It would be wrong to expect stability in any large and complex industry today. The Australian telecommunications industry is no exception and in the lead-up to 2017 there have been a number of government reviews and inquiries announced that are certain to add to the instability if the outcomes do not focus on the long-term interests of end users. Whilst stability may not be achievable in an industry that is dependent on rapidly changing technology, there are aspects of telecommunications competition policy that are broken and need to be fixed urgently. Now is not the time to take an axe to the telecommunications competition legislation, especially when the underlying causes of the instability are not being tackled. Mr Graham Shepherd, a leading member of the Telecommunications Association Board and the Journal Board for many years, has retired from the Telecommunications Association Board.

In this paper we analyse the evolution of the Italian telecommunications market since the beginning of the liberalisation and privatisation process in Italy started in the mid-nineties.  The role of competition and regulatory authorities is also considered. We present a survey of the main regulatory interventions in the industry as well as the market structure and its dynamics in the period 2000-2015. We also provide some insights on the current state of the ultra-fast broadband access and the evolution of the so called “next generation networks”. In this regard, the recent Italian government’s plan regarding the deployment of the broadband services is also discussed. Our research can provide helpful information on telecommunications trends in Italy and would help to assess past as well as ongoing policies.

Background To date, several medication adherence apps have been developed. However, the existing apps have been developed without involving relevant stakeholders and were not subjected to mobile health app guidelines. In addition, the usability and utility of these apps have not been tested with end users. Objective This study aimed to describe the usability and utility testing of a newly developed medication adherence app—Med Assist—among ambulatory care patients in Malaysia. Methods The Med Assist app was developed based on the Theory of Planned Behavior and the Nielson usability model. Beta testing was conducted from March to May 2016 at a primary care clinic in Kuala Lumpur. Ambulatory care patients who scored ≥40% on the electronic health literacy scale, were aged ≥21 years, and were taking two or more long-term medications were recruited. Two rounds of in-depth interviews were conducted with each participant. The first interview, which was conducted upon participant recruitment, was to assess the usability of Med Assist. Participants were asked to download Med Assist on their phone and perform two tasks (register themselves on Med Assist and enter at least one medication). Participants were encouraged to “concurrently think aloud” when using Med Assist, while nonverbal cues were observed and recorded. The participants were then invited for a second interview (conducted ≥7 days after the first interview) to assess the utility of Med Assist after using the app for 1 week. This was done using “retrospective probing” based on a topic guide developed for utilities that could improve medication adherence. Results Usability and utility testing was performed for the Med Assist app (version P4). A total of 13 participants were recruited (6 men, 7 women) for beta testing. Three themes emerged from the usability testing, while three themes emerged from the utility testing. From the usability testing, participants found Med Assist easy to use and user friendly, as they were able to complete the tasks given to them. However, the details required when adding a new medication were found to be confusing despite displaying information in a hierarchical order. Participants who were caregivers as well as patients found the multiple-user support and pill buddy utility useful. This suggests that Med Assist may improve the medication adherence of patients on multiple long-term medications. Conclusions The usability and utility testing of Med Assist with end users made the app more patient centered in ambulatory care. From the usability testing, the overall design and layout of Med Assist were simple and user friendly enough for participants to navigate through the app and add a new medication. From the participants’ perspectives, Med Assist was a useful and reliable tool with the potential to improve medication adherence. In addition, utilities such as multiple user support and a medication refill reminder encouraged improved medication management.

The rapid development of the Internet has drastically changed the competitive environment and the profit model of global telecommunications market. Virtual value chain is the information-based projection of value chain in the virtual world. It transcends the original one by creating new value in the value-adding process. This article studies the changes in Guangdong Telecom Company Limited’s market strategy from the perspective of virtual value chain, and allows telecom operators to take it as a reference for market strategies’ formulation. This article uses literature research method and case research method to draw conclusions that virtual value chain has helped Guangdong Telecom Company Limited establish new customer relationships, and the significance of virtual value chain is surpassing traditional value chain continually. Therefore, proper use of Internet-related technologies will help the long-term development of telecommunications industry.

Abstract BackgroundAccording to sustainable development goals (SDGs), societies should have access to affordable, reliable, and sustainable energy. Deregulated electricity markets have been established to provide affordable electricity for end-users through advertising competition. Although these liberalized markets are expected to serve this purpose, they are far from perfect and are prone to threats, such as collusion. Tacit collusion is a condition, in which power generating companies (GenCos) disrupt the competition by exploiting their market power. MethodsIn this manuscript, a novel deep Q-network (DQN) model is developed, which GenCos can use to determine the bidding strategies to maximize average long-term payoffs using available information. In the presence of collusive equilibria, the results are compared with a conventional Q-learning model that solely relies on past outcomes. With that, this manuscript aims to investigate the impact of emerging DQN models on the establishment of collusive equilibrium in markets with repetitive interactions among players. Results and ConclusionsThe outcomes show that GenCos may be able to collude unintentionally while trying to ameliorate long-term profits. Collusive strategies can lead to exorbitant electric bills for end-users, which is one of the influential factors in energy poverty. Thus, policymakers and market designers should be vigilant regarding the combined effect of information disclosure and autonomous pricing, as new models exploit information more effectively.

Traffic chaos, stress, congestion, environmental pollution, as well as the social problems resulting from the uncoordinated shopping trips of private citizens and companies and online deliveries represent key problems of the modern cities. In our opinion, their solving requires an intelligent coordination of the end-consumer supply-chain-related actions and movements, which should be based on mutual visibility, self-organization, and cooperation of the involved actors. This chapter presents and comments from a sustainability perspective several IT concepts that can optimize the modern ECM (end-consumer movements) related logistics. They rely on the intelligent coordination of end-consumers demands (ranging from short-term to long-term), the resulting reduction of supply-chain-related traffic, and the networking of social resources involved in such city supply chains. The focus is placed on the creation of multidimensional synergies among the involved actors on all scales and the support of the participative supply-chain networks, which are driven by end-users.

Delirium is a common, complex neuropsychiatric syndrome that occurs in approximately one in five hospitalized patients. It is associated with a range of adverse healthcare outcomes that are independently predicted by the severity and duration of delirium. Typically 50% or more of cases of delirium are missed, misdiagnosed, or diagnosed late in everyday practice. However, routine systematic cognitive testing aligned to formal screening for delirium in high-risk cases can improve detection in everyday practice. The relationship between delirium and dementia is complex; dementia is a potent risk factor for delirium and 50% of delirium occurs in the context of a pre-existing dementia, but evidence also indicates that the occurrence of delirium can accelerate the course of dementia and that many cases of delirium are followed by long-term cognitive impairment. Delirium is highly preventable, with a third of cases avoidable by addressing a variety of patient, illness, and treatment factors. Treatment of incident delirium requires careful consideration of underlying causes, aggravating environmental factors, and prudent use of pharmacological strategies, with antipsychotic agents the preferred pharmacological intervention. Careful attention to post-delirium care can minimize functional loss, address any psychological sequelae, and reduce the risk of further episodes.
