL); control group – 16 patients received treatment without using of RSL. Clinical and biochemical parameters of endogenous intoxication (tissue destruction products, medium weight molecules (MWM), concentrations of lipid oxidation products (LOP) – by diene conjugate content and oxidatively modified proteins (OMP) – by carbonyl groups) were studied. Results and discussion. Syndrome of endogenous intoxication was an increase in burn sepsis patients, which correlated with an increase in the number of LOP, OMP, MWM, namely an increase in the hydrophobic (most toxic) fraction of MWM, which is 2-3 times higher than in healthy, sick and both groups. MWM and hydrophobic fraction remained high during the observation period in patients of the comparison group. MWM decreased by 1.35 times in patients of the main group relative to the group of crystalloids. The level of carbonyl groups of serum OMP in patients of the comparison group was 50 % higher than in the main group. The indicators of LOP exceeded 1.45 times in the comparison group, which testified to the continuation of oxidative stress and the continuation of systemic inflammation. Acute respiratory distress syndrome (ARDS) was characterized by the appearance of bilateral infiltrates in the lungs, according to radiography. This common organ dysfunction was detected in 75 % of patients in the comparison group and in 63 % of patients in the RSL-group. The duration of mechanical ventilation in the main group decreased by 1,6 days. Conclusions. The use of Rheosorbylact for patients with burn sepsis at a dose of 15 ml/kg has led to a decrease in the manifestations of intoxication syndrome, reduction of MWM, LOP, OMP, clinical manifestations of systemic inflammatory response, septic complications by 1.3 times, septic shock from 16 to 12 %, and the incidence of ARDS from 75 to 63 %.

Introduction. The vast majority of recommendations for conservative treatment of patients with chronic hemorrhoids contain recommendations for the use of systemic drugs with venotonic and angioprotective properties and the use of topical drugs to treat and prevent exacerbation of the disease. The action of the main components of the topical drugs used is aimed at controlling local inflammation, implemented through various mechanisms. The choice of a drug is mainly subjective.Aim of the study. Comparative clinical evaluation of the effectiveness of topical drugs for the treatment of chronic hemorrhoids.Materials and methods. There are two treatment schemes that are distinguished by the type of the drug that is used. The main group includes patients in treatment of whom a combined preparation based on lidocaine and fluocortolone in the form of rectal suppositories manufactured by Bayer (221 (50.6%) patients) was used. In the control group – a combined preparation on the basis of tribenoside and lidocaine in the form of rectal suppositories by Recordati company (216 (49,4%) people). The rate of suppositories administration and duration of their use was 21 days. Clinical efficacy was estimated by the rate of regression of the main clinical manifestations of the disease: bleeding, pain syndrome, discomfort.Results and discussion. By the 14th day of treatment with Bayer preparation, bleeding as the main sign of disease exacerbation persisted in 15.2% (5) of the main patients against 26.7% (8) of the control group patients. Positive effect was obtained in the majority of patients with the initial prevalence of pain syndrome. On the 14th day the pain remained in 17.0% (8) patients of the main group against 27.9% (12) of the control group. Sense of discomfort was preserved in 10% (4) of the main group patients against 15.8% (6) of the control group patients. Regression of combined clinical manifestations of the disease in the comparison groups showed the fastest elimination of inflammation in the main group in 79.4% of patients against 73.0% in the control group on the 14th day of treatment.Conclusions. Thus, the use of Bayer preparation allowed ensuring the regression of the main clinical manifestations of the disease in a shorter time. This drug can be recommended for use in complex treatment of chronic hemorrhoids in everyday clinical practice.

e17514 Background: Malignant pelvic tumors account for more than 25% of cancer incidence in Russia. Radiation therapy is the most common treatment for such patients; however, 10-15% of patients develop radiation-induced complications of the pelvic organs, and more effective treatments are required to manage these complications. Methods: The study included 30 patients with cervical cancer T3NхM0 after combination treatment. 7-10 months after combined radiation treatment (total radiation dose to the primary focus 80 Gy), patients developed erosive ulcerative radiation rectitis (RTOG grade 1 and 2). Patients were divided into 2 groups: main group (n = 15) – conservative treatment combined with LILEDR. Each course included 10 LILEDR sessions, the red spectrum λ = 640 nm on the cubital vein projection (exposure time 5 minutes, dose 6.86 J/cm2) and locally on the ulcerated zones (exposure time 3 minutes, dose 3.96 J/cm2). Patients received 2 LILEDR courses with a 1-month interval. The control group received only conservative therapy. Results: Main clinical manifestations of rectitis (tenesmus, bloody mucous discharge) disappeared in the main group already on the 3-4th day of the first course, epithelialization of ulcerative defects occurred in a shorter period of 7-10 days. Soft superficial scars not causing rectal stenosis formed at the site of the ulcer by the end of LILEDR courses. The control group showed long periods of the ulcer epithelialization up to 30 days, late remission and a lingering recurrent character of the disease. Conclusions: LILEDR in combination with the main conservative therapy allows rapid managing with the clinical symptoms of radiation rectitis and regression of disorders developed after the complex treatment, which improves the quality of life of patients and shortens the rehabilitation period.

Aim. Comparative analysis of the changes in clinical manifestations of organic mental disorders in persons who have repeatedly committed socially dangerous acts, and in patients without recurrence of misconduct.Methods. 93 patients with organic mental disorders who have committed socially dangerous acts and were declared as mentally insane and in whom compulsory medical measures assigned to them by the court were used were examined. Patients who have committed repeated socially dangerous acts after treatment were included in the main group (30 persons), patients who does not commit repeated socially dangerous acts were allocated to the comparison group (63 persons).Results. In the main group, patients with organic delusional disorder (F06.2, 20% vs 11.1%) dominated. In the control group, the share of patients with dementia of mixed origin (F02) was 22.2%, and in the main group - only 3.3%. In the main group, the prevalence of patients with psychotic conditions within the organic mental disorder was 43.3% versus 30.2% in the comparison group. At that, the rate of progression varied only slightly. The proportion of patients with no previously recorded episodes of decompensation was higher in the control group - 46.0% versus 13.3% in the study group. A few patients in both groups (10%) were supervised and regularly visited a psychiatrist. The number of patients who were not supervised or who attended psychiatrist rare was almost the same (46.7 and 47.6% in the study group and control group, respectively). At that, in the control group 12.7% of patients were not supervised due to the short disease duration (patients with dementia). There were a few more patients with a disease duration of more than 10 years in the comparison group - 54.0%, whereas in the study group - 43.3%.Conclusion. There is a number of differences in clinical manifestations of organic mental disorder and their changes over time, affecting the criminogenic risk of this population; an integrated approach to disease prevention, which can lead to an improvement in the criminal forecast, is needed.

The aim of the study was to examine an association of the rs602662 FUT2 genetic locus with the status of H. pylori infection and development of related diseases (chronic gastritis, gastric ulcer and 12 duodenal ulcer).Methods: The study included 91 patients, divided into two groups - "case" and "control". The aim: Evaluation of the effectiveness of the biotherapeutic agent Saccharomyces boulardii in the treatment complex of patients with clinical manifestations of dysbiosis and/or signs of enteric insufficiency refractory to previous therapy, to develop ways for their medical correction. Materials and methods: The study included 209 patients, treated during 2018-2020, grouped into main and the comparison group. The main group was divided into subgroups depending on the nosological form of the underlying disease: surgical – 36, general therapeutic – 58, gynecological – 47 patients. Main group treatment was supplemented with Saccharomyces boulardii for 10-14 days from the first day of inclusion of patients in the observation group. Results: The analysis of motor-evacuation disorders verified flatulence in 89.47%, localized epigastric pain in 22.48%, diarrhea in 55.02%, and constipation in 11.48% of cases. Based on the results of laboratory parameters, an absolute increase in the number of leukocytes in the range from 12.4 ± 1.8 x 109 / l to 14.7 ± 2.8 x 109 / l with a neutrophilic formula shift, bilirubinemia to 54.4 ± 12.2 mmol / l , elevated levels of urea to 14.7 ± 3.9 and creatinine to 0.199 ± 0.07 mmol / L. Conclusions: After completion of the treatment course (main group), a significant (p <0.05) improvement in the general condition (decrease in stool frequency within 1–4 times (2.3 ± 0.28) per day, the absence of rumbling, flatulence and pain in abdomen) in 131 (92.9%) of 141 patients included in the study was stated. Clinical improvement was confirmed by bacteriological studies of copro-culture.
