Dentoalveolar Growth Inhibition Induced by Bone Denudation on Palates: A Study of Two Isolated Cleft Palates with Asymmetric Scar Tissue Distribution

Objective: This report presents two cases of isolated cleft palate with asymmetric distribution of postsurgical scar tissue determined by laser Doppler flowmetry. To determine the effect of mucoperiosteal denudation of the bone on maxillary alveolar growth, the analysis of dentoalveolar structures compared the affected side to the unaffected side of each case. Method: Two Japanese girls with isolated cleft palates were examined. Both subjects had undergone pushback operations (a modified version of the procedure of Wardill) for palatal repair at 18 months of age. Palatal blood flow was examined by laser Doppler flowmetry when the girls were 12 years old to determine the extent of postsurgical scar tissue over the denuded bone. To analyze the maxillary dentoalveolar structures three dimensionally, the whole surface of the upper dental cast was measured and recorded by an optical measuring device when the girls were 7 years old. Results and Conclusions: Analysis via flowmetry showed that the palatal scar tissue area was limited to the anterior tooth region on the right (unaffected) side but extended posteriorly to the premolar region on the left (affected) side in both subjects. The two girls had similar dentoalveolar structures, with the dental and alveolar arches deflected lingually at the deciduous molar area on the affected side. There were no differences in the buccolingual inclination of deciduous molars or in the vertical growth of the alveolar processes between the affected and unaffected sides. In both girls, bone denudation in the premolar region appeared to result in less than 3 mm of displacement of the teeth palatally, with no change in lingual inclination. Any effects of scar tissue on the vertical development of the alveolus were not substantiated.

Serotonin2 (5-HT2) receptor antagonists (ketanserin, ritanserin) can normalize a hyperoxemia-induced disturbance in skeletal muscle oxygenation, presumably by local microflow changes. The purpose of this study was to develop equipment for local hydrogen clearance measurements with a modified eight-channel platinum electrode to assess changes in local skeletal muscle capillary blood flow induced by hyperoxemia and ritanserin (0.035 mg/kg) during hyperoxemia. Laser-Doppler flowmetry was used for regional microflow measurements. Two groups of six anesthetized and artificially ventilated rabbits were studied: group I with normoxemia and hyperoxemia [arterial PO2 (PaO2) 48 kPa; 360 Torr] and group II before and after ritanserin with hyperoxemia (PaO2 46 kPa; 345 Torr). In group I, hyperoxemia induced a mean local hydrogen clearance decrease of 22% while laser-Doppler flowmetry signal decreased 31%. In group II, ritanserin induced a 125% mean local hydrogen clearance increase compared with hyperoxemia (or 37% compared with group I normoxemia); laser-Doppler flowmetry signal increased 30%. The sum distribution of local hydrogen clearances shifted to the left during hyperoxemia and to the right after ritanserin. The conclusion from this study is that local and regional microflow changes can explain the effects of hyperoxemia and ritanserin on skeletal muscle oxygenation.

OBJECTIVE. We wanted to relate the perfusion index (M) in burn wound in the period of treatment with the development of the type of pathological scar tissues of different origin during the convalescence period. METHODS AND MATERIALS. We studied burn wounds and scars in 25 patients with thermal trauma in the postoperative period and the convalescence period during the year. We determined the values of perfusion of burn wounds with help of laser Doppler flowmetry, which were diagnostically significant for predicting the outcome of treatment of burn wounds. RESULTS. If the perfusion index was lower than 4.0 PU and higher than 10 PU, then a high risk of developing the pathological type of scar tissue was predicted. If the perfusion index was between 4.0 and 10 PU, then a minimal risk of developing the pathological type of scar tissue was predicted. CONCLUSION. The perfusion index is an important criterion for choosing early preventive measurements and specialized treatment of burn wounds to reduce probable developing the pathological scars and, as a result, obtaining good cosmetic and functional effects. Based on the results of the study, we developed the diagnostic algorithm for predicting the development of various types of scar tissue. In this study we demonstrate what kind of relative alterations can be expected in average perfusion and blood flow oscillations during postural changes being measured in the skin of limbs and on the brow of the forehead by wearable laser Doppler flowmetry (LDF) sensors. The aims of the study were to evaluate the dynamics of cutaneous blood perfusion and the regulatory mechanisms of blood microcirculation in the areas of interest, and evaluate the possible significance of those effects for the diagnostics based on blood perfusion monitoring. The study involved 10 conditionally healthy volunteers (44 ± 12 years). Wearable laser Doppler flowmetry monitors were fixed at six points on the body: two devices were fixed on the forehead, on the brow; two were on the distal thirds of the right and left forearms; and two were on the distal thirds of the right and left lower legs. The protocol was used to record three body positions on the tilt table for orthostatic test for each volunteer in the following sequence: (a) supine body position; (b) upright body position (+75°); (c) tilted with the feet elevated above the head and the inclination of body axis of 15° (−15°, Trendelenburg position). Skin blood perfusion was recorded for 10 min in each body position, followed by the amplitude–frequency analysis of the registered signals using wavelet decomposition. The measurements were supplemented with the blood pressure and heart rate for every body position analysed. The results identified a statistically significant transformation in microcirculation parameters of the average level of skin blood perfusion and oscillations of amplitudes of neurogenic, myogenic and cardiac sensors caused by the postural changes. In paper, we present the analysis of microcirculation in the skin of the forehead, which for the first time was carried out in various positions of the body. The area is supplied by the internal carotid artery system and can be of particular interest for evaluation of the sufficiency of blood supply for the brain.

Background: Prostanoids are used in the treatment of Raynaud’s phenomenon and acral perfusion disorders secondary to collagenosis. In subjective terms, intravenous administration of these agents produces success in more than 50% of patients. The therapeutic outcome of clinical administration of alprostadil or iloprost may vary from individual to individual. Patients and methods: The following variables were analysed in a cross-over study in 27 patients with collagenosis and Raynaud’s phenomenon: plasma viscosity and erythrocyte aggregation (rheological variables), partial pressure of oxygen and laser Doppler flowmetry in the finger region, and lymphocyte phenotyping and interleukin (IL) determinations (immunological variables). Results: Laser Doppler flowmetry revealed significant differences between patients with secondary Raynaud’s phenomenon and a control group of 25 healthy subjects. Laser Doppler readings did not change significantly as a result of the treatments. Therapy with iloprost produced a reduction in IL-1beta, L-selectin (CD 62 L) and IL-6. Conclusion: The change in immunological variables due to iloprost may explain the long-term effects of prostaglandins in the treatment of Raynaud’s phenomenon. From our results it is not possible to infer any preference for iloprost or alprostadil.

Relevance. Morpho-functional changes in peripheral circulation established in type 1 diabetes mellitus correlate with changes in central hemodynamics, allowing the use of microcirculation indicators as diagnostic and prognostic criteria for assessing the degree of functional vascular disorders. Identifcation of microcirculation features of the blood by the method of laser Doppler flowmetry in children with different experience of type 1 diabetes in key age categories.Materials and methods. The study included 67 children with type 1 diabetes mellitus aged 12-15 years with an experience of the disease from six months to ten years. The comparison group consisted of 38 healthy children. The state of the microvasculature was assessed by laser Doppler flowmetry using a laser analyzer for capillary blood flow LAKK-OP.Results. In children with an experience of type 1 diabetes of less than two years, microcirculation disorders in periodontal tissues correspond to the hyperemic form, accompanied by increased perfusion, a decrease in the amplitude of low-frequency oscillations, increased heart rate, high blood flling, and blood flow bypass. For children with an endocrinopathy experience of more than three years, microcirculation disorders correspond to a stagnant form, combined with a decrease in perfusion due to stagnation of blood in the venular link, endothelial domination with suppression of neurogenic and cardiac fluctuations, low efciency and redistribution of blood flow in favor of the nutritive link.Conclusions. With the increase in experience, the degree of compensation of type 1 diabetes, the progression of diabetic microangiopathy, it is advisable to designate two stages of development of microcirculatory disorders. Early – compensatory with active adaptation, including neurogenic and endothelial regulation mechanisms. Late – decompensation with passive adaptation, supporting the effectiveness of microcirculation due to myogenic control of regulation, shunting and increasing the rate of blood outflow.
