<p><strong>Background:</strong> Gut flora have important trophic effects on intestinal epithelia and on immune structure and fonction.They also protect colonized host against invasion by alien microbes.Recent research suggests that an imbalance of the intestinal microbiota and a dysfunctional intestinal barrier might trigger irritable bowel syndrome (lBS). As probiotics have been reported to restore the intestinal microbiota and the gut barrier, the therapeutic potential of probiotics within IBS became of strong interest. <strong></strong></p><p><strong>Objectives: </strong>To assess the efficacy of probiotics in lBS. <strong></strong></p><p><strong>Methods: </strong>Patients of 15 to 60 years old and both sexes were included f om the out patient department (OPD) of gastroenterology, Bangabandhu Sheikh Mujib Medi­cal University (BSMMU). A validated lBS-QOL instmment consisted of 34 questions used to assess improvement of quality of Iife before and after treatment. A total of 65 diarrhoea predominant LBS patients were randomised to receive either probiotics(n-33) or placebo(n-32) twice daily frir 6 weeks.</p><p><strong>Results:</strong> At the end of 6 weeks therapy, improvement in various symptoms(abclominal pain, stool frequency, consistency and 11atulence) in probiotics group was statistically significant. Mean QOL score before treatment was 103 in probiotics group and I 06 in placebo group. After 6 weeks of treatment mean QOL score was 82 in probiotics group and I 02.58 in placebo group. No side effects of the therapeutic agents were observed in any patient during the trial. <strong></strong></p><p><strong>Conclusions:</strong> Probiotics effectively alleviates global IBS and improves TBS symptoms simultaneously with an improvement of quality of life.</p>

ABSTRACTRifaximin is one of the common drugs used in clinical practice in the management of traveler’s diarrhea, irritable bowel syndrome (IBS), and hepaticencephalopathy. Hyponatremia is one of the rare side effects of this drug. We hereby present the case of a middle-aged woman who was a known caseof IBS: Diarrhea predominant who developed symptoms of hyponatremia after a short duration of rifaximin treatment, no other cause of hyponatremiawas found on evaluation, so we suspect this as a rare side effect of rifaximin therapy.Keywords: Rifaximin, Hyponatremia, Irritable bowel syndrome.

Several research studies have revealed that migraine has a solid link with gastrointestinal diseases especially irritable bowel syndrome (IBS). This study was carried out to investigate therapeutic potential of diet based on IgG elimination combined with probiotics on migraine plus irritable bowel syndrome. A total of 60 patients diagnosed with migraine plus IBS were recruited for the study. IgG antibodies against 266 food varieties were detected by ELISA. Then, the subjects were randomized into three groups for treatment of IgG elimination diet or probiotics or diet combined with probiotics. Migraine symptom, gut function score, medication use, and serum serotonin level were measured at baseline, 7 weeks, and 14 weeks. Improvement of migraine and gut symptom was achieved at a certain time point. Reduced use of over-the-counter- (OTC−) analgesics was seen in all groups. However, use of triptans did not show significant difference. An increased serum serotonin level was seen in subjects treated with elimination diet and elimination diet combined with probiotics. IgG elimination diet combined with probiotics may be beneficial to migraine plus IBS. It may provide new insight by understanding the intricate relationship between migraine and gastrointestinal diseases.

An open multicentre study was conducted at 18 Canadian centres in patients diagnosed as suffering from irritable bowel syndrome ((BS). One hundred and thirty-one (72%, female and 28% male) patients, mean age 44 years, were treated with 200 mg tid trimebutine maleate (Modulon; Jouveinal) for periods of four weeks. Frequency and severity of pain, constipation and diarrhea were analyzed, as were stool frequency and consistency. A statistically significant reduction of frequency and severity was discerned in all symptoms (pain, constipation. diarrhea). Stool frequency foll within normal range (three per week to three per day) in close to 90% of patients as compared to 66% prior to the study. Similarly, stool consistency was normal in over 40% more patients at the end of the study than before treatment. Global evaluation by physicians and patients showed that approximately 70% of patients felt better at the end of the treatment than before the study. Side effects such as nausea, headaches and fatigue were reported in 14 patients.leading to discontinuation of therapy in seven. In conclusion , trimebutine maleate was effective in reducing the frequency and severity of the major symptoms of IBS as well as in normalizing transit disturbances as measured by stool frequency.
