Quality healthcare delivery is the bedrock to exponentially accelerate the development of a country. Unfortunately, in Pakistan healthcare has been neglected since a long time, with the common man bearing the brunt of this acute situation. There are critical challenges in health care, with paucity of trained human resource and deficit of regulated infrastructure and service delivery being the predominant dilemmas. Primary and secondary healthcare are in an unseemly state, to say the least. Maternal and child health care, accident, and emergency departments and mental health are among the most undermined and forsaken areas of healthcare, primarily in the far flung Gilgit Baltistan region of Pakistan. The only way forward is if the political regime, administration and the medical personnel work in concurrence to revise the health infrastructure of the country.

AbstractMountains regions like Gilgit-Baltistan (GB) province of Pakistan are solely dependent on seasonal snow and glacier melt. In Indus basin which forms in GB, there is a need to manage water in a sustainable way for the livelihood and economic activities of the downstream population. It is important to monitor water resources that include glaciers, snow-covered area, lakes, etc., besides traditional hydrological (point-based measurements by using the gauging station) and remote sensing-based studies (traditional satellite-based observations provide terrestrial water storage (TWS) change within few centimeters from the earth’s surface); the TWS anomalies (TWSA) for the GB region are not investigated. In this study, the TWSA in GB region is considered for the period of 13 years (from January 2003 to December 2016). Gravity Recovery and Climate Experiment (GRACE) level 2 monthly data from three processing centers, namely Centre for Space Research (CSR), German Research Center for Geosciences (GFZ), and Jet Propulsion Laboratory (JPL), System Global Land Data Assimilation System (GLDAS)-driven Noah model, and in situ precipitation data from weather stations, were used for the study investigation. GRACE can help to forecast the possible trends of increasing or decreasing TWS with high accuracy as compared to the past studies, which do not use satellite gravity data. Our results indicate that TWS shows a decreasing trend estimated by GRACE (CSR, GFZ, and JPL) and GLDAS-Noah model, but the trend is not significant statistically. The annual amplitude of GLDAS-Noah is greater than GRACE signal. Mean monthly analysis of TWSA indicates that TWS reaches its maximum in April, while it reaches its minimum in October. Furthermore, Spearman’s rank correlation is determined between GRACE estimated TWS with precipitation, soil moisture (SM) and snow water equivalent (SWE). We also assess the factors, SM and SWE which are the most efficient parameters producing GRACE TWS signal in the study area. In future, our results with the support of more in situ data can be helpful for conservation of natural resources and to manage flood hazards, droughts, and water distribution for the mountain regions.

AbstractIodine and selenium deficiencies are common worldwide. We assessed the iodine and selenium status of Gilgit-Baltistan, Pakistan. We determined the elemental composition (ICP-MS) of locally grown crops (n = 281), drinking water (n = 82), urine (n = 451) and salt (n = 76), correcting urinary analytes for hydration (creatinine, specific gravity). We estimated dietary iodine, selenium and salt intake. Median iodine and selenium concentrations were 11.5 (IQR 6.01, 23.2) and 8.81 (IQR 4.03, 27.6) µg/kg in crops and 0.24 (IQR 0.12, 0.72) and 0.27 (IQR 0.11, 0.46) µg/L in water, respectively. Median iodised salt iodine was 4.16 (IQR 2.99, 10.8) mg/kg. Population mean salt intake was 13.0 g/day. Population median urinary iodine (uncorrected 78 µg/L, specific gravity-corrected 83 µg/L) was below WHO guidelines; creatinine-corrected median was 114 µg/L but was unreliable. Daily selenium intake (from urinary selenium concentration) was below the EAR in the majority (46–90%) of individuals. Iodine and selenium concentrations in all crops were low, but no health-related environmental standards exist. Iodine concentration in iodised salt was below WHO-recommended minimum. Estimated population average salt intake was above WHO-recommended daily intake. Locally available food and drinking water together provide an estimated 49% and 72% of EAR for iodine (95 µg/day) and selenium (45 µg/day), respectively. Low environmental and dietary iodine and selenium place Gilgit-Baltistan residents at risk of iodine deficiency disorders despite using iodised salt. Specific gravity correction of urine analysis for hydration is more consistent than using creatinine. Health-relevant environmental standards for iodine and selenium are needed.

The genus Pholcus Walckenaer, 1805 is reported from Pakistan for the first time. Two new species of the Pholcus nenjukovi species-group are described: Pholcus hamuchal Yao & Li sp. nov. (Gilgit Baltistan, male and female) and Pholcus kalam Yao & Li sp. nov. (Khyber Pakhtunkhwa, male and female). Type material is deposited in the Institute of Zoology, Chinese Academy of Sciences (IZCAS) in Beijing, China.
