<p><strong>Background</strong>: Diabetes mellitus is an emerging non communicable, life style disease. The aim was to evaluate the drug utilization pattern of anti-diabetic drugs in diabetic outpatients and monitor the adverse drug reactions (ADRs) associated with anti-diabetic therapy.</p><p><strong>Materials and</strong> <strong>methods</strong>: A prospective observational study was carried out in adult diabetic patients from the Department of General Medicine of a rural tertiary care hospital in October 2013- December 2014 after obtaining written informed consent from the patient and approval from institutional ethics committee. Demographic data, drug utilization pattern, cost of the antidiabetic drugs along with defined daily dose was calculated. ADRs due to anti-diabetic drugs were summarized.</p><p><strong>Results</strong>: A total of 450 patients were enrolled in the study of which there was male preponderance and majority belong to geriatric age. The most common comorbidity was hypertension. Insulin was most commonly prescribed drug during hospital stay and metformin at the time of discharge. Of the combination therapy dual therapy of metformin and glimipiride was most preferred. The total antihyperglycaemic drug consumption was 15.65 DDD/100 bed days. 67 ADR were reported of which the most common was hypoglycaemia.</p><p><strong>Conclusions</strong>: The study showed insulin and metformin were the most commonly used drugs. The prescribing trend also appears to be moving towards combination therapy particularly two drug therapies. The most commonly reported ADR was hypoglycaemia.</p>

Background: Diabetes Mellitus is a chronic disease and its life-long management causes burden on lifestyle and financial condition of the patients. Drug utilization studies provide useful insights into the current prescribing practices.Methods: To evaluate the drug utilization pattern of anti-diabetic drugs in diabetic patients. A prospective observational study was carried out in adult diabetic patients visiting the Wards and Outpatient Department of General Medicine of a tertiary care hospital. The demographic data and utilization of different classes of anti-diabetic agents as well as individual drugs were analyzed.Results: In 125 patients (Male-65, Female-60), a total of 379 drugs (average 3.032±2.05) were used per day, out of which 76 (20.05%) were rational fixed dose combinations (FDCs) and 261 (68.86%) were prescribed from National List of Essential Medicines (NLEM) 2015. The number of drugs prescribed to be ingested was 326 (86.01%) and 63 (16.62%) were injectables.Conclusions: It was found that the prescription tendencies of the doctors were quite rational. More improvement can be done by sensitizing them to prescribe more drugs from NLEM. The limitations in the affordability of rural population should be taken care of while prescribing drugs for this chronic disease.

Corticosteroids are widely used compounds for allergic reactions, autoimmune diseases, inflammatory conditions, hormone replacement therapy etc. Hence, with widespread use and actions, these have several interactions with drugs and diseases. The purpose of this study was to obtain information about Corticosteroids prescribing and utilization pattern, to understand the prescribing behaviour of physicians and to identify drug interactions. A retrospective observational study was conducted in the department of dermatology and general medicine in a tertiary care hospital for 6 months. All the patients receiving any category of steroid therapy were included, and the prescribing and tapering pattern of steroids were reviewed. Drug utilization pattern (DUR) was observed and analysed among 150 patients during the study period. The results revealed that steroids were prescribed for various respiratory illnesses (66%) and skin-related conditions (34%). The steroid utilization was found to be more in elderly patients, particularly in males. Intravenous administration was common in 33% of cases. Budesonide was the most commonly prescribed steroid (36%), followed by Hydrocortisone (24%) and Dexamethasone (14%). The most frequent drug-drug interaction was between Hydrocortisone and Theophylline as well as Hydrocortisone and Hypoglycaemic agents. Most drugs were prescribed rationally, although some factors like prescribing drugs in the brand name, without mentioning route of administration, frequency and dose were deviating away from rationality. Not much variation was found in the pattern of prescription amongst healthcare professionals. Although most of the drugs were prescribed rationally, the involvement of a clinical pharmacist in patient care can help in more rational prescribing along with prevention and early detection of ADRs which can directly promote drug safety and better patient outcomes.

Introduction: Carbapenem Resistant Enterobacteriaceae (CRE) are major cause of community as well as healthcare associated infections and have limited treatment options. Measuring the magnitude of the problem of CRE, it is important for making strategies to lower its spread. Aim: To assess the incidence and prevalence rate of CRE in a tertiary care hospital of Gujarat, India. Materials and Methods: Retrospective data was collected for a period from 2014 to 2018 using Laboratory Information System (LIS). Prevalence of CRE was determined as number of CRE isolated per 100 Enterobacteriaceae isolated during the study period whereas incidence rate was determined as number of CRE cases per 1000 patient-days. Consumption of Carbapenems was calculated as Defined Daily Dose (DDD) per 1000 patient-days. Demographic data including age, gender, location in the hospital and sample type from which CRE was isolated was also analysed using Microsoft Excel. Results: The incidence of CRE cases per 1000 patient-days in 2014 to 2018 was 1.66, 2.11, 1.90, 2.26 and 1.91, respectively with an overall incidence of 1.99 per 1000 patient-days. The overall prevalence of CRE over a period of five years was found to be 29.07%. Klebsiellasp. was the most common CRE and had the highest percentage of Carbapenem resistance among all Enterobacteriaceae. Conclusion: The rate of CRE in present study was high and worrisome. Screening of the patient for CRE, source isolation and stringent implementation of infection control practices is required to confine the spread of CRE in this institute.

Background: Despite alarming rise in prevalence of cardiovascular diseases (CVDs), there is paucity of Indian data in such population. Drug Utilization Study (DUS) in government clinical setup is instrumental in developing effective healthcare system of the country. The study was carried out with objectives to study the pattern of usage of drugs in CVDs in patients of Medicine Outpatient Department (MOD), Sir Sayajirao General Hospital, Vadodara and to have information about the current trends with CVDs and their correlation with different WHO indicators. Methods: A cross-sectional, observational DUS was carried out in adult patients of either sex. The data were recorded in previously prepared case record form for a period of four months (March 2014 to July 2014). The data were analyzed as per WHO prescribing indicators. Results: Among 1020 patients, 570 (55.88%) were female and 450 (44.11%) were male. Most of patients (30.49%) belonged to the age-group of 60-69 years. Total 19 CVDs were observed during the study period, with hypertension prevailing in maximum cases (70.88%). Diabetes mellitus was most frequently encountered (21.6%) non-cardiovascular comorbidity. Number of drugs prescribed by brand name was as low as 15 (19.7%) and >80%, drugs were prescribed by generic name. Number of drugs per prescription was 6.4. Conclusion: This study gives an idea of treatment measures being conducted in a large population at a tertiary care hospital in Gujarat and gives a comparative description of drug utilization pattern in similar settings across other regions of India.

Background: Dengue fever (DF) is a serious public health problem, gaining global attention because of its morbidity and mortality. Less studies on Drug utilization pattern of DF in India. The present study was undertaken to analyze Drug utilization pattern of dengue infection in a tertiary care hospital.Methods: A prospective cross-sectional study was conducted for a period of 2 months in a tertiary care hospital and the data was analyzed by using various drug use indicators.Results: A total of 52 prescriptions were analyzed. Male and female were (46.16%, 53.84%), A total of 330 drugs were prescribed. 86 (26.06%) antibiotics, 70 (21.21%) antipyretics, 51 (15.46%) antacids, 19 (5.76%) multivitamins, 52 (15.76%) anti-emetics and 52 (15.76%) papaya leaf extract was prescribed. 84 (25.46%) oral and 246 (74.54%) injectable drugs. IV-fluids given to all patients. 10 (19.24%) were given blood transfusion. 6.17 drugs per prescription. 96% drugs were prescribed by brand names. High DDD for drugs like diclofenac (184), doxycycline (115), metronidazole (2.5), pantoprazole (161.5), rabeprazole (34.6), ondansetron (200) was observed.Conclusions: Most commonly used drugs are antipyretics, antibiotics, antacids, antiemetic, papaya leaf extract. As the incidence of DF is increasing with epidemics, demand for specific treatment guidelines is in great need. Early recognition of the disease, with a rational approach in case management leads good clinical outcome.
