Abstract Purpose: Monozygotic twin (MZT) pregnancies increase the risk of maternal and infant mortality and reveal many complications. In the present study, we aimed to share our assisted reproductive techniques (ART) through the analysis of perinatal outcomes in MZT pregnancies.Materials and Methods: In this retrospective clinical cross-sectional study, 1159 cycles in the In vitro fertilization (IVF) program between October 2014 and December 2019 were retrospectively evaluated and perinatal outcomes and general clinical conditions were analyzed. Results: 16 cycles with MZT were achieved with IVF, resulting in a MZT incidence of 1.38%. The MZT incidence aged≤35 and >35 years were 0.2% and 1.1%, respectively. Eight MZT pregnancies resulted in a live birth, while 5 MZT resulted in an abortus. A significant positive correlation was found between the number of trials and the age of female (r:0.674; p=0.004) and male (r:0.657; p=0.006). Cumulus-Oocytes Complexes (COC) (r:0,635; p=0,008), Metaphase II Oocyte (MIIO) (r:0,627; p=0,009), Pronucleus Oocyte (PO) (r:0,585; p=0,017) were correlated with serum AMH levels. Number of MZT was positively correlated with the male age (r:0,527; p=0,036), while negatively correlated with embryo transfer day (ETd) (r: -0,548; p=0,028). Conclusion: The pregnancies that we identified and achieved with IVF had a similar incidence of MZT with the literature, although its risk was more for women >35 years. MZT pregnancies can become a problem as the increasing number of IVF applications continue due to its potential risks for both maternal and fetus.

Anamnesis, physical examination, and additional tests may reveal a specific cause of reproductive failure in infertile men. Whenever this is found, a specific treatment or cure should be applied. When no such treatment is available, or when specific treatment has failed, techniques of assisted reproduction may be proposed to couples suffering from long-standing male infertility. The rationale behind these is to bring the spermatozoa closer to the oocyte in an attempt to enhance the fertilization process. In recent years the role of assisted reproduction has become more important, and it has often been stated that these techniques have made clinical work-up or specific treatment of the male partner pointless. However, this is far from true. Not only may correction of a specific dysfunction in the male avoid the use of assisted reproductive techniques, but careful work-up and treatment may also enhance the outcome of these treatments. Assisted reproductive techniques should not be viewed as a primary treatment option, but rather as a complementary treatment when other treatments have failed, or have been judged inadequate after a complete work-up.

Abstract Background Exposure of oocytes to the endometrioma fluid has an adverse effect on embryonic quality. To determine whether adding transferrin and antioxidants to culture medium could counteract detrimental effects on mouse cumulus-oocyte complexes (COCs) induced by exposure to endometrioma fluid or not, we conducted an in vitro cross-sectional study using human and mouse COCs. Methods Eighteen women who had their oocytes exposed to endometrioma fluid during oocyte retrieval were enrolled. COCs from superovulated ICR female mice were collected. They were first exposed to human endometrioma fluid and then treated by transferrin and/or antioxidants (cysteamine + cystine). Subsequently, COCs function was assessed by molecular methods. Results This study observed that human COCs inadvertently exposed to endometrioma fluid in the in vitro fertilization (IVF) group led to a lower good quality embryo rate compared to intracytoplasmic sperm injection (ICSI) group. Exposure of mouse COCs to endometrioma fluid accelerated oocyte oxidative damage, evidenced by significantly reduced CCs viability, defective mitochondrial function, decreased GSH content and increased ROS level, associated with the significantly higher pro-portion of abnormal spindles and lower blastocyst formation (p < 0.05, respectively). This damage could be recovered partly by treating COCs with transferrin and antioxidants (cysteamine + cystine). Conclusions Transferrin and antioxidants could reduce the oxidative damage caused by COCs exposure to endometrioma fluid. This finding provides a promising new possibility for intervention in the human oocyte oxidative damage process induced by endometrioma fluid during oocyte pick-up.

Worldwide, about 200 million couples are infertile. Almost 15 per cent of them require ART and IVF treatments. Couples who are unable to conceive even after years of trying could use procedure called Assisted Reproductive Techniques (ART) to get pregnant. These includes Intrauterine Insemination, In-Vitro Fertilization (IVF), Spern / Egg donation, and Gestational Carriers called “Surrogacy”. People join for Surrogacy when they want to have their genes in the child but can not make it happen in natural course. There are two types of Surrogacy: (1) The man’s sperm and the woman’s ova is injected in the uterus of the surrogate mother who rents out her womb for the child, (2) If the Egg of the mother is not good enough to produce a baby, an Egg is then rented out by donor mother. A tripartite agreement is duly signed according to the terms and conditions prevalent at that time. This Editorial viewed Surrogacy Industry through the lens of intent, efficacy, and its purview.

Objective: To determine factors which are related to the number of antral follicles on infertile patients. Method: This cross sectional study was conducted in In-Vitro Fertilization (IVF) clinic of Graha Tunjung, Sanglah hospital, Bali. All fertile patients following the IVF program were calculated the number of antral follicles in both ovarian using transgene USG. This sample was recruited by random sampling from April 1st, 2001 to April 30th, 2011. We analyzed the data using Chi square test through SPSS for Windows 17.0 version. Result: Of 102 samples, the mean of patients’ age was 32.9% (SD 4.6) years old. From 72 patients (70.6%) experienced above 3 years of infertile period, the primary infertile was on 69 patients (67.7%). There was a relationship between patients’ age and the number of antral follicles significantly (prevalence ratio (PR) 1.41; 95% CI 1.11- 1.79). Meanwhile, the number of antral follicles and type of infertile (PR 1.02; 95% CI 0.76-1.37) also infertile period (PR 0.95; 95% CI 0.72-1.27) were not associated significantly. Conclusion: Patients’ age has an association with the number of antral follicles on IVF. Ectopic pregnancy (EP) is a rare and severe complication of assisted reproductive techniques (ART). A specific feature of EP after ART is a high frequency of rare atypical forms of EP, the absence of a specific clinical picture and a high probability of recurrent EP, which makes certain difficulties in terms of diagnosis. Interstitial pregnancies occur in 1–6% of all ectopic pregnancies and in 7.3% cases of EB which were the results of in vitro fertilization programs (IVF). The anatomical features of the blood supply in the intramural segment of the fallopian tube provide favorable conditions for the progression of pregnancy on the average up to 10 weeks of gestation, simulating the intra-uterine pregnancy. Chorionic villi, growing into the thickness of the myometrium, create a high risk of profuse intra-abdominal bleeding. The article presents a clinical case of a rare form of ectopic pregnancy in the interstitial segment of the fallopian tube after IVF. The patient had previously undergone a bilateral salpingectomy by tubal pregnancies. This clinical example shows the meaning of precise diagnosis and timely provision of highly qualified specialized medical care.

Purpose: This descriptive study was conducted to identify the factors causing stress in women undergoing in-vitro fertilization (IVF) treatment. Materials and methods: The study sample consisted of 151 women who were receiving IVF therapy in the assisted reproductive techniques units of three state hospitals in Istanbul. The study data were collected using a Questionnaire and a Distress/Problem Identification form both developed by the investigators. Results: A considerable percentage of the women whose income was less than their expenses stated that they feared their infertility treatment would be a failure. The majority of the women who gave the answer “the cause is not known” or “me” when they are asked “who is responsible for not being able to have a child” stated that they were annoyed when people asked questions about having children and felt strained and uneasy on the days of coming to the center. Conclusions: We suggest that trainings should be planned for health teams and health teams should establish support groups and organize meetings for couples
