Background: Pregnancy loss in the first trimester is one of the most disappointing matters for a mother. But spontaneous pregnancy loss in the first trimester is the most common negative outcome of pregnancy. It's estimated that about 10% of known pregnancies are lost in the first trimester whereas fewer than 4% of pregnancies miscarry in the second trimester. Aim of current study was to assess the effect of the maternal vitamin D level on the risk of spontaneous pregnancy loss in the first trimester.Methods: It was a case-control study conducted in the department of obstetrics and gynecology, Sir Salimullah medical college Mitford hospital, Dhaka, Bangladesh during the period of September 2018 to August 2019. A total of 100 patients were included in this study. Statistical analyses of the results were obtained by using window-based computer software devised with SPSS version 22.0.Results: In analyzing the association of serum vitamin D status with first-trimester pregnancy state it was observed that more than half (52.0%) patients had severe deficiency (<10 ng/ml) in the case group and 14 (28.0%) patients in the control group. In total 24 (48.0%) patients had deficiency (10-20 ng/ml) in case and 35 (70.0%) in control group. Only 1 (2.0%) patient had insufficiency (21-29 ng/ml) in control group. The difference was statistically significant (p<0.05) between the two groups.Conclusions: Maternal serum vitamin D deficiency was significantly associated with early spontaneous pregnancy loss in the first trimester.

Objectives: To analyse the levels of serum 25-hydroxyvitamin D 25(OH) Dand calcium concentrations in patients with pre-eclampsia, Hyderabad, Sindh, Pakistan. Methodology: Cross sectional study was conducted at the Gynaecology and Obstetrics Department, LUHS, Hyderabad during the period of January 2020 to June 2020. Total 150 pregnant women were selected according to predesign Proforma and divided into two groups. 50 females were normal normotensive pregnant women as a control group, 100 were pre-eclamptic patients as case group. Obstetric history, clinical data was gathered and then 5ml of blood sample was collected from each patient. The blood samples were taken for the analysis of 25-hydroxyvitamin D or 25(OH)D and calcium levels. 25(OH)D were performed on chemiluminescence, While calcium concentration of Microlab 300. Data was analysed by using SPSS version 23. Student t-test was used for analysed of continuous variables. Results: The maternal serum vitamin D and calcium levels were significantly decreased as compared to patient with preeclampsia and normotensive healthy pregnant ladies. The blood pressure also declined in case group with P<0.001.Multinomial logistic regression showed significant odd ratio of Vitamin-D and Calcium deficiency in preeclampsia. Conclusion: The present study reveals that 25-hydroxyvitamin D, and Calcium levels declined significantly in preeclamptic women, and mighty be biomarker of this life threating disease.

Background: This study was done to evaluate the association of serum vitamin D level with GDM (n=30) and without GDM (n=30). The age and body mass index of the participants along with their gestational age, gravidity and parity were harmonized. The serum vitamin D levels and blood glucose were investigated. The results revealed that, normal pregnant women had significantly higher vitamin D level than their GDM counterparts.Methods: This case-control study was conducted on healthy pregnant women attending routine antenatal care at Bangabandhu Sheikh Mujib medical university from January 2019 to December 2019 recruited at third trimester of gestation. We measured maternal serum vitamin D status (25[OH]D) in third trimester of pregnancy. GDM was diagnosed according to the American diabetes association. guidelines. Vitamin D status was defined as, vitamin D sufficiency (≥30 or ≥75 nmol/L), insufficiency (20-30 and 50-75 nmol/L), were used to categories participants according to their 25[OH]D concentrations. We calculated adjusted odds ratios and 95% confidence intervals (CIs) using logistic regression.Results: The mean serum vitamin D level was lower in case group case 23.4 (17.4±35.1) compared to that in control group 29.7 (15.4±39.8) and the difference between the 2 groups was statistically significant (p≤0.001). Sufficient level of vitamin D was more in control group (66.7%) than that of case group (26.1%). Insufficient level of vitamin D was higher in case (73.3%) group than that of control (33.3%) group. These findings were significant (p=0.021). Respondents with insufficient level of vitamin D have 3.1 times more chance to develop GDM.Conclusions: Serum vitamin D level is reduced in pregnant women having GDM.

Abstract Background This study aimed to investigate the relationship between maternal serum vitamin D status in the first trimester of pregnancy and maternal as well as neonatal outcomes, considered the prevalence of vitamin D deficiency (serum 25(OH)D < 50 nmol/L) around the world, especially in the pregnant women. Methods From January 2015 to December 2016, in this cross-sectional retrospective study, we enrolled women receiving regular prenatal examinations and giving birth in the International Peace Maternity and Child Health Hospital. Cases confirmed as multiple pregnancy, incomplete medical records, and vitamin D level recorded after 13 weeks of gestation were excluded. A total of 23,394 mother-infant pairs were included ultimately. Obstetric and neonatal information were extracted from the database. Maternal serum vitamin D concentration was measured by chemiluminescence microparticle immunoassay. Logistic regression analysis (unadjusted and adjusted models) was used to analyze the association between vitamin D and maternal and neonatal outcomes. Results The average 25(OH) D concentration was 43.20 ± 0.10 nmol/L; 67.09% of patients were vitamin D deficient(25(OH) D < 50.00 nmol/L), 29.84% were vitamin D insufficient (50 nmol/L ≤ 25(OH)D < 75 nmol/L), 3.07% were sufficient (25(OH)D ≥ 75 nmol/L). The maternal 25(OH)D levels varied with age, pre-pregnancy BMI, season when blood sample was collected, number of previous-pregnancy. Notably, newborns delivered by women with deficient vitamin D status had a higher incidence rate of admission to NICU (Deficiency: 12.20% vs Insufficiency: 10.90% vs Sufficiency: 11.70%, Pbonferroni = .002) and a longer stay (deficiency: 6.2 ± 4.1 days vs insufficiency: 5.9 ± 3.1 days vs sufficiency: 5.1 ± 2.1 days, Pbonferroni = .010). Moreover, maternal vitamin D deficiency was a dependent risk factor for admission to NICU (unadjusted OR = 1.35, 95% CI,1.05–1.74 Pbonferroni = .022; adjusted OR = 1.31, 95% CI,1.010–1.687 Pbonferroni = .042). Conclusions Maternal vitamin D deficiency (25(OH) D < 50 nmol/L) was prevalent in eastern coastal China. The incidence rate of GDM as well as preeclampsia was higher in vitamin D insufficient group while vitamin D deficiency group was liable to intrauterine infection when compared with the other two groups. Most importantly, low vitamin D status in the first trimester of pregnancy was a dependent risk factor for admission to NICU. More well-designed perspective researches are necessary to clarify the role of vitamin D in the early stage of pregnancy.

Objective: This study was aimed to estimate the Vitamin D serum concentration in a cohort of women with a history of recurrent spontaneous abortion.Methods: The current cross-sectional study was conducted at Al-Diwaniya maternity and child teaching hospital, Al-Diwaniya province, Iraq. Data collection, lab work, and procedures related to the study extended from August 2016 to August 2017. The study enrolled 42 women of childbearing age who have a history of spontaneous early pregnancy loss. From each woman, a venous sample was obtained and sent to a lab to assess serum Vitamin D concentrations using standard routine biochemical methods.Results: Assessment of mean maternal serum Vitamin D concentration revealed that the mean was 21.48±11.82 (ng/ml) and the range was from 5 to 50 (ng/ml). The number of women with low serum Vitamin D (<20 ng/ml) was 25 (approximately 60%), the results also revealed a highly negative correlation was obtained between number of abortions and maternal serum Vitamin D (r=−0.717, p<0.001) and that the value or R2 was 0.514 which indicated that the level of Vitamin D alone can act as an independent predictor to explain 51.4% of spontaneous abortions in the participating women.Conclusion: Vitamin D level is a strong predictor of pregnancy loss in early pregnancy, and that correction of Vitamin D status among Iraqi pregnant ladies may substantially reduce the frequency of spontaneous abortion.

Aging is accompanied by various functional modifications determined by their environment, lifestyle, nutrition, and genetics. Based on these factors, it is essential to verify the vitamin deficiency in the elderly population. Hypovitaminosis D is commonly present in human aging, increasing the chances of developing noncommunicable chronic diseases. The VDR gene TaqI polymorphism may modify the vitamin D metabolic pathway by altering the interaction between the vitamin D receptor and the active circulating vitamin D. Therefore, this study aimed to investigate the association between serum vitamin D and biochemical and genetic factors, considering the TaqI polymorphism of the VDR gene, in an elderly population of the Federal District. The study was a descriptive, case-control, quantitative, and cross-sectional type and was conducted in two basic health units in the administrative region of Ceilândia, Federal District, DF, Brazil, with women aged 60 years or older. Anthropometric, biochemical, and genetic parameters (VDR TaqI polymorphism) were evaluated. The adopted significance level was 5%, and statistical analyses were performed using the SPSS version 20.0 program. The study consisted of 128 participants. The most prevalent age was from 60 to 65 years (N = 53; 41.4%). 66 elderly (51.6%) were part of the case group (hypovitaminosis D), while 62 were in the control group. In the case group, 30.2% had grade I obesity, 77.3% were hypertensive, and 51.5% were diabetic. The TT genotype was present in 47% of the case group and 54.8% in the control group (p=0.667). There was no association between serum vitamin D levels and the VDR gene variant TaqI polymorphism in an elderly Brazilian population.

Despite the abundant sunlight, vitamin D deficiency is prevalent in South Asian countries including Bangladesh. Information on vitamin D level is insufficient in adults particularly in female garment workers in Bangladesh. This study was designed to evaluate the status of vitamin D, parathormone (PTH), calcium, and alkaline phosphatase (ALP) among the female garment workers in Bangladesh. Blood samples were collected from female garment workers (n=40, case group) and general female workers (n=40, control group) in Dhaka. Serum vitamin D, PTH, calcium, and ALP were measured by chemiluminescence microparticle immunoassay. The mean level of vitamin D was significantly (p<0.001) lower in case (14.2±2.6 ng/mL) than in the control (22.4±2.4 ng/mL) group. No significant difference was found at mean of PTH and calcium between case (33.9±17.2 pg/mL;9.1±0.6 mg/dL, resp.) and control (35.9±16.3 pg/mL;9.3±0.6 mg/dL, resp.) group. The mean ALP in case (117.2±14.4 U/L) group was significantly (p<0.001) higher than the control group (80.5±30.6 U/L). Overall, PTH level did not show significant correlation with vitamin D. However, calcium and ALP levels showed a significant positive (p<0.05) and negative (p<0.001) correlation with vitamin D, respectively. This study indicates a high prevalence of vitamin D deficiency in the female garment workers in Bangladesh.

Background Vitamin D deficiency and vitamin D receptor (VDR) gene polymorphisms are strongly associated with tuberculosis (TB) susceptibility in countries with four seasons. As a country with sufficient sunlight for vitamin D production in skin, the incidence of TB in Indonesia remains high. Objective To assess for possible associations between the incidence of tuberculosis and serum vitamin D level, as well as VDR FokI polymorphisms in children. Methods A case-control study was conducted at the Department of Child Health, Dr. Mohammad Hoesin Hospital, Palembang from November 2011 to April 2012. Subjects were children with TB (the case) and children without TB who had been exposed to TB in the home (the control). Serum vitamin D [1,25(OH)2D3 or calcitriol] level was measured by immunodiagnostic system (IDS) 1,25-dihydroxy vitamin D enzyme immunoassay (EIA) kit. The VDR FokI polymorphisms were identified by polymerase chain reaction (PCR) and restriction-fragment length polymorphism (RFLP) analysis. Results Sixty subjects was divided equally into the case and control groups. The mean serum calcitriol level in the case group was significantly lower than that of the control group [105.5 (SD 66.9) pmol/L vs. 162.9 (SD 52.9) pmol/L, respectively; (P=0.001)]. We found 9 subjects with calcitriol deficiency, 8 in the TB group and 1 in the healthy contact group (OR 10.5; 95%CI 1.2 to 90.7) The VDR FokI polymorphism was seen in 28 subjects in the case group and 22 in the control group (OR 5.0; 95%CI 0.9 to 26.4). Conclusion Vitamin D (calcitriol) deficiency and lower serum levels are associated with higher risk of TB in children. The VDR gene FokI polymorphism also contributes to susceptibility for TB.
