Abstract Background: Flavors in tobacco products is a subject of public health debate and increasing regulatory attention. There is interest in gaining an in-depth understanding of flavored cigar smoking prevalence and behaviors to address the use of flavors in cigars and questions of public health.Methods: Seven publicly available data resources that assess flavored cigar use were analyzed. Two focus on youth tobacco use (NYTS, MTF), four focus on adult tobacco use (HINTS-FDA, NATS, TPRPS, TUS-CPS), and one on both groups (PATH). Available data (2011-2019) were analyzed to assess usage trends over time. In addition, longitudinal analysis of PATH adult data examined whether flavored cigar use was associated with future use of cigarettes or increased use of cigars.Results: Youth past 30-day estimates of cigar use ranged from 2%-10% for both flavored and non-flavored cigars, slightly higher in high school vs. middle school age subpopulations. These estimates have been stable or declined across all survey years within the respective surveys. Consistent trends were observed regarding frequency of use; most youth using cigars do so 1-2 days per month. Similar findings were observed for adult cigar users, with five surveys indicating less than 10% currently use cigars. Flavored cigar use is at less than 5% across all data sources. These overarching use estimates were essentially flat over time. Frequency of youth cigar use remained consistent over time, with most youth reporting cigar use on 1-2 days per month. In addition, multivariable modeling of PATH adult data did not identify an association between flavored cigar use and future use of cigarettes or increased use of cigars.Conclusions: No evidence was found of increased use or different usage patterns, among either youth or adults, of flavored cigars vs. non-flavored cigars. While these trends should continue to be monitored, there is no indication of existing or emerging public health concerns related to flavored cigars within the seven large, nationally representative, US government-funded epidemiologic databases examined.

Use of heated tobacco products (HTPs) among current smokers is becoming increasingly popular in Japan. This study aims to compare characteristics and tobacco-related behaviors among concurrent users of HTPs and combustible cigarettes (n = 644) with exclusive smokers (n = 3194) or exclusive HTP users (n = 164). The secondary aim was to explore heterogeneity within concurrent use subgroups. Data were from Wave 1 of the ITC Japan Survey, a nationally representative web survey conducted from February to March 2018. Concurrent cigarette-HTP users were younger and wealthier than exclusive smokers. However, there were no difference in the frequency of smoking, number of cigarettes per day, and smoking cessation behaviors between the two groups, suggesting that HTPs reinforce nicotine dependence. Compared to exclusive HTP users, concurrent cigarette-HTP users reported higher frequency of non-daily HTP use, and lower number of tobacco-containing inserts per day. Almost all concurrent cigarette-HTP users smoked every day (93.9%); 48.4% both smoked and used HTPs daily (dual daily users, n = 396), while 45.5% were daily smokers and non-daily HTP users (predominant smokers, n = 213). Concurrent user subgroups differed from each other on age, tobacco use behaviors, and quit intention. Alongside heterogeneity between concurrent and exclusive product users, differences across concurrent use subgroups highlight the importance of considering frequency of use in characterizing poly-tobacco users.

Flavored cigar use is common among cigar smokers, particularly those at younger ages. Several US localities have implemented policies restricting the sale of flavored tobacco products, including cigars. We estimated the population health benefits of removal of flavored cigars throughout the US in terms of reductions in cigar smoking-attributable mortality due to increased cessation and reductions in cigar smoking prevalence due to decreased initiation and continuing use. Monte Carlo simulation was used to estimate possible ranges for these values. We used published estimates of cigar use and attributable mortality in the US, as well as prior study conclusions on the effect of local and national flavor restriction policies. We estimated that removal of flavored cigars would result in approximately 800 (90% prediction interval = 400–1200) fewer cigar smoking-attributable deaths in the US each year and 112,000 fewer cigar smokers (90% prediction interval = 76,000–139,000) in each cohort of 18 year olds. The removal of characterizing flavors in cigars sold in the US is thus projected to have substantial public health benefits over time.

Abstract Objective To assess changes in engagement with online tobacco and electronic cigarette (e-cigarette) marketing (online tobacco marketing) among adolescents in the United States between 2013 and 2015. Methods We assessed the prevalence of six forms of engagement with online tobacco marketing, both overall and by brand, among adolescents sampled in Wave 1 (2013–2014; n = 13651) and Wave 2 (2014–2015; n = 12172) of the nationally representative Population Assessment for Tobacco and Health Study. Engagement was analyzed by tobacco use status: non-susceptible never tobacco users; susceptible never tobacco users; ever tobacco users, but not within the past year; and past-year tobacco users. Results Among all adolescents, the estimated prevalence of engagement with at least one form of online tobacco marketing increased from 8.7% in 2013–2014 to 20.9% in 2014–2015. The estimated prevalence of engagement also increased over time across all tobacco use statuses (eg, from 10.5% to 26.6% among susceptible adolescents). Brand-specific engagement increased over time for cigarette, cigar, and e-cigarette brands. Conclusion Engagement with online tobacco marketing, both for tobacco and e-cigarettes, increased almost twofold over time. This increase emphasizes the dynamic nature of online tobacco marketing and its ability to reach youth. The Food and Drug Administration, in cooperation with social networking sites, should consider new approaches to regulate this novel form of marketing. Implications This is the first study to estimate the national prevalence of engagement with online tobacco marketing among adolescents over time. The estimated prevalence of this engagement approximately doubled between 2013–2014 and 2014–2015 among all adolescents and, notably, among adolescents at relatively low risk to initiate tobacco use. This increase in engagement could represent public health harm if it results in increased initiation and use of tobacco products. Stronger federal regulation of online tobacco marketing and tighter control of access to tobacco-related content by social media sites could reduce adolescents’ exposure to and engagement with online tobacco marketing.

Background: Understanding the size of a population is necessary to define the burden of disease, evaluate opportunities to improve health, inform service planning and assess demographic trends over time. Methods: In this article, we described available data on the number of admissions and number of people admitted to custody in Canada. We identified gaps in data, and described the potential value of these data for public health and health care purposes. Conclusion: We recommend the systematic collection and dissemination of relevant data on this population in Canada.

BioSense and Louisiana influenza-like-illness, gastrointestinal, and upper respiratory syndrome classifications were applied to Louisiana emergency department data to examine if varying syndrome definitions yield similar results when applied to the same data. Results were highly correlated for each syndrome pair however syndrome percentage means were significantly different. Most C2 alerts occurred on corresponding weeks, providing confidence in the use of C2 on current syndrome definitions for aberration detection. As public health jurisdictions work towards developing common syndrome classifications for comparability across jurisdictions, this analysis provides evidence that current differences in syndrome definitions may not hinder comparability of trends over time.

Abstract This review summarizes the current and previous data on dietary supplement (DS) use collected from participants in the NHANES, describes the NHANES DS database used to compute nutrient intakes from DSs, discusses recent developments and future directions, and describes many examples to show the utility of these data in informing nutrition research and policy. Since 1971, NHANES has been collecting information on the use of DSs from participants. These data are critical to national nutrition surveillance and have been used to characterize usage patterns, examine trends over time, assess the percentage of the population meeting or exceeding nutrient recommendations, and help to elucidate the sources contributing nutrients to the diet of the US population. More than half of adults and approximately one-third of children in the United States currently use ≥1 DS in the course of 30 d. DSs contribute to the dietary intake of nutrients and bioactive compounds in the United States and therefore need to be assessed when monitoring nutritional status of the population and when studying diet-health associations. With the recent development and availability of the Dietary Supplement Label Database, a comprehensive DS database that will eventually contain labels for all products marketed in the United States, NHANES DS data will be more easily linked to product information to estimate nutrient intake from DSs. NHANES provides a rich source of nationally representative data on the usage of dietary supplements in the United States. Over time, NHANES has both expanded and improved collection methods. The continued understanding of sources of error in collection methods will continue to be explored and is critical to improved accuracy.
