Primary rectal melanoma is a rare malignant tumor with an aggressive course and poor prognosis. The article describes a clinical observation demonstrating the successful comprehensive treatment of primary rectal melanoma. A patient with a complicated course of rectal melanoma, stage IV of the first stage was performed by surgical treatment with removal of the external intestine according to the Hartmann type with removal of the uterine appendages from both sides of the en-block, and resection of the left pelvic plexus. In the postoperative period, the patient underwent chemoembolization of the liver with combined immunotherapy. The patient is alive two years after the operation; there is no evidence for local recurrence and progression of the disease.

It is possible to find some reports on isolated thickening of the internal anal sphincter and the causes of its development in the literature. However, no clear diagnostic program and surgical correction description has been detected for this condition. This clinical observation demonstrates a rare case of the hyperplasia of the internal sphincter syndrome with the obliteration of the anal canal, and also successful rehabilitation of the patient during the multi-stage surgical treatment.

Relevance: Esophageal cancer ranks eighth in the structure of malignant tumors in the Republic of Kazakhstan. According to the Kazakh Institute of Oncology and Radiology, in 2018, 1225 were primarily registered with esophageal cancer, and 825 patients died from this disease. The purpose of this study was to analyze the esophageal cancer incidence and mortality among the population of the Republic of Kazakhstan in 2009-2018 and determine the cancer service efficacy. Results: The analysis of intensive indicators of esophageal cancer incidence showed a decrease in the number of primary cases from 8.2‰ in 2009 to 6.7‰ in 2018, and the mortality – from 6.9‰ to 3.8‰. The share of early detection (Stage I-II) has increased by 38.2% since 2009 to reach 52.1% in 2018. The share of stage IV esophageal cancer has decreased from 7.2% in 2009 to 5.8% in 2018. One-year mortality has decreased by 14.7% since 2009 to reach 41.2% in 2018, which speaks of the improvements in the timely diagnosis of esophageal cancer. The number of patients who received radical comprehensive treatment has increased by 8.6% to reach 32.2% in 2018. The number of patients who received surgical treatment in 2009-2018 remained almost at the same level and amounted to 18.9 and 18.0%, respectively. However, during the years of screening for esophageal cancer, the proportion of surgical treatment has increased from 26.6% to 30.4%. The ratio of incidence to mortality has decreased from 91.4% to 68.8% in the study period indicating an improvement in the level of cancer service in the Republic of Kazakhstan. Conclusion: The analysis of intensive indicators of esophageal cancer incidence showed a decrease in primary incidence. The screening conducted in 2013-2016 has shown an improvement in early detection of esophageal cancer (stages I-II) and a decrease in the share of the advanced stage. The findings of this study shall serve as the basis for planning anti-cancer activities

The case report presents a clinical observation of urethral erosion that developed after a long-term presence of a urethral catheter in a patient after implantation of an artificial bladder sphincter. The stages of the patients surgical treatment are described. The frequency of such complications in patients with an established artificial bladder sphincter according to literature data is presented.

Angiosarcoma of the spleen is a rare malignancy that arises from vascular endothelial origin. This neoplasm is highly malignant and diagnosis is often delayed due to the vague presentation of clinical symptoms. A case report and concise review of the current diagnostic criteria and surgical treatment are provided to aid in the detection and treatment of this malignancy. We present a case of a 56-year-old female who presented with massive splenomegaly secondary to angiosarcoma of the spleen. The patient suffered from longstanding symptomatic anemia and thrombocytopenia. Diagnosis of a splenic angiosarcoma can be difficult due to the vague presentation and lack of concrete risk factors. Early identification and splenectomy are paramount. However, it is an aggressive malignancy with poor prognosis. We reviewed the literature of the current diagnostic and surgical treatment of primary splenic angiosarcoma.

Cavernous hemangiomas are benign tumours of mesodermal origin. Even though various localizations of hemangioma have been described in the literature, its occurrence in the greater omentum is very rare. Only symptomatic hemangiomas are indicated for surgical treatment. There are case reports presenting resection or surgical removal of the greater omentum with hemangioma because of mechanical syndrome, consumption coagulopathy, bleeding, infection or suspicion of a malignancy. This article presents a case report of a patient operated on for a suspicion of carcinomatosis of the greater omentum. Histological examination found hemangiomatosis in the resected greater omentum.
