Abstract Background: There is a great need for a valid hearing loss measurement tool in the Persian language to help identify hearing handicap and potential communicational difficulties among Persian speaking older adults. The present study was aimed to validate and adapt the original English version of Hearing Handicap Inventory for the Elderly (HHIE) into Persian language.Methods: A cross-sectional study was designed and data was collected from August to November 2019 in Tabriz, Iran among the older adults aged 60 years and above whose hearing loss had been confirmed by audiometry. Self-reporting and face-to-face interviews were the data collection methods in this study. The statistical analysis was performed using SPSS 26.0 (SPSS and STATA 14).Results: An exploratory factor analysis of data resulted in two factors, which included 9 of the 10 items and accounted for 87.00% of the variance. Cronbach’s alpha coefficient (0.85) and the test–retest reliability score (0.73) indicated good internal consistency.Conclusions: The results showed that HHIE-S is a valid and reliable tool for assessing hearing handicap among Persian speaking and Iranian older adults.

Introduction Hearing losses inherent to the natural process of aging represent today a major public health issue, despite the little attention that their adequate care still receives. Early recognition and proper management of these shortcomings can significantly improve hearing, as well as the patient's general quality of life, reducing the overall impact of this important and prevalent condition of the aging process. Objective The aim of this research was to evaluate the accuracy of the Hearing Handicap Inventory for the Elderly - Screening Version (HHIE-S) in the diagnosis of hearing loss in the elderly when compared with the audiometric test. Methods Through a cross-sectional study, our target population was composed of 138 individuals, aged over 60 and with any otorhinolaryngological complaints, recruited at the Clinic of Otorhinolaryngology and Speech Therapy of the Faculdade de Medicina de Marília (Famema), in the city of Marília, SP, Brazil. Patients already in the process of auditory rehabilitation were excluded, as well as those who did not demonstrate the minimum level of oral understanding necessary to allow the interview. Results The prevalence of hearing loss according to the questionnaire was of 76.1%, while audiometry showed 79.7%. We found the diagnostic accuracy of the instrument to be of 86.2%, with a sensitivity of 89.1% and a specificity of 75.0%, regardless of gender. Conclusion Thereby, we conclude that the standardized questionnaire under rating is suitable for the screening of hearing loss in the elderly, given its high accuracy and user-friendly quality.

Objective: To determine the level of hearing handicap and its age and gender association in older adults. Study Design: Cross sectional analytical study. Place and Duration of Study: Study conducted at Ear, Nose & Throat outpatients of Yusra General Hospital, National Institute of Rehabilitation Medicine Islamabad and Cantonment General Hospital Rawalpindi, from Jul to Sep 2017. Methodology: We recruited two hundred cases of self-reported hearing difficulty, using non-probability consecutive sampling, who fulfilled inclusion criteria. After recording demographic details including history, subjects were screened by the Screening Version of Hearing Handicap Inventory for the Elderly (HHIE-S). Followed by otoscopy and pure tone audiometry. Analysis was done using SPSS-24. Results: Study revealed 133 (65.5%) males and 67 (33.5%) females with mean age of 65.45 ± 7.50 years. Out of 179 (89.5%) had significantly high level of hearing handicap with HHIE-S score >43, while 21 (10.5%) revealed mild to moderate handicap with score of 17-42. Hearing handicap was significantly associated with aging (p<0.001), while no significant gender association was noted. Hearing loss was mainly of sensory-neural type 192 (96%). Conclusion: In older adults with self-reported hearing loss, high level of hearing handicap was present in majority (89.5%) and mild to moderate in 10.5%, with significant association with aging and hearing loss was predominantly sensory- neural type.

ABSTRACT Objective: To propose a care need classification for elderly people by identifying their functional demands. Method: Cross-sectional study carried out in São Paulo, in 2006, with 1,413 elderly (≥ 60 years old), participants in the Health, Well-being and Aging study (SABE – Saúde, Bem Estar e Envelhecimento). For the care need classification, we used the Guttman Scaling method e the frequency of assistance required by the elderly. Results: The hierarchy of activities of daily living had good internal consistency (α = 0.92) and satisfactory coefficients of reproducibility (98%), scalability (84%) and minimum marginal reproducibility (87%). Care need was categorized into: no need (requires no caregiver), minimum need (requires caregiver sporadically), moderate need (requires caregiver intermittently) and maximum need (requires full-time caregiver). Conclusion: This classification will allow identifying elderly that need assistance in everyday activities and will orientante health professionals in the development of a line of care.

OBJECTIVE: To evaluate the prevalence of undernutrition in older adults aged >75 years living in communities and to identify the main factors independently associated with undernutrition. METHODS: A cross-sectional study was conducted using a random sample of family physicians' medical records of 86 older adults aged >75 years living in the community studied. Their nutritional status was evaluated using the Mini Nutritional Assessment. RESULTS: A total of 10.5% of the elderly were undernourished and 41.9% were at undernutrition risk. According to the logistic regression multivariable model, the following characteristics: being widowed (OR=6.7; 95%CI=1.8-24.6); being institutionalized (OR=12.6; 95%CI=1.7-90.5); or having a negative self-perception of health (OR=15.0; 95%CI=3.3-69.1) were independently associated with a significant increase of undernutrition risk. CONCLUSION: The current study shows that undernutrition is highly prevalent in Portuguese older adults aged >75 years living in communities. The major factors independently associated with their undernutrition are being widowed and institutionalized and having negative self-perception of health. The results obtained show that undernutrition and its associated factors are very serious problems for older adults and a challenge in their health care.

The purpose of this study is to understand among a convenience sample of 400 adults aged 60 years of age or older (1) reasons for being willing or unwilling to participate in a vaccine clinical research study and (2) overall perceptions about vaccine clinical research. A cross-sectional study using a sample of older adults residing in the metro-Atlanta area and surrounding neighborhoods was conducted. The study questionnaire contained 37 questions, including questions about socio-demographics and perceptions about clinical trial processes. Statistical analysis was conducted using logistic regression. The adjusted modeling results indicated that sex, distance to research clinic, and being informed about the research findings played a role in the likelihood of an elderly person participating in a vaccine study. Males were more likely to participate in clinical trials as compared to females (OR: 2.486; CI: 1.042–5.934). Most participants were willing to travel up to 25 miles from the research clinic. Of the respondents, 45% were unlikely to participate if the results of the current trial are not shared. Improving access to clinical trials in terms of distance traveled and ensuring streamlined processes to inform participants about the results of the trial in the future would increase willingness to participate in vaccine clinical trials. The survey could serve as a useful tool for conducting vaccine studies and other clinical trials by understanding the barriers specific to the elderly.

Objective To assess physical performance (PP) and physical activity (PA) among older adults visiting primary healthcare centres (PHCCs) in Riyadh. Methods In this cross-sectional study, men and women (n = 74) aged ≥60 years were recruited from five PHCCs across Riyadh, Saudi Arabia, between 19 February and 6 August 2017. The Timed Up & Go test was used to assess PP. Self-reported PA was assessed using the Physical Activity Scale for the Elderly. Means and standard deviations were used to describe PP and PA scores across genders. A two-tailed independent samples t-test was used to estimate mean differences (MD) and 95% confidence intervals (CI) between genders. Results Men scored significantly lower than women on PP (MD: 2.11, 95% CI: 0.59 to 3.64) and PA (MD: −46.1, 95% CI: −80.96 to −11.25). Significant gender differences were observed for leisure time activities (MD: 17.35, 95% CI: 3.29 to 31.40), light household activities (MD: –10.86, 95% CI: –16.19 to –5.53) and heavy household activities (MD: –6.48, 95% CI: –11.73 to –1.23). Conclusions Men show significantly lower PP and PA than women. There are gender differences in leisure time activities and in light and heavy household activities.

The prevailing COVID-19 pandemic has dramatically affected the mental health and well-being of individuals. This cross-sectional study aimed to assess the perceived fear of COVID-19 among older adults in Poland and identify subpopulations with the highest risk of potential mental health disorders. The study was conducted in November–December 2020 on 500 people aged ≥60 years (mean M = 67.9, standard deviation SD = 4.2). In order to collect information on participants’ characteristics and COVID-19-related information, they were asked to complete a questionnaire based on recorded telephone calls. Perceived fear of COVID-19 was measured using our generated and validated seven-item tool: “Scale of fear of COVID-19 infection”, which ranged from 7 to 35. Multiple linear regression was performed to identify factors associated with the perceived fear of COVID-19. Our results showed that the highest level of fear of COVID-19 infection was observed among women (p = 0.025) and patients taking anticoagulants (p = 0.004). Moreover, older adults with higher anxiety levels were more likely to be fearful of COVID-19 (according to the GAS-10 scale; p < 0.001). These findings may help policy makers and healthcare workers to adapt and implement better mental health strategies to help the elderly fight fear and anxiety during the prevailing pandemic.

Key Points: Question: Can the traditional Chinese version of the hearing handicap inventory for elderly screening (HHIE-S) checklist screen for age-related hearing loss (ARHL) in elderly individuals? Findings: In this cross-sectional study of 1696 Taiwanese patients who underwent annual government-funded geriatric health checkups, the Chinese version of the HHIE-S had a sensitivity of 76.9% and a specificity of 79.8% with a cutoff score greater than 6 for identifying patients with disabled hearing loss (defined as a PTA > 40 dB). Meaning: The traditional Chinese version of the HHIE-S is an effective test to detect ARHL and can improve the feasibility of large-scale hearing screening among elderly individuals. Purpose: The traditional Chinese version of the hearing handicap inventory for elderly screening (TC-HHIE-S) was translated from English and is intended for use with people whose native language is traditional Chinese, but its effectiveness and diagnostic performance are still unclear. The purpose of this study was to evaluate the validity and reliability of the traditional Chinese version of the HHIE-S for screening for age-related hearing loss (ARHL). Methods: A total of 1696 elderly people underwent the government’s annual geriatric medical examination at community hospitals. In this cross-sectional study, we recorded average conducted pure-tone averages (PTA) (0.5 kHz, 1 kHz, 2 kHz, 4 kHz), age, sex, and HHIE-S data. Receiver operating characteristic (ROC) curve analysis was used to identify the best critical point for detecting hearing impairment, and the validity of the structure was verified by the agreement between the TC-HHIE-S and PTA results. Results: The HHIE-S scores were correlated with the better-ear pure-tone threshold averages (PTAs) at 0.5–4 kHz (correlation coefficient r = 0.45). The internal consistency of the total HHIE-S score was excellent (Cronbach’s alpha = 0.901), and the test-retest reliability was also excellent (Spearman’s correlation coefficient = 0.60, intraclass correlation coefficient = 0.75). In detecting disabled hearing loss (i.e., PTA at 0.5–4 kHz > 40 dB), the HHIE-S cutoff score of > 6 had a sensitivity of 76.9% and a specificity of 79.8%. Conclusions: The traditional Chinese version of the HHIE-S is a valid, reliable, and efficient tool for large-scale screening for ARHL.
