Background. Rehabilitation of patients after radical treatment of breast cancer is especially relevant due to the fact that successes in diagnostics and treatment of this disease in recent years have led to an increase in the life expectancy of female patients. Aim. to compare the efficiency of various methods of low-frequency low-intensity magnetotherapy in patients operated on for breast cancer, in the early terms (24 days) after surgery. Methods. Objective and instrumental examination was performed in 78 patients after radical surgical treatment of breast cancer in the early stages (24 days) after surgery. All patients received a course of low-frequency low-intensity magnetotherapy. Results. When applying the extended technique (the effect on the segmental-reflex region and upper limb from the side of the surgery), patients noted an improvement in the quality of life, a decrease in swelling of the upper limb, and a decrease in pain syndrome. A decrease in the number of postoperative complications and the duration of lymphorrhea was noted. Conclusion. Thus, the use of various methods of low-frequency low-intensity magnetotherapy is advisable to use in the early terms (24 days) after surgical treatment, however, the use of the advanced technique provides high function capabilities and enables to get a more pronounced and lasting clinical result.

We conducted an objective and instrumental examination in 64 patients who underwent radical surgical treatment of breast cancer (breast cancer) at an early stage (24 days) after surgery. Patients were divided into 2 groups by simple randomization: the 1st group of patients received a course of general magnetic therapy, the 2nd group received a placebo procedure. Patients who received general magnetic therapy showed improvement in quality of life, reducing the intensity, duration, frequency and radiation of pain; decrease in the circumference of the upper limb from the surgical treatment; a decrease in the manifestations of the syndrome of inflammation, improve microcirculation from the surgical treatment, and also on reducing venous stasis and muscular-tonic syndrome of shoulder region. Thus, the use of general magnetic therapy is advisable to use in the early stages (24 days) after surgical treatment for breast cancer.

Background: Various methods of physical therapy are widely used in the rehabilitation of patients with cancer (magnetotherapy, pneumocompression, electrotherapy, laser therapy). The goal of such rehabilitation is to minimize side effects and complications after radical treatment. However, a staged approach to the appointment of physiotherapeutic factors in the early period after surgical treatment of breast cancer has not been developed enough. Aim: To develop a staged approach to the appointment of physiotherapeutic factors at different times after surgical treatment of breast cancer. Design: a randomized-placebo-controlled study. Location: the clinic named after professor Yu. N. Kasatkin of the Russian Medical Academy of Continuing Professional Education of the Ministry of Health of the Russian Federation, Moscow. Population: Examination and treatment of 78 patients aged 30-70 after surgery for breast cancer were performed. The main group of 39 patients underwent a 2-stage course of medical rehabilitation: the first course on days 2-4 after surgery and the second course after 1-1.5 months against the background of adjuvated radiation therapy. The control group of 39 patients underwent physiotherapy placebo procedures. Methods: All the patients underwent medical rehabilitation: individual exercise therapy, balance-therapy, sessions with a medical psychologist, course exposure to an alternating low-frequency electrostatic field and general magnetic therapy. Results: two-stage physical rehabilitation improves the quality of life, decreases swelling, increases the range of motion, decreases pain, decreases the number of postoperative complications, and shortens the duration of lymphorrhea. Conclusions: It is advisable to include general magnetic therapy procedures and exposure to an alternating low-frequency electrostatic field in combination with physiotherapy exercises, balance-therapy and individual lessons from a medical psychologist in rehabilitation courses. Clinical Rehabilitation Impact: Prescribing two courses of medical rehabilitation after breast cancer surgeries in the early stages (on days 2-4) and 1-1.5 months after surgery contributed to a decrease in pain and postoperative edema, and an increase in the range of motion in the shoulder joint, preventing severe lymphostasis and improving the quality of life.

Results of randomized prospective study with participation of 94 patients aged from 20 to 70 years with monosegmental lumbar spine lesions are presented. Minimum invasive surgical interventions were performed in 55 patients from the main group. Control group consisted of 39 patients in whom decompressive-stabilizing operations via conventional posteromedian approach with skeletization of posterior segments of vertebral column were performed. Average size of operative wound in open interventions more than 10 times exceeded that size in minimum invasive interventions and made up 484 ± 56 and 36 ± 12 sq.cm, respectively. Mean blood loss was 326.6 ± 278.0 ml in the main group and 855.1 ± 512.0 ml in the comparative one. In the main group no one patient required substitution hemotransfusion, while in 13 patients from the comparative group donor erythrocytic mass and/or fresh-frozen plasma were used to eliminate the deficit of blood components. Intensity of pain syndrome in the zone of surgical intervention by visual analog scale in the main group was lower than in comparative group. In the main and comparative groups the duration of hospitalization made up 6.1 ± 2.7 and 9.7 ± 3.7 bed days, respectively. In no one patient from the main group complications in the zone of operative wound were noted. Three patients from the comparative group required secondary debridement and in 1 patient early deep operative wound suppuration was observed. Application of low invasive surgical techniques for the treatment of patients with degenerative lumbar spine lesions enabled to perform radical surgical treatment with minimal iatrogenic injury. The method possessed indubitable advantages over the conventional open operations especially intraoperatively and in early postoperative period.

Background: This study examined the effect of a portable low-frequency electrostimulation (ES) device on patients diagnosed with chemotherapy-induced peripheral neuropathy (CIPN) immediately after chemotherapy for breast cancer. Methods: A single-center, randomized, placebo-controlled trial was conducted. A total of 72 patients newly diagnosed with CIPN were enrolled and randomly placed into the ES (n = 36) or the sham ES group (SES; n = 36). Duloxetine or pregabalin was prescribed to all participants from the initial assessment. The devices for 14 days, at least twice a day, for at least 120 minutes. The primary outcomes were the overall intensities of the CIPN symptoms as assessed using Numerical Rating Scale (NRS). Secondary outcomes included Total Neuropathy Score (TNS), European Organization for Research and Treatment of Cancer Quality of Life (EORTC-QLQ), Chemotherapy-Induced Peripheral Neuropathy 20 (CIPN20), Functional Assessment of Cancer Therapy-Breast (FACT-B), and Instrument on Pattern Identification and Evaluation for CIPN (IPIE-CIPN). Results: No differences in NRS scores were found between the patients in the ES and the SES group ( P = 0.267). Patients in both groups showed significantly reduced CIPN intensities (ES P < .001; SES P < .001). No significant differences between the groups were found in TNS, EORTC-QLQ, CIPN20, and FACT-B. The general symptoms of CIPN diagnosed as cold arthralgia showed significance only in the ES group ( P = .006). Conclusion: Compared with a placebo, the effectiveness of the low-frequency ES device with pharmacological intervention was not significantly different, but a therapeutic effect was possible.

Objective: To assess the functionality of the affected upper limb in children diagnosed with hemiplegia aged between 4 and 8 years after applying low-intensity modified constraint-induced movement therapy(mCIMT). Methods: Prospective case series study. A mCIMT protocol was applied for five weeks, with two hours of containment per day. The study variables were: quality of movement of the upper limb, spontaneous use, participation of the affected upper limb in activities of daily living, dynamic joint position, grasp-release action, grasp strength, supination and extension elbow movements. Four measurements were performed, using the QUEST scale, the SHUEE Evaluation, a hand dynamometer and a goniometer. Results: The sample was composed of 8 children with moderate manual ability. Statistically significant differences were detected in all the studied variables (p&amp;lt;0.05). The greatest increase occurred in spontaneous use from assessment 1-4 (p = 0.01), reaching 88.87% active participation in bimanual tasks. The quality of movement of the upper limb obtained a significant value due to the increase in dissociated movements and grasp (p = 0.01). Conclusion: A low dose (50 hours) of mCIMT increased the functionality of children diagnosed with congenital hemiplegia between 4 and 8 years of age with moderate manual ability.

The aim of the research is to study the results of minimally invasive surgery for the treatment of nonspecifi c infectious spine lesions, and to present personal experience. Material and methods. Three patients with purulent nonspecifi c spondylodiscitis of lumbar spine got minimally invasive surgical treatment. The essence of surgical intervention included X-ray-assisted placement of tubular retractors in the area of intervertebral disc aff ected by purulent process; debridement and irrigation with antiseptic solution of interbody space, and setting a drain tube. All patients were treated during the period from December 2016 to February 2019. They were examined before surgery and in 3 months aft er the discharge. Laboratory parameters (CBT, C-reactive protein, bacteriological analysis of intervertebral disc contents) and instrumental research methods (computer tomography, magnetic resonance imaging) were evaluated. ODI, VAS, SF-36 questionnaires helped to assess pain syndrome and life quality of patients. Results. In 100 % of studied cases infection was caused by Staphylococcus aureus. The result of treatment in all cases was assessed as good, because pain syndrome, neurological disorders and inflammatory process were arrested; there were no signs of disease progression. In all cases, life quality of patients was restored. In one case, additional surgical intervention was required (posterior instrumental fi xation of spine) because of instability signs. Conclusion. Minimally invasive surgical treatment of purulent single-level spondylodiscites can be recommended for practical use. The use of such approache allows to verify purulent infection agent, to damage soft tissues less getting proper sanitation of interbody space. It also allows to stop the infection and restore life quality of a patient
