Tooth retention is a fairly common orthodontic anomaly. Of all the retained teeth, canines are most commonly involved. There are several approaches to treating retention, including displacement and removal. When choosing treatment strategies, localization of the retained tooth is of great importance. In this report, we sought to present an example of the removal of one of such difficult-retained teeth.

The article provides a review of literature on the long-term consequences of Lyell's syndrome (one of the forms of toxic epidermal necrolysis). Among them: chronic eczema with xerosis and itching, skin depigmentation, hypertrophic and keloid scars, nail lesions, eyes, including dry eye syndrome and chronic conjunctivitis with/without obliteration of the nasolacrimal canal, kidney disease, lung disease, Sjogren's syndrome, Hashimoto's thyroiditis and others. Clinical case. The authors present their clinical case of an 11-year-old girl who suffered from Lyell's syndrome at the age of 8 and has been observed for the past three years, share their clinical and laboratory-instrumental observations and treatment strategies. In particular, they report about the revision of the clinical diagnosis with which the girl was admitted to the OHMADYT hospital, from BA to chronic secondary obstructive bronchitis, the detection of chronic conjunctivitis with obliteration of the nasolacrimal canals, chronic sinusitis, secondary immune deficiency, keloid skin scars. Conclusions. Despite constant rehabilitation therapy and a significant improvement in the general condition of the girl, it was not possible to fully restore the function of mucociliary transport, as a result ventilation of small bronchi gradually deteriorates, bronchiectasis is observed in the basal segments of the right lung, a polyserial strain S. aureus is formed, which is constantly sown from the child's sputum; it was not possible to fully restore the patency of the nasolacrimal canals and achieve a stable remission of chronic sinusitis. The research was carried out in accordance with the principles of the Helsinki Declaration. The informed consent of the patient was obtained for conducting the studies. This chapter presents general treatment strategies for carpal tunnel syndrome using a clinical case example. It discusses assessment and planning, diagnostic pearls, decision-making, surgical procedures (open and endoscopic carpal tunnel release), management pearls, aftercare, complications and their management, and evidence and outcomes. Physical exams, such as Phalen’s test or a Tinel sign over the median nerve, are introduced, whereas steps in the surgical procedure are shown with intraoperative photographs. The chapter provides information on modern practices for comprehensive management of carpal tunnel syndrome from start to finish.

Aims and MethodA medical second opinion is an expert clinical case evaluation requested by a colleague for a patient already under specialist care. There is no literature on the provision for second opinions in psychiatric practice other than those relating to the Mental Health Act 1983, and the availability of clinics offering second opinions varies greatly from one area to another. We undertook a survey of an expert second opinion service in Leeds over the period 1988–2000 and examined the case notes of 103 referred patients, of whom 71 were included in the analysis data.ResultsIn the majority of cases diagnoses were not changed, but alternative treatment strategies were suggested. The exception was for patients with a diagnosis of personality disorder, in whom neither a new diagnosis nor alternative management was commonly suggested. Half of the assessments were completed in a single visit, but a substantial minority required further evaluation as outpatients, close follow-up or even hospital admission for detailed review.Clinical ImplicationsWe believe that a second opinion service offers clinicians valuable support and expertise, especially in sector psychiatry. Evidence presented suggests that further therapeutic options are available for many patients regarded as ‘difficult to treat’. In some cases this may involve a period of observation free of all psychotropic medication. As in other specialities, psychiatrists should be aware of the value of further advice for patients with particularly refractory or unusual disorders.

ABSTRACT Aim This clinical case report demonstrates the successful management of a complex endodontic-periodontal iatrogenic lesion following a critical evaluation and decision-making process with interdisciplinary treatment strategies. Background Defective dental restorations can affect the periodontal and endodontic health of the teeth. Case report An endodontic-periodontal lesion that resulted from a faulty restoration in a 30-year-old male patient is reported. The management of the case is done through periodontal and endodontic treatment. The detailed method of managing of this type of cases is discussed. Conclusion In conclusion, the use of a systematic diagnosis process will help in the identification and treatment of iatrogenic endodontic-periodontal lesions. Exaggerated overbite can be corrected by several treatment modalities. As the primary objective is to correct the problem by addressing its root cause, treatment options are closely related to etiology. Therefore, the main treatment strategies are: the extrusion of posterior teeth, intrusion of teeth (maxillary/mandibular) or the combination of these. The purpose of this paper is to describe the fabrication of a modified thermoplastic plate (MTP) for correction of exaggerated overbite in mixed dentition and describe a clinical case treated with this device. MTP favored the extrusion of posterior teeth, in addition to being shown to be a device that was easy to manufacture and effective in the treatment of excessive overbite, enabling patients to return to normal condition during the development of occlusion.

Mesenchymal chondrosarcoma is a rare highly differentiated malignant tumor with a characteristic histological structure, which affects bones or soft tissues and has a high potential for local recurrence. The small number of observations of mesenchymal chondrosarcoma in the head and neck region causes the episodic nature of studies of this problem, and the treatment and diagnostic tactic remains controversial. The article presents the results of the most extensive and systematic studies, which show that the main method of treatment for this category of patients is surgical. Neoadjuvant or adjuvant chemotherapy tends to improve overall survival. Radiation therapy is used as adjuvant treatment. The work provides a clinical case of a long lifetime of a patient with local recurrence of the disease in the absence of regional, distant metastasis and the use of all current methods of treatment for cancer.

IntroductionClozapine, is widely prescribed for treatment of refractory schizophrenia, but its use may be limited by potentially serious adverse effects. The most feared complication remains agranulocytosis [absolute neutrophil count (ANC) < 500/mm3], which occurs in 1% of patients. Guidelines recommend immediate cessation until the granulocyte count normalizes, but little is known about the subsequent treatment and the possibility of restoring clozapine.ObjectivesTo know procedures that allow clozapine rechallenge after induced agranulocytosis in refractory schizophrenia.MethodsWe present a clinical case of agranulocytosis and evolution after simple reinstitution of clozapine.ResultsA 38-year-old woman diagnosed refractory schizophrenia. After 10 years with clozapine (300 mg/day), we find neutropenia (ANC 1420/mm3) in a monthly control blood count with progression to agranulocytosis (ANC 460/mm3) in the following month. We suspend clozapine and started olanzapine (20 mg/day) with restoration of haematological values in a period of one month. The patient had psychotic decompensation at two months after the change with lack of response to different psychopharmacological strategies for five months. According to the hematology department we decided to re-introduce clozapine (200 mg/day) in combination with olanzapine with complete clinical remission. Between the 3rd and 9th week after rechallenge we observe a progressive decline in ANC, while remaining within the range of normal. From the 9th week and in the last 6 months neutrophil counts remained stable.ConclusionsAlthough, more research is needed to establish the safety to rechallenge of clozapine after agranulocytosis, it must be an alternative to consider when other treatment strategies fail.Disclosure of interestThe authors have not supplied their declaration of competing interest.
