The complex root canal system of maxillary second premolars shows variations among different populations. The aim of this study was to evaluate the root canal morphology of second maxillary premolars in the romanian population. The number of roots, root canals and the root canal configuration were investigated using radiographs in both buccal-oral and mesio-distal incidence. They were categorized using Weine’s classifi cation. Among the 480 teeth, 90.83% had one root and 66.67% one root canal. The frequency of secondary maxillary premolars with two root canals (33.12%) shouldn’t be underestimated. Knowing the variations of the internal root canal morphology of maxillary second premolars in a certain population can lead the clinician to a better approach of the endodontic treatment.

Objective: The aim of this work was to show the management of severe curvatures done with the new generation of rotating files through a safe and predictable protocol. Methods: Three clinical cases were described. A safe protocol was explained using the latest generation of rotating files for the shaping of the anatomy of complex root canals. All the cases presented 90°, 45° curvatures and/or double curvatures. Results: The instrumentation of canals increased their surface area and volume. All the cases with 90° and 45° curvatures were optimally shaped. Conclusion: With the help of rotating files in the martensitic alloy, it is possible to work safely, while avoiding the risk of file separation, even in root canals with severe curvatures, thus achieving success even in complex clinical cases.

Abstract Background Mandibular premolars demonstrate high variability in root canal morphology, especially mandibular first premolars. The purposes of this study were to determine the prevalence of root canal configurations of mandibular premolars according to Vertucci classification in a Thai population. Methods Total of 1159 CBCT images of Thai patients who received radiographic imaging at the Department of Radiology, Faculty of Dentistry, Chulalongkorn University in 2017–2018 was evaluated. The data were reported using descriptive statistics and the relationship between the prevalence of the root canal complexities and sex was analyzed using the chi-squared test. Results The most common root canal configuration was Vertucci type I, with a 63.1% and 98% prevalence in the mandibular first and second premolars, respectively. More than 98% of mandibular premolars had a single root. The prevalence of a bifurcation was 28.5% and 1.5% in the mandibular first and second premolars, respectively. The prevalence of a trifurcation was 3.2% in the mandibular first premolar. A C-shaped root canal was observed at 23.7% and 0.7% in the mandibular first and second premolars, respectively. The level of branching was mostly found at the middle 1/3 of the root. Bilateral appearance of the same root canal configuration was identified in 80.3% and 95.9% in the mandibular first and second premolars, respectively. There was no relationship between sex and the prevalence of a bifurcation, trifurcation, or C-shaped root canal. Conclusion Mandibular first premolars have more root canal complexities than mandibular second premolars. Horizontal tube shift x-ray technique, CBCT, dental operating microscope, and knowledge of root canal configurations have an important role in root canal identification in mandibular premolar with suspected complex root canal morphology.

The proper knowledge of the root canal system anatomy is crucial to perform successful endodontic treatments. Maxillary lateral incisors usually have a single root. However, aberrant canal configurations of this tooth have been reported in the literature. The clinician should be aware of the difficulties arising from this more complex root canal anatomy and should be able to manage them. A rare case of a maxillary lateral incisor with three root canals is presented in this article; the challenge of diagnosis and treatment in similar situations is also discussed.

ABSTRACT Aim To investigate the influence of cervical preflaring on apical file size determination using four different rotary instruments. Materials and methods Fifty root canals from extracted human maxillary premolars with complete root formation, straight roots were used for the study. Access opening was done and the working length established with 8 no K-file for each canal. Teeth were randomly divided into five groups of 10 canals. In Group 1- no preflaring was done and acted as control and in Groups 2, 3, 4 and 5 cervical and middle third preflaring of the root canals were done using Gates Glidden drills, Protaper instruments, Race instruments and Galaxy files respectively. After preflaring, the apical file size determination was done and the initial apical file (IAF) was fixed at the working length. Teeth were sectioned transversally 1 mm from the apex, with the binding file in position. The samples were imaged under stereomicroscope with 30× magnification. Root canal and file maximum diameters were recorded for each sample. The readings were subjected to analysis of variance test and Scheffe's multiple comparison test. Results Preflaring with Race instruments lead to most accurate determination of the IAF. It was followed by Protaper, Galaxy files and Gates Glidden drills. Conclusion Traditional method of apical size determination may lead to a substantial underestimation of actual canal size. Cervical preflaring increases the accuracy of apical size determination. Clinical significance Thus, cervical preflaring is recommended before selection of IAF as it increases the accuracy of apical size determination. How to cite this article Ashwini TS, Bhandari S. This study aimed to assess the effectiveness of ultrasonic vibration and thermo-hydrodynamic obturation (VibraTHO) using two types of root canal sealers, in comparison to the single-cone (SC) technique and a calcium silicate-based root canal sealer in complex root canal anatomies. Thirty single-rooted human maxillary premolars with two canals that had a complex root canal anatomy of transverse anastomoses or ramifications were prepared and assigned to the following three experimental groups, according to the filling method: SE group, SC technique with Endoseal TCS; VE group, VibraTHO with Endoseal TCS; and VG group, VibraTHO with GuttaFlow 2. Each tooth was scanned using micro-computed tomography, and the volume percentages of the filling material were calculated. The analysis of variance was used to analyze the statistical differences between the three groups (p < 0.05). The mean volume of the filling material was higher in the VG and VE groups than that in the SE group (p < 0.05) along the apical to middle-to-coronal thirds, and significant differences were observed between each root canal area (p < 0.05), with the only exception being at the apical thirds between the VE and SE groups. The VibraTHO technique using GuttaFlow 2 can be a more effective root canal filling method for anatomically complex root canal systems than the SC technique with Endoseal TCS. On the other hand, the VibraTHO technique using Endoseal TCS has a limited effect on improving the quality of the root filling at the apical portion of anatomically complex root canal systems, compared to the SC technique with Endoseal TCS.
