Abstract ObjectiveFrasier syndrome is a rare genetic nephropathy characterized by the presence of progressive glomerulopathy with proteinuria associated with male pseudo hermaphroditism. This case study described a picture of a young boy where the clinical suspicion context reminded the Frasier Syndrome. To our knowledge, this case is the first described in Haiti.Case studyThis is a 19 years old young phenotypically male, born with a genital anomaly, was seen on referral at the nephrology/dialysis unit of the internal medicine department of HUEH for evaluation and follow-up. Insidious progression of symptoms had occurred over 3 years. Over three months of outpatient follow-up, he had four sets of renal labs drawn, and all showed impaired renal function. At the ultrasound a bilateral cryptorchidism is described in the inguinal, and presence of functional ovaries with follicles of variable size scattered in the parenchyma. So, in the light of these anamnestic, clinical and paraclinical findings we concluded to the diagnosis of end-stage renal failure by progressive glomerulopathy in a context of Frasier's syndrome.ConclusionWith any clinical picture consisting of genital anomalies at birth, renal symptomatology during childhood and the diagnosis of renal failure during adolescence, rare genetic nephropathies, such as Frasier syndrome must be considered.

Introduction Renal failure either acute or chronic is a global public health problem. In developed countries, this condition occurs mainly in the elderly. The World Health Organization (WHO) has made preventing chronic disease a vital investment. The incidence of RF, long underestimated, increases with the age of the patient. Estimated at eight per million in children, it increases to 949 in octogenarians. In the registers of the Internal Medicine department of the HUEH, the country's reference center, kidney failure is very common after cardiac and gastric pathologies. The aim of this study will be to describe the epidemiological aspects of renal failure cases by studying quantitative and qualitative variables. Methodology This is a retrospective quantitative study conducted on sample patients in the Internal Medicine Department of the Hospital of the State University of Haiti (HUEH). The study was conducted on 39 patients admitted and registered in the internal medicine department of the HUEH during the period from May 2018 to December 2018 for the concept of renal failure. Result A total of 39 patients with 17 women (44%) and 22 men (56%).The mean age of the patients is 48.66 years, the main length of hospitalization is 21.7 days. Chronic renal failure (CRF) is the dominant diagnosis 27 cases (69.23%) compared to acute renal failure (ARF), 7 cases (17.95%), end-stage chronic renal failure (ESRF) 4 cases (10.26%) and severe chronic renal failure (SCRF) 1 case (2.56%). For patient outcome, discontinuation was 16 patients (41.03%), exeated was 15 patients (38.46%), Death was 7 patients (17.95%), transferred was 1 patient (2.56%). The department of origin was not mentioned for 20 patients (51.28%), the dominant department is the West or 11 patients (28.21%).

We present the case of aorto-iliac aneurysm in a patient with chronic renal failure requiring dialysis who were treated with an endovascular stent graft and, later on, submitted to kidney transplantation. A 53-year-old male with renal failure requiring dialysis presented with an asymptomatic abdominal aorto-iliac aneurysm measuring 5.0cm of diameter. He was treated with endovascular repair technique, being used an endoprosthesis Excluder®. After four months, he was successfully submitted to kidney transplantation (dead donor), with anastomosis of the graft renal artery in the external iliac artery distal to the endoprosthesis. The magnetic resonance imaging, carried out 30 days after the procedure, showed a good positioning of the endoprosthesis and adequate perfusion of the renal graft. In the follow-up, the patient presented improvement of nitrogenous waste, good positioning of the endoprosthesis without migration or endoleak. The endovascular repair of aorto-iliac aneurysm in a patient with end-stage renal failure under hemodialysis treatment showed to be feasible, safe and efficient, as it did not prevent the success of the posterior kidney transplantation.

Essential hypertension is high blood pressure that doesn't have any known etiopathology. Most of sufferers (85%) are asymptomatic and as per available reports, in more than 95% cases of hypertension under lying cause is not found. It is estimated that 600 million people are affected worldwide. Hypertension is a major risk factor for the development of cardiovascular diseases (CVD). Its impact is greatest on stroke, MI and end stage is renal failure as it’s known as a Silent Killer. Hence there is no direct reference of hypertension in Ayurvedic classics by name as well as by its path physiological views.
